    Purpose: The treatment of middle turbinate (MT) during endoscopic sinus surgery (ESS) for chronic rhinosinusitis (CRS) remains a controversial issue. Several authors recommended preservation of MT to avoid complications, but MT resection is sometimes necessary. In the last years, our group has applied an original surgical technique to preserve MT, stabilize its head and ensure patency of the ostiomeatal complex (OMC) in ESS. We herein described this approach to the MT and reported its preliminary results in terms of post-operative endoscopic features and complications. Methods: The study retrospectively considered 34 consecutive adult patients suffering from CRS and treated with primary or revision ESS. Medialization of the MT and its stabilization to achieve post-operative patency of the OMC was obtained by means of: (i) creation of a surgically controlled synechia between the MT head and nasal septum; (ii) positioning of a silicone plate in the OMC. The silicone splints kept the MT head in an obliged position to contact the septum. Results: At 12-month follow-up control, we found: (i) stabilized MT medialization in all treated cases, (ii) only one case of lateral synechia without recurrent CRS, and (iii) a polypoid recurrence rate of 13%. Conclusions: The MT handling technique during ESS herein preliminarily described seems to be an effective approach to medialize and stabilize the MT. This technique could be particularly appropriate for unstable MT after removing massive inflammatory lesions of the mucosa

    Three-dimensional ultrasound in monitoring progression of labor: a reproducibility study

    Objective The aim of this study was to evaluate the intraobserver and interobserver reproducibility of measurements, obtained from stored ultrasound volumes, related to fetal head progression in the birth canal. Methods From January to September 2009, serial ultrasound volumes were obtained from women in labor, stored and separately evaluated by two different operators using the SonoVCAD labor software. Volumes were aligned using the pubic bone and the urethra as references. In the sagittal plane of the pelvis, the following data were calculated: head progression distance (the distance between the infrapubic line and the lowest part of the fetal skull), head direction (the angle between the infrapubic line and the major longitudinal axis of the fetal head) and head progression angle (the angle between the longitudinal axis of the pubic bone and a line joining the lowest edge of the pubis to the lowest convexity of the fetal skull). In the transverse plane, the angle formed by the midline with the anteroposterior axis of the maternal pelvis (midline angle) was also measured. For each measurement, intraobserver and interobserver reproducibility was determined. Results We analyzed 30 ultrasound volumes. For all parameters, interobserver variation was significantly higher than intraobserver variation. Reproducibility was good for all parameters, except for the midline angle. Among the different ultrasound measurements, the progression angle presented the highest reproducibility. Conclusions Measurements obtained from stored ultrasound volumes to assess fetal head progression in the second stage of labor have good reproducibility, with progression distance and progression angle being the most reproducible parameters

    The aim of the investigation is to evaluate the cognitive outcome of 40 patients with epilepsy who did not undergo any surgery procedures, 100 patients with epilepsy who underwent neurosurgery, and 50 controls, in a 5 years longitudinal study, in order to identify possible significant differences. We found a statistical significant difference related to age between S. Paolo patients (who did not undergo surgery) and Niguarda patients (who underwent surgery); neuropsychological results of Niguarda patients at the beginning of the research were globally better than those of S. Paolo patients. We noticed a statistical significant difference on all cognitive variables among the three groups, but when we corrected for age final results, we did not highlight statistical significant differences. This is a preliminary study for further analysis, which could deeply investigate the impact of epilepsy on cognitive profile over years

    Contro il paternalismo giuridico

    Sommario In questo articolo, si argomenterà che il paternalismo giuridico non è una concezione etico-politica condivisibile. In base a tale concezione, infatti, lo Stato può usare la forza, contro la volontà di un individuo adulto, anche qualora le sue scelte siano razionali e libere da coazione altrui, al fine di tutelare (ciò che si pretende sia) il suo bene. In particolare, si argomenterà che quello dell’autonomia individuale sia un argomento, in favore dell’antipaternalismo, più forte e condivisibile dei due principali argomenti in favore del paternalismo, quello utilitarista/organicista e quello perfezionista. Questi ultimi due argomenti, correttamente interpretati, sono, infatti, incompatibili con alcuni principi costituzionali adottati, o con principi morali condivisi da molte persone, in molti Stati occidentali (oltre che, ça va sans dire, dall’autore). Abstract In this article, it is argued that legal paternalism is a wrong conception. According to legal paternalism, the State can use coercion, against the will of an adult agent, even if his choice is rational and free, for promoting (what the State pretends to be) his good. In particular, it is argued that the argument of individual autonomy, in favor of legal antipaternalism, is stronger than two main arguments in favor of legal paternalism, the utilitarian and the perfectionist argument. Both last arguments are contrary to some constitutional principles adopted, and to some moral principles shared by many people, in many Occidental countries (as well as, ça va sans dire, by the author)
