    The pediatrician and steroid resistance in ulcerative colitis

    Despite the availability of new immunomodulators, steroids still represent a milestone of Inflammatory Bowed Disease (IBD) therapy. Steroid resistance in IBD is an uncommon, yet existing and epidemiologically defined event involving one fifth of the patients with Crohn's disease and one sixth of the patients with ulcerative colitis (UC). We report of an 11 year old female patient with previously diagnosed UC that was hospitalized during a flare of the disease and that failed to respond to a four week intravenous and oral administration of steroids (2 mg/kg). The radioligand binding of the steroid receptors showed that a significantly higher dose of metilprednisolone was necessary to inhibit the growth of 50% of the lymphocytes (EC50) in our patient, if compared to the values obtained on 11 healthy controls (2,326 μM and 1,056 ± 53.14 μM respectively). Infliximab was administered and successfully induced. The epidemiology of steroid resistance and possible therapeutic approaches in steroid resistant patients are discussed

    Upper gastrointestinal involvement in paediatric onset Crohn's disease: Prevalence and clinical implications

    BACKGROUND AND AIMS: Our study evaluated the prevalence, the characteristics and implications of the upper gastrointestinal localisation (UGI+) in paediatric Crohn's Disease (CD) patients. METHODS: This prospective study evaluated 45 newly diagnosed CD patients at diagnosis and follow up with respect to CD localisation. RESULTS: All patients presented CD at the colon and/or ileum. In 24/45 patients (53.3%, 12 F and 12 M) an UGI+ involvement was also found. UGI+ patients had a younger age of onset (10.9 years versus 12.6 years; P<0.05). PCDAI at diagnosis was significantly higher in the UGI+ (41 vs. 25 P<0.01). UGI+ patients were overall more symptomatic. Pancolitis and extraintestinal manifestations were also more frequent (19/24 (80%) vs. 12/21 (57%) P<0.01). Growth was more impaired at diagnosis in UGI+ patients. By the end of the follow-up (mean 3 years, range 2 to 4) no significant difference was found in PCDAI (17 in UGI+ patients vs. 11 in UGI- P=NS), or the number of relapses. Weight and growth catch-up in UGI+ patients were comparable to UGI- ones. However, UGI+ patients required a more aggressive therapeutic approach. CONCLUSION: At least half of paediatric onset CD patients have an upper gastrointestinal localisation. UGI+ patients present an earlier onset and a more severe disease. The final outcome does not differ, but UGI+ patients require a more aggressive therapeutic approach

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed.     “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

    Appropriate Similarity Measures for Author Cocitation Analysis

    We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.     Dispelling the Myths Behind First-author Citation Counts

    We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more sophisticated methods

    Anglicisms in Italian. Types of language contact phenomena with particular references to word-formation processes

    Questo articolo si sofferma su diverse tipologie di fenomeni di contatto linguistico attraverso l'analisi di una selezione di anglicismi nell'italiano contemporaneo. Tali anglicismi permettono ad un tempo di fotografare gli aspetti del rinnovamento lessicale e strutturale dell’italiano sulla base della neologia esogena e di essere terreno di verifica delle tipologie dell’interlinguistica. L’attenzione si focalizza sulle interferenze morfologiche in particolare sull’induzione di nuovi morfemi, di nuove regole di ‘formazione delle parole’ e sull'influenza di questi processi sulla dinamica della ristandardizzazione dell'italiano, lingua ‘in movimento’. I casi di interferenza lessicale e morfo-sintattica in esame hanno favorito la diffusione e il successo di nuovi tipi di word-formation diventati frequenti inizialmente nelle lingue speciali e successivamente nella lingua comune contribuendo a caratterizzare l'italiano neostandard. Presenteremo casi in cui l'esito dell'interferenza risulta non solo in prestiti di nuove parole ma nell'induzione di morfemi e nella valorizzazione e rinforzo di tendenze linguistiche già presenti nella lingua come la composizione dotta, il blend tutti processi di formazione delle parole che hanno un impatto sulla struttura dell’italiano neostandar
