    Ontogeny of adult B-cell acute lymphoblastic leukemia : characterization of the cell of origin and clonal hematopoiesis involvement

    La leucémie aiguë lymphoblastique B (LAL-B) représente le cancer le plus fréquent chez l'enfant bien que pouvant survenir à tout âge. Elle représente un modèle d'oncogenèse bien caractérisé et un paradigme de succès thérapeutique basé sur une optimisation progressive du traitement couplée à une stratification basée sur des facteurs pronostiques. Cependant, alors que le taux de guérison dépasse 90% chez l'enfant, son pronostic reste réservé chez l'adulte. Ce contraste s'explique en partie par la toxicité des traitements et les comorbidités qui augmentent avec l'âge. Une autre cause, complémentaire, tenant à la nature de la leucémie elle-même, nous a amené à formuler l'hypothèse de travail selon laquelle l'ontogénie des LAL-B de l'adulte se distinguerait de celle de l'enfant et déterminerait des caractéristiques génétiques, cliniques et pronostiques différentes. Nous avons centré notre étude sur les 3 entités de LAL-B les plus fréquentes chez l'adulte, définies par la présence d'un chromosome de Philadelphie (Ph+), d'une hypodiploïdie sévère (HoTr) ou d'un réarrangement de KMT2A (KMT2A-r). L'objectif de ce travail était de mieux comprendre la biologie de ces leucémies, leur lien avec le vieillissement et la réponse au traitement. Une attention particulière a été portée à la question de la cellule d'origine, aux altérations génomiques associées et à l'architecture clonale. Dans les LAL Ph+, nous avons révélé la persistance sous traitement de cellules BCR::ABL1-positives non leucémiques dans une proportion importante de patients. Cette population cellulaire répondant à la définition d'une hématopoïèse clonale témoignait de la survenue de l'évènement initiateur dans un progéniteur multipotent, remettant en question un dogme en rapprochant la LAL Ph+ de novo de la crise blastique de leucémie myéloïde chronique. Nous avons de plus montré que la maladie résiduelle mesurée sur la fusion BCR::ABL1 n'avait pas d'impact pronostique, contrairement à la réponse évaluée sur des marqueurs spécifiques des lymphoblastes, ceci conduisant à une transformation majeure de la pratique médicale concernant le suivi de la maladie et l'adaptation du traitement. Dans les LAL HoTr, après avoir décrit précisément l'épidémiologie, la composition chromosomique et le paysage mutationnel, nous avons analysé l'architecture clonale à l'aide d'une approche combinée de génotypage et d'immunophénotypage moléculaire à l'échelle unicellulaire. Ceci nous a permis d'identifier la mutation TP53 comme évènement initiateur, à l'origine d'une population pré-leucémique multi-lignée ou hématopoïèse clonale, pouvant persister au cours du traitement de la LAL. Enfin, dans les LAL-B KMT2A-r, nous avons décrit un paysage mutationnel particulièrement pauvre suggérant que le réarrangement de KMT2A peut être suffisant à la transformation oncogénique. L'analyse des réarrangements lymphoïdes suggère de plus que la transformation précède l'engagement lymphoïde. En accord avec ce modèle, nous avons montré que le réarrangement de KMT2A représentait un marqueur de MRD plus pertinent et fiable que les réarrangements lymphoïdes, souvent sous-clonaux. Nous avons également montré que les altérations additionnelles de TP53 et IKZF1 conféraient une plus grande résistance au traitement. L'ensemble nous a permis de proposer une nouvelle stratification du risque tenant compte de l'hétérogénéité biologique et clinique au sein de cette entité. Au total, nos travaux ont permis d'apporter de nouveaux éléments à la compréhension de l'oncogenèse des LAL-B de l'adulte, identifiant la cellule d'origine à un stade multipotent. Dans les LAL Ph+ ou HoTr, nos résultats indiquent que l'évènement initiateur de la leucémie donne naissance à une hématopoïèse