    Exploring treatment strategies for mood stabilization in patients with major depressive disorder and mixed features

    We discuss strategies for mood stabilization in patients with major depressive disorder (MDD) and mixed features, challenging traditional paradigms and calling for a reevaluation of current diagnostic classifications. The DSM-5 "mixed features specifier" applies to several mood disorders, including MDD. In patients with MDD, this specifier blurs the traditional distinctions between depression and mania by introducing criteria that include symptoms such as elevated, expansive mood, inflated self-esteem, increased talkativeness, flight of ideas, increased energy, involvement in activities with potentially painful consequences, and decreased need for sleep. According to the DSM, when 3 of the above 7 symptoms are present during a major depressive episode, the episode may be characterized by the specifier "with mixed features". Despite the relatively high prevalence of mixed features in patients with MDD, no specific pharmacological treatments have been approved for these cases. We propose a treatment approach that includes: 1) initiating treatment with an antimanic agent (e.g., a mood stabilizer or a new-generation antipsychotic); 2) considering adjunctive low-dose serotonergic antidepressants; and then 3) considering discontinuation of the antimanic agent once the acute episode has resolved. We emphasize the lack of adequate studies of maintenance treatment with mood stabilizers and antipsychotics in patients with MDD and mixed features and the need for further research to improve our mood stabilization strategies for these patients

    Typical antipsychotics, also called conventional antipsychotics, first-generation antipsychotics or major tranquilizers, and tricyclic antidepressants, are drugs developed since 1950 and still used in the treatment of many psychiatric disorders, despite several compounds that were developed later, with the intention of improving their therapeutic response and tolerability. Although the efficacy, tolerability, value and usefulness of the new compounds are undeniable, none of these is so effective and tolerated to make us consider the older generation drugs as always and completely overtaken.     Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed.     Il tema dell’Age diversity assume oggi rilevanza primaria all’interno del dibattito più ampio relativo al Diversity Management, vale a dire all’interno delle politiche aziendali volte a gestire e valorizzare le diversità dei lavoratori. A rendere ancora più urgente il tema, oltre alla consapevolezza che è in atto un processo di invecchiamento della forza lavoro (come conseguenza diretta dell’invecchiamento della popolazione mondiale) è il nuovo e recente indirizzo normativo che ha sancito il prolungamento dell’età lavorativa, al di là di ogni aspettativa. Ma i dati presentati nel libro dimostrano che esiste solo una coorte di età in cui le aziende investono sugli individui: mediamente, solo se il lavoratore ha un’età compresa tra i 30 e i 45 anni le imprese mettono in atto politiche gestionali per ascoltare il talento, per farlo crescere e acquisire competenze. Chi lavora e ha meno di 30 o più di 45 anni sembra pagare un prezzo dal momento che lo sviluppo e la carriera nelle organizzazioni moderne sono fortemente influenzati da pregiudizi nei confronti dell’età. La sostenibilità delle aziende nell’immediato futuro sarà determinata da un nuovo modello in grado di trovare un punto di equilibrio tra la capacità di motivare e ingaggiare la forza lavoro più senior e la capacità di mantenere attivo il mercato delle assunzioni e i processi di sviluppo verso i target più giovani. E la revisione degli attuali meccanismi di gestione delle persone, tra cui il modello di carriera, è uno step fondamentale
