Who Really Needs a Rhinoplasty?
ven if the indication for rhinoplasty is to do with nasal function, the patient undergoing rhinoplasty usually still considers cosmetic aspects. Why patients agree to put themselves in discomfort, accept the risk, and shoulder the cost of a procedure, the main aim of which is an improvement in physical appearance, remains poorly understood. In the majority of cases, there is no direct correlation between the willingness to have the procedure and objective measures of nasal deformity. Likewise, psychometric measurements bear little relationship to how deformed the patient's appearance really is. For those cases where cosmetic considerations predominate, the degree of distress is greater than in those having the procedure solely to improve nasal function. Patients' satisfaction after rhinoplasty undertaken to address both functional and cosmetic needs depends more on aesthetic result than on improvement in function. Since patients undergoing rhinoplasty are often preoccupied with deformities that others would neither notice nor be concerned about, it shows clearly that this group has already undergone alterations in the way they think. Candidates for rhinoplasty are unhappy with their looks than those contemplating other cosmetic procedures, and each time they look in the mirror, they recall their dissatisfaction, a situation which has generally already begun at the age of puberty. In 80% of cases, the motivating factor is a wish for an alteration in facial appearance or the experience of seeing someone else benefit from rhinoplasty
Application of Computational Fluid Dynamics Methods to Understand Nasal Cavity Flows
Computational fluid dynamics methods enable to numerically predict complex flows with the help of computers. In the fields of Engineering and Physics they are already in use for decades to support design decissions and to get insight into complex physical phenomena. The simulation techniques have massively evolved over the past years and can nowadays be applied in medical context to analyze bio-fluidmechanical processes. Thanks to the continuous increase of computational power and parallelism as well as algorithmic advancements, accurate predictions of the flow in the nasal cavity are possible today. This chapter introduces the reader to the concepts of the computational fluid dynamics of the nose. It delivers some fundamentals on pre-processing medical image data, various techniques to generate computational meshes and gives an overview of methods to solve the governing equations of fluid motion. Thereby, advantages and disadvantages of the various approaches are explained. Subsequently, a variety of methods to analyze the flow and particle dynamics in the nasal cavity, ranging from streamline visualizations, pressure loss and temperature increase considerations, wall-shear stress and heat-flux distributions, to the analysis of the particle deposition behavior and transitional flow, is presented. The chapter concludes with how such methods can be used in clinical applications and elaborates how future developments might support decision making in medical pathways
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
Diabetes mellitusta kohleovestibuler fonksiyonların incelenmesi

Vücutta birçok sistemin yanında iç kulak fonksiyonlarını da etkilemektedir. Çalışmamız diabet ile işitme kaybı ve iç kulak fonksiyon bozukluklarının ilişkisini bir defa daha göstermek, yaş, cinsiyet, diabetin tipi ve süresi, insülin kullanımı, hipertansiyon, hiperkolesterolemi, hiperlipidemi, kan şekeri düzeyi ile ilişkisini, nefropati, retinopati ve nöropatı gibi komplikasyonlarla assosiasyonunu araştırmak amacıyla 50 diabetik olgu ve 50 kontrol olgusu üzerinde yapıldı. 21 erkek ve 29 kadından oluşan diabetli olguların yaş dağılımı 24 ile 67 arasında olup, yaş ortalaması 51.64 idi. Kontrol grubu daha sonra oluşturulduğundan diabetli grup ile aynı cinsiyet ve yaş dağılımı göstermekteydi. Olgularımız diabete mellitus, hipertansiyon ve kulak burun boğaz yönünden anamnez, fizik ve laboratuvar muayenesi, diapazon testleri, pure tone ve konuşma odyometresi, elektro akustik impedansmetrik muayene ve denge muayenesi ile incelendi. Bulgular t testi ile istatistiki olarak değerlendirildi. Çalışmamız sonucunda, diabetes mellituslu hastalarda hafif derecede, yavaş ilerleyen, simetrik ve daha çok yüksek frekansları tutan sensorioneural işitme kaybı tesbit edildi. Bu kayıp hastanın biolojik yaşı ve hastalığın süresi ile ilgiliydi. Bulgularımız klasik bilgiler, aynı konuda daha önce yapılan araştırmalar ile tartışıldı
Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.
