Fractal time statistics of AE-index burst waiting times: evidence of metastability

    No full text
Recent observations and analyses evidenced that the magnetotail, as well as the magnetospheric dynamics are characterised by a scale-free behaviour and intermittence. These results, along with numerical simulations on cellular automata, suggest that the observed scale-invariance may be due to forced and/or self-organised criticality (FSOC), meaning that the magnetotail operates near a marginally stable state (Chang, 1999). On the other hand, it was underlined that a complex magnetic field topology in the geotail regions may play a relevant role in the impulsive energy relaxation (Consolini and Chang, 2001).Published419-4233.9. Michel Saint-Denis, dal Théâtre du Vieux-Colombier al London Theatre Studio. Premises to the Training

Behçet’s disease in children: Diagnostic and management challenges

Behçet’s Disease (BD) is an inflammatory disease of unknown etiology with multi-systemic involvement, being the main clinical manifestations represented by recurrent oral and genital ulcerations and uveitis. The disease has typically a chronic-relapsing course and may cause significant morbidity and mortality due to eye, vascular and neurological involvement. Although BD is more frequently diagnosed in adulthood, the disease onset can also be in pediatric age. Pediatric-onset BD is commonly featured by an incomplete clinical picture, and therefore the diagnosis represents a considerable clinical challenge for the physicians. The first classification criteria for pediatric BD, based on a scoring system, have been proposed few years ago. This work focuses on the main difficulties concerning both the diagnostic approach and the treatment of BD in pediatric age. The recommendation for the treatment of pediatric BD has been recently updated and allowed a considerable improvement of the therapeutic strategies. In particular, the use of anti-TNFα drugs as a second-line option for refractory BD, and as a first-line treatment in severe ocular and neurological involvement, has demonstrated to be effective in improving the outcome of BD patients. The knowledge about the molecular pathogenesis is progressively increasing, showing that BD shares common features with autoimmune and autoinflammatory disorders, and thus leading to the use of new biologic agents targeting the main mediators involved in the determination of BD. Anti-IL-17, anti-IL-23, anti-IL-1 and anti-IL-6 agents have shown promising results for the treatment of refractory BD in clinical trials and will represent an important alternative for the therapeutic approach to the disease

Rank ordering multifractal analysis of the auroral electrojet index

In the second half of the 90s interest grew on the complex features of the magnetospheric dynamics in response to solar wind changes. An important series of papers were published on the occurrence of chaos, turbulence and complexity. Among them, particularly interesting was the study of the bursty and fractal/multifractal character of the high latitude energy release during geomagnetic storms, which was evidenced by analyzing the features of the Auroral Electrojet (AE) indices. Recently, the multifractal features of the small time-scale increments of AE-indices have been criticized in favor of a more simple fractal behavior. This is particularly true for the scaling features of the probability density functions (PDFs) of the AE index increments. Here, after a brief review of the nature of the fractal/multifractal features of the magnetospheric response to solar wind changes, we investigate the multifractal nature of the scaling features of the AE index increments PDFs using the Rank Ordering Multifractal Analysis (ROMA) technique. The ROMA results clearly demonstrate the existence of a hierarchy of scaling indices, depending on the increment amplitude, for the data collapsing of PDFs relative to increments at different time scales. Our results confirm the previous results by Consolini et al. (1996) and the more recent results by Rypdal and Rypdal (2010)

Pediatric Systemic Lupus Erythematosus: Learning From Longer Follow Up to Adulthood

Background: Pediatric systemic lupus erythematosus (pSLE) is a rare condition, representing approximately 10% of SLE cases. The aim of this study was to identify variables to improve the diagnostic awareness and management of pSLE patients. Methods: This retrospective study included 25 patients diagnosed with pSLE and followed at the University of Pisa. We collected data about clinical profile at disease onset and during a long-term follow-up, including disease activity, organ damage development, and treatments received. Results: The mean patient age at disease onset was 14.6 ± 1.6 years, and the mean follow-up period was 14.17 ± 8.04 years. The most common initial manifestations were arthritis, malar rash, and cytopenias. The median time to diagnosis since the first symptoms was 6 months, and was significantly longer in patients with hematological onset (54 months). During follow-up, the number of patients with renal involvement showed a significant increase, from 36% at diagnosis to 72.2% after 10 years of disease evolution. Patients who developed chronic organ damage maintained a higher time-averaged disease activity during follow-up and received a significantly higher dose of corticosteroids. Conclusion: Patients with immune cytopenia represent a group deserving strict clinical follow-up for the risk of evolution to SLE. Intense surveillance of renal function, early treatment and steroid-sparing strategies should be strongly considered in the management of pSLE patients

Appropriate Similarity Measures for Author Cocitation Analysis

    No full text
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. Finally, we show by means of an example that our findings have a high practical relevance.information science;Pearson correlation;cosine;similarity measure;author cocitation analysis

Micro-channel flow boiling heat transfer of R-134a, R-236fa, and R-245fa

The rapid development of micro-thermal technologies has conveyed an increasing interest on convective boiling in micro-channels. Although there is general agreement that these systems may be able to dissipate potentially very high heat fluxes per unit volume, their heat transfer characteristics are still unclear and require investigation. The present study illustrates heat transfer data for flow boiling in a single micro-channel, for two channel diameters, namely, 510 and 790 mu m, three fluids, namely, R-134a, R-236fa and R-245fa, mass velocities from 300 to 2,000 kg/m(2) s, and heat fluxes up to 200 kW/m(2). Stable flow boiling heat transfer data are analyzed through a parametric investigation, and are also confronted with measurements in the presence of two-phase oscillatory instabilities, which were found to significantly change the trends with respect to vapor quality.LTCMNational Licence