clonale pré-leucémique, éclairant le lien observé avec le vieillissement. Ces résultats ont un impact majeur sur les pratiques cliniques quant à l'évaluation de la réponse au traitement, la stratification du risque et les choix thérapeutiques.B-cell acute lymphoblastic leukemia (B-ALL) is one of the most common cancers in children but can occur at any age. It is a well-characterized model of oncogenesis and a paradigm of successful cancer therapy, based on progressive treatment optimization combined with risk-adapted treatment stratification. However, while the cure rate in children now exceeds 90%, the prognosis remains poor in adults. This difference can be attributed partly to age-related treatment toxicity and comorbidities. In addition to this, our working hypothesis proposes that the ontogeny of adult B-ALL may differ from that of children and would determine distinct genetic, biological and clinical features. To explore this, we focused our study on the three main B-ALL entities in adults, defined by the presence of Philadelphia chromosome (Ph+), low hypodiploidy (LH), or a KMT2A rearrangement (KMT2A-r). The aim of this work was to better understand the biology of these leukemias, their link with aging and response to treatment. Specific attention was given to the questions of the cell of origin, cooperating genomic alterations, and clonal architecture. In Ph+ ALL, we revealed the persistence of BCR::ABL1-positive cells during treatment in the absence of leukemic blasts in a significant fraction of patients. These cells met the definition of clonal hematopoiesis with the BCR::ABL1 initiating event occurring in a multipotent progenitor, challenging the established dogma and drawing closer parallels between de novo Ph+ B-ALL and blast crisis of chronic myeloid leukemia. Importantly, BCR::ABL1 persistence was not predictive of poorer outcome, by contrast with minimal residual disease (MRD) assessed on lymphoblast-specific markers. This has led to a major shift in medical practice regarding disease monitoring and treatment adaptation. In LH ALL, after providing a precise characterization of epidemiology, chromosomal composition, and mutational landscape, we analyzed the clonal architecture using a multimodal approach of single cell genotyping and immunophenotyping. This allowed us to identify the TP53 mutation as the first hit of leukemogenesis, giving rise to a pre-leukemic multilineage population. This clonal hematopoiesis can persist during treatment, with the potential to evolve toward myeloid malignancy. In KMT2A-r B-ALL, we observed very few cooperating mutations, indicating that KMT2A rearrangement alone may be sufficient to drive oncogenic transformation. Furthermore, analysis of lymphoid rearrangements indicated that the KMT2A-r occurred before lymphoid commitment. In agreement with this model, we here demonstrated that the KMT2A rearrangement serves as a more reliable MRD marker than lymphoid rearrangements that are subclonal and unstable. Moreover, we observed that alterations involving TP53 and IKZF1, representing rare cooperating events in KMT2A-r B-ALL, conferred increased aggressiveness and a dismal prognosis. All of these findings allowed us to develop a new risk stratification algorithm that takes into account biological and clinical heterogeneity within KMT2A-r ALL. In summary, this research has provided new insights into the oncogenesis of adult B-ALL, identifying the cell of origin at an early multipotent stage. In both Ph+ and LH ALL, our results revealed that the initiating event led to clonal hematopoiesis as a predisposing condition to the development of B-ALL, shedding light on their association with aging. These findings will have a significant impact on clinical practices, particularly in terms of treatment response evaluation, risk stratification, and therapeutic choices

