Etude biomécanique d'un nouvel implant rachidien pour préserver la croissance et la mobilité dans le traitement des scolioses
Le "gold-standard" du traitement chirurgical des scolioses est l'arthrodèse, qui consiste, à l'aide d'une instrumentation adaptée, à corriger et redresser les déformations scoliotiques, puis fusionner les vertèbres du segment pathologique afin de consolider la correction réalisée. Cette fusion entraine la destruction de la biomécanique physiologique du rachis, en supprimant sa mobilité et sa croissance. Les travaux réalisés dans le cadre de cette thèse portent sur le développement et la validation d'un nouveau concept d'instrumentation rachidienne ayant pour objectifs de réduire voire d'arrêter l'évolution des déformations rachidiennes, en conservant croissance et mobilité. Ce nouveau dispositif a nécessité une étude biomécanique large, partant du concept nouveau de cet implant, passant par la mise au point d'une méthodologie expérimentale, la conception et la réalisation de prototypes, puis leur validation à travers des études numériques, mécaniques, tribologiques et in vivo sur gros animal. La caractérisation in vitro du dispositif porte sur des essais mécaniques de caractérisation de matériau et des essais tribologiques de caractérisation du frottement. La caractérisation in vivo consiste en deux études menées sur gros animal, le modèle de porc Landrace, une première sur l'étude de l'arrachement de vis pédiculaires, puis une seconde, de validation de concept, avec 2 mois d'implantation du montage. Les premières conclusions tirées de ces travaux sont positives quant au bon fonctionnement du système. Des études en cours et à venir permettront de compléter ces résultats, et de valider le système dans son ensemble, afin de permettre sa future mise sur le marché.The "gold standard" of surgical treatment of scoliosis is arthrodesis, which, with an appropriate instrumentation, corrects and straightens the deformities and fuses the vertebra of the pathologic segment to consolidate the correction. This fusion leads to the destruction of the physiological biomechanics of the spine, destroying growth and mobility. The work done in this thesis focuses on the development and validation of a new concept of spinal instrumentation which objectives are to reduce or even stop the development of spinal deformities, maintaining growth and mobility. This device is composed of materials used in new ways, leading to friction issues that do not exist in the current spinal systems. Thus, the system required a large biomechanical study, starting from the new concept of this implant, carrying on the development of an experimental methodology, designing and prototyping and then validation through numerical, mechanical, tribological and large animal in vivo studies. In vitro characterization of the device involves characterization of material through mechanical tests, and characterization of the tribological behavior of the system. In vivo characterization consists of two studies on large animal, the Landrace pig model : a first one on pedicle screws pullout, and a second one with 2 months of implantation, to validate the concept. The initial findings from this work are positive about the correct behavior of this system. Ongoing and future studies will complement those results, and validate the system as a whole, to allow future marketing
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Mechanicals and numericals contributions for the development of individualised bone/implant models : application to the optimisation of total knee replacement
La gonarthrose est une maladie handicapante qui touche l’ensemble des structures articulaires du genou. Elle est favorisée par un défaut d’alignement du genou sur l’axe mécanique du membre inférieur, ce défaut cause un déséquilibre de la répartition des charges entre la partie médiale et la partie latérale de l’articulation. Lorsque la gonarthrose est trop avancée, une opération chirurgicale, l’arthroplastie totale de genou (ATG), permet le remplacement de l’articulation native par une prothèse totale de genou (PTG). Il n’y a pas de consensus clinique sur le meilleur positionnement des implants de la PTG, les principales causes d’échecs sont le descellement aseptique de l’implant tibial et les douleurs inexpliquées. Ces causes s'expliquent en partie par la perturbation du chargement mécanique de l’os induite par l’implantation d’une prothèse. Ce travail propose une méthode pour déterminer un positionnement des implants spécifique au patient pour limiter ces risques d’échec. Des méthodes automatiques, opérateur-indépendantes, d’association de repères anatomiques aux os du genou ont été développées et évaluées, elles permettent une implantation virtuelle automatique des implants. Nous avons montré que le défaut d’alignement conditionne les propriétés micromécaniques et micro-architecturales de l’os situé sous l’implant. Des modèles éléments-finis patients spécifiques ont été automatiquement construis. Pour chaque patient plusieurs stratégies chirurgicales de positionnement des implants ont été simulées et ont montré que le positionnement et les propriétés mécaniques de l’os trabéculaire influencent les phénomènes à l’origine d’une partie des échecs de PTG.Gonarthrosis is a widespread disabling disease which affects all joint structures of the knee. It is worsened by a non-alignment of the knee on the mechanical axis of the lower limb, this malalignment causes an imbalance in the distribution of loads between the medial and lateral part of the joint. When the gonarthrosis is too advanced, Total Knee Arthroplasty (TKA), allows the replacement of the native joint with a Total Knee Replacement (TKR) prosthesis. There is no clinical consensus on the best positioning of TKR implants, the main causes of failure are aseptic loosening of the tibial implant and unexplained pain. These causes are partly explained by the disruption of the mechanical loading