    Registration of patient-relevant references in edentulous jaws using intraoral scanners: simplification of the clinical procedure

    With the advancement of computer-aided design/computer-aided manufacturing (CAD/CAM) in dentistry, the use of digital technologies is being increasingly adopted by clinicians and laboratory technicians. Clinicians are using intraoral scanners (IOSs) more and more to perform computer-aided impressions. While these are mainly used to digitize dentate or partially edentulous jaws, it is feasible with a few of the available systems to scan edentulous jaws, with or without implants. To scan edentulous jaws, the registration and articulation of patient-relevant references and planes is difficult without the use of physical casts. The present article introduces simple yet effective methods to integrate individual interocclusal information during the intraoral scan procedure. The techniques described aim to simplify the digital registration procedure of edentulous jaws, with or without implants, and integrate patient-relevant information in the virtual articulator without the need to manufacture and articulate casts after the intraoral scan

    Artificial intelligence in dentistry: Current applications and future perspectives

    Artificial intelligence (Al) encompasses a broad spectrum of emerging technologies that continue to influence daily life.The evolution of Al makes the analysis of big data possible, which provides reliable information and improves the decision-making process. This article introduces the principles of Al and reviews the development of Al and how it is currently being used. Al technology has influenced the health care field because of the need for accurate diagnosis and superior patient care. In order to understand the trend of Al in dentistry, electronic searching was carried out, combined with approaching individual companies to obtain the details of Al-based services. The current applications of Al in clinical dentistry were introduced and summarized. In the future, the Al-based comprehensive care system is expected to establish high-quality patient care and innovative research and development, facilitating advanced decision support tools. The authors believe that an innovative in-ter-professional coordination among clinicians, researchers, and engineers will be the key to Al development in the field of dentistry. Despite the potential misinterpretations and the concern of patient privacy, Al will continue to connect with dentistry from a comprehensive perspective due to the need for precise treatment procedures and instant information exchange. Moreover, such developments will enable professionals to share health-related big data and deliver insights that improve patient care through hospitals, providers, researchers, and patients

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed

    白內障 (Cataract) 是隨著老化過程所產生的眼科疾病之一,其主要治療方式為手術,本研究之目的為探討白內障病患接受手術治療前後的視覺相關生活品質探討、病患焦慮及憂鬱與疲憊之相關性。研究採橫斷性描述相關性設計,以立意取樣 (purposive sampling) 使用結構式問卷進行資料收集,包括:「視覺功能生活品質量表」、「SF-12一般生活品質量表」、「醫院焦慮及憂鬱量表」、「簡易疲憊中文量表疲憊程度」、「基本資料表」,收案場所於台北某醫學中心之眼科病房及眼科門診,於97年5月至97年10月共收案78人,研究結果顯示一般生活品質、視覺功能生活品質、焦慮、憂鬱及疲憊於白內障手術後皆有顯著差異 (p<0.01);在視覺品質次項目中,與一般生理生活品質 (PCS) 相關的為心理健康、依賴、遠距離視力、整體健康、整體視力、角色限制 (p<0.01);與一般心理生活品質 (MCS) 相關的為心理健康、整體視力、依賴、整體健康 (p<0.01);接受手術後視覺生活品質的改變,與手術後焦慮、憂鬱及疲憊的改變呈正相關 (p<0.01);本研究有助於臨床護理人員對於白內障手術前後生活品質改變、情緒症狀監測(焦慮憂鬱)及疲憊相關問題更深入瞭解,並早期提供病患諮詢與衛教,可提供學術及臨床照護上具體參考依據。Cataract is one of the most common age-related ophthalmic diseases. The main treatment is surgery. The purposes of this study were to examine the changes of visual quality of life (QOL) and its relationship with the changes of general quality of life (physical and psychological function), depression, anxiety and fatigue status in cataract patients before and after surgery. A cross-sectional correlational study was applied to recruit subjects from a medical center in Northern Taiwan. We used a set of questionnaires: National Eye Institute Visual Functioning Questionnaire (NEI VFQ- 25), the Short-Form Health Survey (SF-12), the Hospital Anxiety and Depression Scale (HADS), the Brief Fatigue Inventory (BFI) and the general information, to assess variables listed in this study. A total of 78 subjects were recruited by purposive sampling. The results show a significant improvement in the general QOL, visual functioning QOL and a reduction in the degree of anxiety, depression and fatigue after the surgery (p<0.01). Within the subcategories of visual quality (NEI VFQ-25), general health, general vision, role limitation, distance vision, mental health, and dependency are associated with the Physical Component Summary (PCS) (p<0.01). In addition, general health, general vision, mental health and dependency are associated with the Mental Component Summary (MCS) (p<0.01). The change of the visual function before and after surgery are positively correlated with the changes of that of levels of anxiety, depression and fatigue (p<0.01). In conclusion, patients with cataract surgery have major improvement in their vision, visual related QOL and the general QOL.     Variations on the Author

    “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

    Use of a sequential two-in-one free arterialised venous flap for the simultaneous reconstruction of two separate defects on the foot

    A sequential free arterialised venous flap carrying two flaps nourished by a single venous pedicle was designed for the simultaneous reconstruction of two separate soft tissue defects on the dorso-medial aspect of the foot. These two defects were separated by approximately 3.5 cm, exposing the extensor hallucis longus tendon and first tarso-metatarsal joint. Superficial epidermolysis was noted postoperatively in the smaller flap. Nonetheless, flap survival was uneventful with no further surgery required, and the patient was discharged 7 days after surgery. Three months after the surgery, the patient was satisfied with the result and no morbidity was evident at the donor site. The sequential flap is not a new flap category, but we are unaware of any previous report of a free arterialised venous flap in the form of a sequential flap. We demonstrate the clinical value of this type of flap design for selected cases. The benefits and possible pitfalls of the sequential free arterialised venous flap are discussed
