    Effect of the covid-19 pandemic on the demand for health services and the level of physical activity of hypertensive patients treated by Primary Health Care

    A doença respiratória que impactou o mundo em 2020 chamada Coronavírus (Covid-19) tomou grandes proporções e foi considerada pandemia. Para conter o vírus, adotaram-se medidas que afetaram diretamente a vida da população. O objetivo desta pesquisa foi avaliar o efeito do isolamento social durante esse período na procura por serviços de saúde e no nível de atividade física habitual de pessoas hipertensas. A amostra foi selecionada de forma randomizada e envolveu pacientes atendidos pela Atenção Primária à Saúde de Presidente Prudente (SP), selecionados igualmente dentre nove unidades de saúde. Prevalência de pacientes com Covid-19, atividade física, comportamento sedentário, presença de doenças crônicas não transmissíveis e características sociodemográficas foram verificadas por meio de questionários. Coletaram-se custos com saúde e pressão arterial mediante prontuários médicos. A amostra foi composta por 278 adultos hipertensos – 73 (26,7%) homens e 204 (73,3%) mulheres –, com mediana de idade de 64,0 (14,0) anos. Sobre mudanças no nível de atividade física, considerando-se o período de isolamento social e pós-isolamento, modificações significativas não foram encontradas (p = 0,182), mas o percentual de participantes considerados sedentários aumentou (p = 0,036). Custos totais e com consultas diminuíram durante o isolamento social e voltaram a se elevar no período pós-isolamento (tempo: p = 0,001). Pacientes que relataram essa prática apresentaram custos superiores para custo total e custos com consultas (grupo: p = 0,001 e tempo grupo: p = 0,001); custos com exames cresceram durante o isolamento e depois reduziram (tempo: p = 0,001), e custos com medicamentos aumentaram ao longo do acompanhamento (tempo: p = 0,001). Quanto ao nível de atividade física, no momento de isolamento social, não foram encontradas modificações nos custos com saúde, apenas aquelas relacionadas ao tempo de acompanhamento, em que custos com consultas e custos totais diminuíram no período de isolamento e depois voltaram a aumentar (p = 0,001), e aqueles vinculados a medicamentos aumentaram em todos os momentos considerados (p = 0,001). Concluiu-se que o nível de atividade física não evidenciou diferenças durante o acompanhamento, já o comportamento sedentário cresceu. Custos totais e custos com consultas diminuíram no isolamento social e voltaram a aumentar, e aqueles que envolvem medicamentos se elevaram durante o acompanhamento. Participantes que realizavam atividade física antes da pandemia apresentaram custos superiores com custos totais e consultas quando comparados aos que não a faziam; não houve diferenças para atividade física realizada no baseline.The respiratory disease that impacted the world in 2020, called Coronavirus (Covid-19), took on major proportions and was considered a pandemic. To contain the virus, measures were adopted that directly impacted the lives of the population. The objective of this research was to evaluate the impact of social isolation during the pandemic on the demand for health services and the level of habitual physical activity of hypertensive patients. The sample was selected randomly and involved patients treated by Primary Health Care in Presidente Prudente (SP), equally selected from nine health units. Prevalence of patients with Covid-19, physical activity, sedentary behavior, presence of chronic non-communicable diseases and sociodemographic characteristics were verified through questionnaires. Healthcare costs and blood pressure were obtained through medical records. The sample consisted of 278 hypertensive adults, 73 (26.7%) men and 204 (73.3%) women, with a median age of 64.0 (14.0) years. Regarding changes in the level of physical activity considering the period of social isolation and post-isolation, there were no significant changes (p = 0.182), however the percentage of participants considered sedentary increased (p = 0.036). Total costs and consultation costs decreased during social isolation and increased again in the post-isolation period (time: p = 0.001), participants who reported this practice had higher costs for total cost and consultation costs (group: p = 0.001 and time group: p = 0.001), exam costs increased during isolation and then reduced (time: p = 0.001) and medication costs increased throughout follow-up (time: p = 0.001). Considering the level of physical activity at the time of social isolation, no changes were found in health costs related to the level of physical activity, only related to the follow-up time, where the costs of consultations and total costs decreased during the period of isolation and then returned to increase (p = 0.001) and medication costs increased at all times considered (p = 0.001). It was concluded that the level of physical activity showed no differences during monitoring and sedentary behavior increased. Total costs and consultation costs decreased during social isolation and increased again, medication costs increased in all periods considered.     