    Les LAL-B représentent le cancer le plus fréquent chez l'enfant, curable dans près de 90% des cas. Chez l'adulte, malgré une amélioration des traitements depuis une quinzaine d'années le pronostic des LAL-B reste médiocre avec des rechutes fréquentes et une survie d'environ 50%. La caractérisation des anomalies génomiques somatiques dans les cancers est non seulement à la base de la compréhension des mécanismes oncogéniques mais permet également de distinguer des groupes homogènes de patients d'un point de vue biologique et clinique, sur le plan de la réponse au traitement et finalement du pronostic. Dans les LAL-B de l'enfant plusieurs sous-types clinico-biologiques sont ainsi reconnus et c'est sur cette classification cytogénétique, ainsi que sur l'évaluation de la réponse précoce au traitement, que repose principalement la stratification thérapeutique des protocoles actuels dite basée sur le risque. Dans les LAL-B de l'adulte, seules quelques entités sont reconnues par les analyses courantes de cytogénétique et biologie moléculaire et plus de la moitié forme une catégorie par défaut dite « B-other ». Notre équipe a entrepris d'élucider sur le plan génomique et de caractériser les LAL B-other de l'adulte. Un séquençage complet du transcriptome de plus de 300 LAL B-other, sélectionnées au sein des patients traités dans les protocoles nationaux a été réalisé. L'analyse de ces données a mis en évidence des groupes caractérisés par des signatures d'expression distinctes, certains décrits ces dernières années (BCR::ABL1 like, DUX4/ERG, fusion de ZFN384) et deux autres jusqu'alors non rapportés. Ce travail de thèse a consisté à décrire ces deux nouveaux sous-types de LAL-B. Le premier est défini par une mutation ponctuelle du facteur de transcription PAX5, PAX5 P80R. Il représente 5% des LAL-B de l'adulte et est associé à un pronostic favorable. Le second, nommé CDX2/UBTF, est caractérisé par l'association unique de deux altérations génomiques, une délétion située sur le chromosome 13 (13q12.2) entrainant l'expression ectopique du gène CDX2 et une autre délétion sur le chromosome 17 (17q21) entrainant la génération d'un transcrit de fusion UBTF::ATXN7L3. Nous avons montré que la dérégulation ectopique du gène CDX2 résulte du détournement d'un enhancer actif dans le locus 13q12.2 au profit de CDX2. La LAL CDX2/UBTF est une entité rare qui touche surtout des femmes jeunes et est associée à un très haut risque de rechute. Ce travail a donc permis d'étendre notre connaissance des altérations génomiques et des oncogènes des LAL-B et de décrire deux nouvelles entités clinico-biologiques, reconnues par la dernière classification consensus internationale. Enfin nos travaux ouvrent la voie à de futures approches thérapeutiques de précision visant à identifier le meilleur traitement pour chaque patient.B-ALL is the most common type of cancer in children and about 90% of children can be cured. In adult, the prognosis of B-ALL is poor with frequent relapses and an overall survival rate of 50% despite a significant improvement over the last decades. The characterization of somatic genomic alterations in cancers is the cornerstone of the understanding of oncogenic mechanisms and also allows to discriminate homogeneous groups of patients with distinct biological and clinical presentation, response to treatment and prognosis. In pediatric B-ALL several oncogenic subtypes are recognized with a well-known prognosis. Cytogenetic classification has a long-standing role in risk prediction and treatment stratification. However, the adult B-ALL genomic landscape is complex and heterogeneous and still remains to be explored. Most of adult B-ALL lack defining cytogenetic alteration, preventing risk assignment and treatment stratification. We aimed to elucidate so-called `B-other ALL' from adult patients. We performed the whole transcriptome sequencing of more than 300 B-other ALL selected within patients treated in national trials. The analysis of these data highlighted group of cases characterized by specific gene expression profiles including subtypes recently described (BCR::ABL1 like, DUX4/ERG and ZFN384 fusion) and two other groups, suggestive of new leukemia genetic entities. My PhD work consisted in describing these two novel B-ALL entities. The first one is defined by a hotspot mutation of PAX5, PAX5 P80R. It represents 5% of B-ALL and is associated with a favorable outcome. The second one, named CDX2/UBTF, is characterized by the unique association of two separate underlying genetic features present in all cases, a focal genomic deletion at 13q12.2 resulting in the ectopic upregulation of the CDX2 gene and a focal 10kb genomic deletion at chromosome band 17q21 resulting in an in-frame UBTF::ATXN7L3 gene fusion. We described the mechanism leading to CDX2 ectopic expression which is the hijacking of an active enhancer in the 13q12.2 locus. The CDX2/UBTF B-ALL is a rare entity that occurs predominantly in young women and is associated with a very high risk of relapse. Overall, this work has extended our knowledge of genomic alterations and oncogenes in B-ALL. Besides, these two novel subtypes we characterized at biological and clinical levels are now recognized as B-ALL distinct entities by the International Consensus Classification of myeloid neoplasms and acute leukemias and new oncogenes in lymphoid disease. Finally, our work paves the way for future precision therapeutic approaches aimed at identifying the best treatment for each patient

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed.     “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

    Appropriate Similarity Measures for Author Cocitation Analysis

    We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.     Dispelling the Myths Behind First-author Citation Counts

    We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more sophisticated methods

    We have done our best to complete the author checklist relating to the use of animals in the hut study. Note that the objective for the hut study was to evaluate the IRS treatment applications for residual efficacy against Anopheles mosquitoes, including the local An. coluzzii mosquito population. Cows were only used to attract mosquitoes into the huts and no tests were carried out directly on the cows. The author checklist is intended for use with studies where experiments are carried out on animals, which is why we have had such difficulty in completing this for the hut study, as many of the questions do not relate to how the cows were used
