    Functional connectivity MRI and post-operative language performance in temporal lobe epilepsy: initial experience

    Anterior temporal lobectomy is an effective treatment for drug-resistant epilepsy of temporal origin, although new language impairment may develop after surgery. Since correlations between functional connectivity (FC) MRI of the language network and verbal-IQ performance before surgery have recently been reported, we investigated the existence of correlations between the preoperative FC of the language network and post-operative verbal-IQ decline. FC between nodes of the language network of the two hemispheres (Interhemispheric-FC) and within nodes of the left hemisphere (LH-FC) and language lateralization indexes were estimated in five right-handed patients with non-tumoral left temporal lobe epilepsy undergoing anterior temporal lobectomy. Correlations between preoperative FC measures and lateralization indexes, and the post-operative (12 months) neuropsychological verbal-IQ decline were investigated. Verbal-IQ decline was inversely correlated with the degree of left lateralization and directly correlated with the strength of Interhemispheric-FC. No significant correlation was found between LH-FC and post-operative verbal-IQ change. The results from this limited number of patients suggest that a stronger preoperative connectivity between homologue regions, associated with the absence of a definite hemispheric lateralization, appears to be an unfavorable prognostic biomarker

    Mucopolysaccharidoses (MPS) represent a group of inheritable lysosomal storage diseases caused by mutations in the genes coding for enzymes involved in catabolism of different glycosaminoglycans (GAGs). They are clinically heterogeneous multisystemic diseases, often involving the spine. Bony abnormalities of the spine included in the so-called dysostosis multiplex and GAG deposits in the dura mater and supporting ligaments can result in spinal cord compression, which can lead to compressive myelopathy. Spinal involvement is a major cause of morbidity and mortality in some MPS (e.g., MPS IVA, VI, and I), and early radiological diagnosis is critical in preventing or arresting neurological deterioration and loss of function

    The SAPHO syndrome revisited with an emphasis on spinal manifestations

    The synovitis, acne, pustulosis, hyperostosis, and osteitis (SAPHO) syndrome includes a group of chronic, relapsing, inflammatory musculoskeletal disorders with similar manifestations, in particular synovitis, hyperostosis, and osteitis, which may or may not be associated with neutrophilic skin eruptions such as palmoplantar pustulosis and acne conglobata. The syndrome occurs at any age, can involve any skeletal site, and its imaging appearances are variable, depending on the stage/age of the lesion and imaging method. The diagnosis is difficult if there is no skin disease. Awareness of the imaging appearances, especially in the spine, may help the radiologist in avoiding misdiagnosis (e.g., infection, tumor) and unnecessary invasive procedures, while facilitating early diagnosis and selection of an effective treatment. In this article, we provide an overview of the radiological appearances of SAPHO syndrome, focusing on the magnetic resonance imaging findings of vertebral involvement, and present relevant clinical and pathological features that assist early diagnosis

However, ES of cauda equina shares some neuroradiological features with other neoplasms that can involve the intradural space. Therefore, differential diagnosis with other tumors of cauda equina should be considered by neuroradiologists and neurosurgeons to provide appropriate treatment