A inatividade física é apontada na literatura como um dos fatores responsáveis pelo aumento das doenças crônicas em todo mundo, entre elas a hipertensão arterial, levando ao aumento nos gastos com saúde pública. Dessa forma, a Atenção Primária à Saúde vem criando, políticas públicas com ações voltadas para a promoção de práticas mais saudáveis. Assim, o objetivo do presente estudo é comparar o nível de atividade física, comportamento sedentário e custos com saúde, de pacientes hipertensos cadastrados em diferentes modalidades de saúde na Atenção Primária do município de Presidente Prudente-SP. A amostra foi composta por pacientes hipertensos igualmente selecionados de forma randomizada dentre Unidades de Saúde da Família com o apoio do núcleo ampliado de saúde da família (USF+NASF), Unidades de Saúde da Família (USF) e Unidades Básicas de Saúde (UBS) do modelo tradicional. Os custos com serviços de saúde foram verificados por meio de informações registradas nos prontuários médicos de cada paciente ao longo de 24 meses. Nível de atividade física e comportamento sedentário foram avaliados através de questionários. Para análise estatística foram utilizados os testes Kruskal-Wallis, Mann-Whitney, Qui-quadrado e para regressão logística teste de Hosmer e Lemeshow descritiva por valores de mediana, DQ (quartil) com intervalos de confiança de 95% (IC95%). A significância estatística foi fixada em valores inferiores a 5% e as análises conduzidas no software estatístico Stata (versão 16.0). Foram avaliados 659 participantes, dos quais a mediana de idade da amostra foi de 64 anos (15), sendo 187 (28,4%) do sexo masculino e 474 (71,6%) do sexo feminino. Participantes cadastrados nas USF-NASF apresentaram maior idade e maior custos com saúde, entretanto foi a modalidade que apresentou menor ocorrência de pessoas inativas fisicamente e com comportamento sedentário. Conclui-se que os pacientes atendidos pela USF-NASF, apresentam maiores custos com saúde, contudo apresentaram maiores níveis de atividade física e menor tempo sentado.Physical inactivity is identified in the literature as one of the factors responsible for the increase in chronic diseases worldwide, including high blood pressure, leading to an increase in public health spending. Thus, the Primary Health Care, has been creating public policies with actions aimed at promoting healthier practices. Thus, the objective of the present studyis to compare the level of physical activity, sedentary behavior and health costs of hypertensive patients enrolled in different health modalities in Primary Care in the city of Presidente Prudente- SP. The sample consisted of hypertensive patients randomly selected from Family Health Units with the support of the expanded family health center (USF- NASF), Family Health Units (USF) and Basic Health Units (UBS), of the traditional model.Health service costs were verified using information recorded in each patient's medical records over a 24- month period. Level of physical activity and sedentary behavior were assessed using questionnaires. For statistical analysis, the Kruskal-Wallis, Mann-Whitney, Chi-square tests were used, and for logistic regression the Hosmer test and descriptive Lemeshow by median values, DQ (quartile) with 95% confidence intervals (95%CI).Statistical significance was set at values below 5% and analyzes were performed using Statastatistical software (version 16.0). The participants registered at the USF-NASF were older and had higher health expenses, but it was the modality that presented the lowest occurrence of physically inactiveand sedentary behavior. It is concluded that the patients assisted by the USF-NASF have higher healthcosts, however theyhave higher levels of physical activity and less sitting time

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed.     Impact of different primary care models on the health and health costs of hypertensive patients

    A Atenção Primaria à Saúde apresenta diversas modalidades de atendimento, entretanto pouco se sabe sobre o impacto desses diferentes modelos. Assim, o objetivo do estudo foi analisar como as diferentes modalidades de serviços, ofertadas pela AP, impactam na saúde e nos custos com saúde de pacientes hipertensos. A coleta de dados foi realizada entre os anos de 2020 e 2021. A amostra foi selecionada de forma randomizada dentre os pacientes atendidos pela AP de saúde de Presidente Prudente – SP, totalizando 659 participantes. Os participantes foram igualmente selecionados dentre as unidades de USF com NASF-AP, USF sem NASF-AP e UBS. Os custos com serviços de saúde foram verificados por meio de informações registradas nos prontuários