    Ferdinand von Schirach is a German criminal defense lawyer who has previously published two vivid and brilliant short story collections. His latest book, The Collini Case: A Novel, like his short stories, gives the reader telling details that offer insights into the human condition. But The Collini Case seems less interested in its characters than in teaching about the continuing stain of Germany’s past. This leads von Schirach to use stock figures who have suffered stock tragedies and who engage in stock actions. The novel is simply not realistic enough to suspend disbelief, and only barely avoids being a melodrama. Despite these criticisms, however, The Collini Case resonates because of its connections to Germany’s sordid history, reminding readers that, as Faulkner said, “The past is never dead. It’s not even past.” It continues to haunt the present

    USE OF HYPERBARIC PRILOCAINE IN AMBULATORY SURGERY F. Troisi, A. Ferrara, P. Merlin, F. Sacco, E. Finetti, S. Collini Anestesia e Ranimazione, Az Ospedaliera S Andrea, Roma, Italy Background and aims: Data on prilocaine use for ambulatory intrathecal anaesthesia are limited. In this randomized study we compared 2% hyperbaric prilocaine and sufentanil with 2% hyperbaric bupivacaine and sufentanil for spinal anesthesia in outpatients undergoing elective anorectal surgery without increasing side - effects or delaying. Methods: Patients ASA I-III were randomly allocated to receive 30mg hyperbaric prilocaine plus 5 microg sufentanyl (group PS,n=20) or 10 mg hyperbaric bupivacaine plus 5 microg sufentanyl (group BS, n=20) in sitting position. After sitting for 3 min (group PS) and 10 min (group BS), patients were positioned for surgery. Written informed consent was obtained from all subjects. Results: There were no significant differences in intraoperative outcomes (hypotension, bradycardia, respirator depression) but mean recovery, discharge time, postoperative pain and urinary retention were significantly shorter in group PS. Conclusions: This study demonstrates the superiority of the combination of low-dose hyperbaric prilocaine and sufenatnyl for spinal anaesthesia in ambulatory anorectal surgery. It is an excellent alternative to bupivacain because of faster attainment and resolution block, greater haemodynamic stability, earlier deambulation, reduced risk of urinary retention and early patients discharge.     Peribulbar anesthesia in sclero-retinal surgery: two quadrants vs single injection

    Purpose. To evaluate the advantage of peribulbar anesthesia in two quadrants (infero-temporal and upper nasal) vs single injection tedchnique (infero-temporal) for scleral rhegmatogenous detachment surgery (RRD). Patiets and methods. 57 patients, aged between 33 and 75 years (57.01 + - 8.68), waiting for retinal detachment surgery, were randomized in two gruops: a group S (29 patients), with single quadrant injection technique and a group C (28 patients) with two quadrants injection. Patients in Group S were injected in two quadrants with a 10 ml mixture containing Mepivacaine 2% 5ml and Ropivacaine 0.75% 5ml, with hyaluronidase 10UI/ml. Patients in group C were injected with the same mixture in one quadrant. Number of additional injections, during surgery, in two groups, were reported. Pain was tested with VAS (Verbal Analogue Scale 0-10) after regional block, when muscles were insulated, during scleral buckling positioning, when conjunctive was closed and 6, 12, 24 hours after surgery. Results. The need for a second peribulbar injection of anesthetic occurred in 9 patients of group C (32.14%); further injection was necessary in 1 patient (3.57%) of group S during scleral buckling (VAS=7), statistically significant p<0.05). Postoperative VAS 6-12-24 hours after surgery was not significant. Conclusions. The peribulbar anesthesia may be proposed in scleral surgery of retinal detachment; the injection in two quadrants secured solid and satisfectory anesthesia, increasing the outcome of retinal surgery in loco-regional anesthesia

    Mechanical stress and deformation analyses of pressurized cylindrical shells based on a higher-order modeling

    In this research, mechanical stress, static strain and deformation analyses of a cylindrical pressure vessel subjected to mechanical loads are presented. The kinematic relations are developed based on higher-order sinusoidal shear deformation theory. Thickness stretching formulation is accounted for more accurate analysis. The total transverse deflection is divided into bending, shear and thickness stretching parts in which the third term is responsible for change of deflection along the thickness direction. The axisymmetric formulations are derived through principle of virtual work. A parametric study is presented to investigate variation of stress and strain components along the thickness and longitudinal directions. To explore effect of thickness stretching model on the static results, a comparison between the present results with the available results of literature is presented.     The 48-Hour Protocol on Deaths Following Traumatic Events at the Hospital of Novara in Eastern Piedmont, Italy: A Review of the Effectiveness of Reporting to the Judicial Authority

    In the past decades, the autopsy rate has been declining worldwide, including in Italy, especially of judicial autopsies, partially losing sight of the pivotal importance of autopsy in detecting the cause and the manner of death.1-4 Indeed, discrepancies between clinical diagnosis and hospital autopsy are quite high.5,6 This study aims to review the effectiveness of the “48-hours protocol” in operation at the hospital “Maggiore della Carità” of Novara. This protocol states that after the death of a patient with a traumatic event in their past medical history, a notification of death must be sent to the Judicial Authority, which must be made aware that, after a time of 48-hours, a non-judicial autopsy is planned, giving the time to decide if the autopsy should be changed into a judicial one or not. The review was made on all cases in which the “48h protocol” came into effect, in the years 2017, 2020, and 2023. Out of the 127 medical death records, 116 were correctly reported as 48-hour protocols. A total of 107 autopsies were carried out, among which 103 were hospital ones, and only 4 were judicial ones. The most documented traumatic events were falls at home, followed by road traffic accidents. Over 46% of the autopsy results totally or partially differed from clinical diagnosis made in the hospital. The interpretation of the results of this study suggests three conclusions: first, the autopsy is pivotal to correctly detect the cause of death in traumatic deaths; second, the Judicial Authority does not make enough and proper use of the “48-hours protocol,” underestimating the role of judicial autopsies; and third, that the protocol might be too superficially used. Therefore, some changes to the protocol should be made in order to have a better communication to the Public Prosecutor’s Office, maybe adding more specific information to the reports sent to hopefully increase the authorities’ interest. Once these details have been refined, and the usefulness of this protocol validated, it would also be desirable to disseminate it to other prosecutors’ offices to raise awareness of the issue throughout Italy