médicos. Internações, nível de atividade física, obesidade, hipertensão arterial, presença de doenças crônicas não transmissíveis, risco cardiovascular e número de ocorrências clinicas foram avaliados através de questionários pré-estruturados. Para análise estatística foram utilizados os testes Kruskal-Wallis, Mann-Whitney e qui-quadrado. A significância estatística foi fixada em valores inferiores a 5% e as análises conduzidas no software estatístico Stata (versão 16.0). Foram avaliados 659 participantes, dos quais a mediana de idade da amostra foi de 64 anos (10,36), sendo 474 (71,6%) do sexo feminino. Participantes cadastrados nas USF com NASF-AP apresentaram maiores valores versus aos demais grupos, apresentando maior idade (mediana=65,00 (DI=12)), maior ocorrência de doenças, sendo que 40,5% dos diagnósticos de dislipidemia, 42,7% dos diagnósticos de diabetes e 38,9% dos casos de infarto estão localizados entre os pacientes atendidos pela USF com NASF-AP , além disso apresentaram os maiores custos com saúde em consultas (mediana=156,52 (DI=349,41)) e custos totais com serviços de saúde (mediana= 330,91 (DI=663,29)) nos anos de 2019/20, e o perfil se demonstrou semelhante no ano de 2020/21 onde os custos com consulta ainda se demonstram maiores (mediana= 82,39 (DI=113,26)) e os custos totais com serviços de saúde (mediana=240,82 (DI=251,63)). Conclui-se que os pacientes atendidos pelo USF com NASF-AP, contam maior abrangência à saúde possuem maiores custos com saúde, assim como apresentaram maior idade e ocorrência de doenças, pelo fato de que a amostra é majoritariamente feminina e pelo fato de as mulheres procurarem mais serviços de saúde preventivo quando comparado a população masculina, também pode -se atribuir ao fato de participarem de um atendimento multidisciplinar aumentando os custos com serviços de saúde.Primary Health Care offers several types of care, however little is known about the impact of these different models. Statistical significance was set at values below 5% and analyzes were performed using Stata statistical software (version 16.0). A total of 659 participants were evaluated, of which the median age of the sample was 64 years (10.36), 474 (71.6%) of whom were female. Participants registered at the USF with NASF-AP had higher values versus the other groups, with higher age (median=65.00 (DI=12)), higher occurrence of diseases, with 40.5% of dyslipidemia diagnoses, 42, 7% of the diagnoses of diabetes and 38.9% of the cases of infarction are located among patients attended by the USF with NASF-AP, in addition, they had the highest health costs in consultations (median=156.52 (DI=349.41 )) and total costs with health services (median= 330.91 (DI=663.29)) in the years 2019/20, and the profile was similar in the year 2020/21 where the costs with consultations are still shown higher (median=82.39 (DI=113.26)) and total costs with health services (median=240.82 (DI=251.63)).     Quality of sleep and health spending of patients attended by the Unified Health System: the role of physical activity

    A qualidade do sono tem sido apontada como importante variável associada à manutenção da saúde e redução de despesas com serviços de saúde. No entanto, pouco se sabe sobre a relação entre qualidade do sono, alterações metabólicas e efeito da prática de atividades físicas, bem como o impacto desses fatores para os custos com saúde no âmbito do Sistema Único de Saúde (SUS). Objetivos: Analisar a relação entre qualidade do sono e gasto com saúde de pacientes do SUS, bem como identificar se estas relações ocorrem independentemente da prática de atividades físicas e fatores associados. Métodos: A amostra foi composta por 168 adultos de ambos os sexos (52 homens [31%] e 116 mulheres [69%]) com média de idade de 63,8 ± 8,6 anos, atendidos por duas unidades básicas de saúde da cidade de Presidente Prudente/SP. A variável dependente analisada foi os gastos em saúde. Como variáveis independentes foram analisados: i) indicadores de adiposidade corporal: percentil de gordura, circunferência de cintura e índice de massa corporal; ii) variáveis hemodinâmicas: pressão arterial sistólica e diastólica, glicemia, colesterol total, lipoproteínas de alta e baixa densidade, e triglicérides; iii) qualidade do sono e iv) a prática de atividade física. Adicionalmente, foram consideradas as variáveis de ajuste: sexo, idade e condição econômica. As análises estatísticas foram realizadas com testes de Kruskal-Wallis, Mann-Whitney e Spearman. Resultados: Alterações nos padrões do sono foram positivamente correlacionados com gastos com medicamentos r= 0,213 [95%IC= 0,052- 0,363] e negativamente com gastos com exames r= -0,166 [95%IC= -0,313- -0,012] (p-valor= -0,152), mesmo após ajuste por fatores de confusão. Além disso, os gastos com serviços de saúde também foram correlacionados ao escore de atividade física (p-valor= 0,162), pressão arterial (p-valor= -0,164), e variáveis bioquímicas. Conclusão: A qualidade do sono está correlacionada com os gastos de cuidados em saúde no Sistema Único de Saúde, mesmo após ajuste do modelo por variáveis de confusão, com destaque para gastos com medicamentos e exames.Sleep quality has been identified as an important variable related to the maintenance of health and reduction of expenses with health services. Objectives: To analyze the relationship between sleep quality and health expenditure of UHS patients, as well as their activities related to the practice of associated physical and physical activities. Methods: The test was performed by 168 adults of both sexes (52 men [31%] and 116 women [69%]) with mean age of 63.8 ± 8.6 years, attended by two basic health units city of Presidente Prudente/SP. A dependent variable analyzed was health spending. The measures of confidence were: i) indicators of body adiposity: fat percentile, waist circumference and body mass index; ii) hemodynamic variables: systemic and diastolic arterial hypertension, glycemia, total cholesterol, high and low density lipoproteins, and triglycerides; iii) quality of sleep and iv) a practice of physical activity. Additionally, adjustment variables were found: gender, age and economic matrix. The analyzes consist of Kruskal-Wallis, MannWhitney and Spearman tests. Results: changes in the patterns were positively correlated with drug expenditures r = 0.213 [95% CI = 0.052-0.363] and with the negative shift with exams r = -0.166 [95% CI = -0.313-0.012] (p = value = -0.152), even if adjusted for confounding factors. In addition, health expenditures were correlated to the physical activity score (p-value = 0.162), arterial measure (p-value = -0.164), and biochemical variables.     “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

    Cost-utility analysis of the treatment of patients of the primary health network of Presidente Prudente-SP

    O alto índice de doenças crônicas têm se associado diretamente com elevados custos relacionados à saúde, influenciando diretamente na diminuição da qualidade de vida. Entretanto, estudos brasileiros que avaliam custo-utilidade através de medidas de qualidade de vida e recursos gastos com saúde, ainda são escassos na literatura. Dessa forma, o objetivo do estudo foi avaliar a razão custo-utilidade do tratamento de pacientes da rede primária de saúde de Presidente Prudente/SP. A amostra foi composta por 292 pacientes, de ambos os sexos, com idade igual ou superior a 50 anos. Os pacientes foram avaliados quanto: i) qualidade de vida; ii) QALY; iii) custos com saúde; iv) nível de atividade física; e v) uso de medicamentos. Para análise estatística, foram usados valores de média, desvio padrão, mediana, diferença entre quartil e valores percentuais para estatística descritiva, teste t para amostras independentes, correlação de Pearson para variáveis numéricas e o teste de qui-quadrado para variáveis categóricas. Para o custo-utilidade dos tratamentos, foram utilizados valores de QALY e o custo médio/ano para cada tratamento. A significância estatística (p-valor) foi pré-fixada em valores inferiores a 5%. O software utilizado foi o BioEstat (versão 5.0). Houve correlação negativa entre o domínio atividade física no lazer e locomoção com consultas e custos adicionais (p-valor<0,05), medicamentos e custos totais (p-valor<0,01), e atividade física habitual com consultas, custos adicionais e custos totais (p-valor<0,01). Consultas, custos adicionais, medicamentos e custo total foram maiores em pacientes que reportaram maior número de doenças (p-valor=0,001). O custo-utilidade do tratamento dos pacientes da atenção primária foi maior no grupo tratamento medicamentoso, seguida do grupo medicamentoso atrelada ao exercício físico que se apresentou custo-efetivo com média de QALY alto. Maiores escores de QV foi observado em homens quando comparados mulheres. Entretanto, variáveis como peso, IMC e presença de hipertensão arterial se associaram com menores escores para QV. Além disso, dados quanto internações e cirurgias foram analisados impactando a QV dos avaliados e por fim custos associados aos serviços de saúde também apresentaram relação com a QV.The high index of chronic diseases has been directly associated with high costs related to health, directly influencing the reduction of quality of life. However, Brazilian studies that evaluate cost-utility through measures of quality of life and resources spent on health are still scarce in the literature. Thus, the objective of the study was to evaluate the cost-utility ratio of the treatment of patients in the primary health network of Presidente Prudente/SP. The sample consisted of 292 patients, of both sexes, aged 50 years or more. Patients were assessed for: i) quality of life; ii) QALY; iii) health costs; iv) level of physical activity; and v) use of medications. For statistical analysis, mean values, standard deviation, median, difference between quartile and percentage values were used for descriptive statistics, t-test for independent samples, Pearson's correlation for numerical variables and the chi-square test for categorical variables. For the cost-utility of the treatments, values of QALY and the mean cost / year for each treatment were used. Statistical significance (p-value) was set at values below 5%. The software used was BioEstat (version 5.0). There was a negative correlation between the physical activity domain in leisure and locomotion with additional consultations and costs (p-value <0.05), medication and total costs (p-value <0.01), and habitual physical activity with consultations, additional costs and total costs (p-value <0.01). Consultations, additional costs, medications and total cost were higher in patients who reported higher number of diseases (p-value = 0.001). The cost-utility of the treatment of primary care patients was higher in the drug treatment group, followed by the drug group linked to physical exercise that was cost-effective with a high QALY mean. Higher QOL scores were observed in males when compared to females. However, variables such as weight, BMI and presence of arterial hypertension were associated with lower QOL scores.     Influência da prática de atividades físicas sobre os gastos com o tratamento ambulatorial de pacientes da rede pública de Bauru, São Paulo

    A ocorrência de doenças crônicas não transmissíveis (DCNT) tem aumentado de forma expressiva no mundo e, consequentemente, seu impacto nos gastos públicos. Existe consenso na literatura de que a prática de exercícios físicos contribui para o tratamento e prevenção de DCNT; entretanto não há dados substanciais que avaliem o seu impacto sobre os custos para a saúde pública. Objetivo: Avaliar a associação entre a prática de atividades físicas e os gastos com saúde de adultos atendidos pelo serviço de atenção básica. Metodologia: Foram avaliados 963 pacientes com idade ≥50 anos, de cinco unidades básicas de saúde da cidade de Bauru-SP. As variáveis analisadas foram: i) gastos com serviços de saúde obtidos através dos prontuários clínicos; ii) condição econômica, histórico de doenças, nível de atividade física atual e prévia, avaliados através de questionários; iii) medidas antropométricas (peso, altura e circunferência de cintura) e; iv) pressão arterial. Os procedimentos estatísticos utilizados foram: teste t de Student, teste qui-quadrado e regressão logística binária. Todos os procedimentos foram efetuados por software específico (SPSS 13.0) e o nível de significância adotado foi de <5%. Resultados: A idade média do grupo foi de 65±9 anos e dos pacientes avaliados, 73,4% (n= 707) eram do sexo feminino Encontraram-se taxas elevadas de doenças do aparelho circulatório (80,1%). Indicadores de adiposidade central e total associaram-se com maiores gastos totais (p= 0,001). Indivíduos fisicamente ativos no esporte (RC= 0.62 [0.40-0.97]) e no lazer (RC= 0.57 [0.37-0.87]) apresentaram chances reduzidas de estarem inseridos nos grupos de maior gasto total. O tracking de atividade física não apresentou efeito sobre os gastos com saúde. It is well documented that physical exercise contributes to the treatment and prevention of CD; however, it is not clear its burden on public health expenditures. Objective: To evaluate the association between physical activity practice and healthcare expenditures among adults by primary care service. Methodology: The sample was composed by 963 patients with age ≥50 years from five basic healthcare units in Bauru city, Brazil. The variables analyzed were: i) healthcare expenditures obtained from clinical records, ii) economic status, disease history, prior and current physical activity assessed by questionnaires; iii) anthropometric measurements (weight, height and waist circumference) and; iv) blood pressure. Statistical analysis was composed by: Student's t test, chi-square test and binary logistic regression. All procedures were performed through specific software (SPSS, release 13.0) and the significance level was set at <5%. Results: The overall mean age was 65±9 years and the sample was composed by 73.4% (n = 707) females. There were high rates of hypertensive diseases (80.1%). Central and general adiposity were associated with higher total expenditures (p=0,001). Individuals that were physically actives in both sports’ domain (OR = 0.62 [0.40-0.97]) and leisure time (OR = 0.57 [0.37-0.87]) had decreased likelihood to be inserted at groups of highest total costs. Tracking of physical activity had no effect on healthcare expenditures. Conclusion: In this age range, there was elevated occurrence of CD (mainly hypertensive diseases), which were more observed among sedentary and obese patients.     Appropriate Similarity Measures for Author Cocitation Analysis

    We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.