of the bone induced by the implantation of a prosthesis. This work proposes a method to determine a patient-specific implant positioning to limit these risks of failure. During this thesis, operator-independent methods were developed and evaluated to automatically construct anatomical coordinate system on the knee bones, allowing automatic and parametric virtual implantation of implants. Using microindentation and µ-CT imaging, we showed that malalignment affects the micromechanical and micro-architectural properties of the bone under the implant. Patient-specific finite-element models were automatically built. For each patient, several surgical strategies for implant positioning were simulated. These simulations have shown that the positioning and mechanicals properties of trabecular bone influence the phenomena which cause a large part of TKR failure. The work carried out led to a numerical and mechanical method for optimizing the positioning of each patient's tibial implant
Cone-Beam weight bearing lower limb imaging : development of multidimensional biometric systems
Développement de systèmes biométriques multidimensionnels dans l'imagerie des membres inférieurs en chargeL'imagerie en charge tridimensionnelle à partir de la tomographie à faisceau conique devient la norme en imagerie squelettique, en particulier du pied et de la cheville. Cela entraîne un besoin de changement dans les systèmes de mesure, passant d'une base 2D à une base 3D. L'avènement du référentiel 3D permet de mettre en œuvre de nouveaux outils biométriques. La mise en œuvre de logiciels de segmentation automatique permet dans un premier temps de séparer les structures osseuses dont les différentes caractéristiques, position et orientation peuvent ensuite être analysées.La cartographie articulaire 3D permet de représenter les distances points par point entre les structures osseuses, ainsi que la qualité de la couverture articulaire, mettant en évidence un excès (dans le cadre de l'arthrose), ou une insuffisance de contact (instabilité ou luxation). La technique a fait l'objet d'une publication évaluant son efficacité dans le cadre des déformations en pied creux varus.Les mesures automatiques, intéressantes en pratique clinique dans le cadre de pathologies fréquentes comme l'Hallux Valgus, ont également fait l'objet d'une publication. Elles nécessitent une évaluation préalable à leur diffusion pour s'assurer de leur pertinence et de leur précision par rapport aux mesures manuelles.L'analyse de la densité osseuse, thème d'une troisième publication, représente la quatrième dimension de cette imagerie de nouvelle génération. L'étude en question évalue la distribution topographique de la densité osseuse en rapport avec l'alignement de la cheville, un paramètre fondamental dans la planification chirurgicale lors de la correction des troubles architecturaux par ostéotomie ou prothèse.Nous démontrons comment ces outils peuvent grandement améliorer les capacités diagnostiques, tout en accélérant le parcours des patients et en aidant à construire des bases de données pour former des systèmes d'intelligence artificielle pour le diagnostic, le pronostic et la planification chirurgicale.Development of multidimensional biometric systems in weight bearing lower limb imagingThree-dimensional weight-bearing imaging from cone-beam tomography is becoming the standard in skeletal imaging, particularly for the foot and ankle. This requires a shift in measurement systems, moving from a 2D to a 3D reference frame. The advent of the 3D framework allows for the implementation of new biometric tools. The deployment of automatic segmentation software initially allows for the separation of bone structures of which various characteristics, position, and orientation can then be analyzed. As part of the present thesis, we carried out three scientific investigations of new biometric tools.Three-dimensional joint mapping allows for the representation of point-by-point distances between bone structures, as well as the quality of joint coverage, highlighting an excess (as in the case of arthritis) or a lack of contact (instability or dislocation). The technique has been the subject of a scientific publication evaluating its effectiveness in the context of cavovarus foot deformities.Automatic measurements, useful in clinical practice for common pathologies like Hallux Valgus. They require prior evaluation before they can be used un daily clinical practice to ensure their relevance and accuracy compared to manual measurements.Bone mineral density was investigated in a third publication, represents the fourth dimension of this new generation imaging. This study assesses the topographical distribution of bone density in relation to the alignment of the ankle, a fundamental parameter in surgical planning for the correction of architectural disorders by osteotomy or prosthesis.We demonstrate how these tools can greatly enhance diagnostic capabilities, while speeding up patient workflows and helping to build databases to train artificial intelligence systems for diagnosis, prognosis, and surgical planning
Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. Theoric study on biomechanical behavior of thumb joint
L’arthrose de l’articulation trapézométacarpienne (ATM), plus communément appelée rhizarthrose, touche essentiellement les personnes âgées et les femmes ménopausées. Cette pathologie douloureuse évolue progressivement vers l’invalidité : avec une perte progressive de la force et des amplitudes du mouvement. Les prothèses totales actuelles sont des options chirurgicales débouchant souvent sur des complications précoces chez les patients jeunes et actifs. Face à ce problème, nous avons décidé d’étudier l’anatomie et la cinématique de l’articulation pour trouver des alternatives à ce problème, et tenter d’augmenter leur espérance de vie.En ces termes :• Une étude cinématique a été menée pour mieux comprendre les mouvements du pouce.• Une étude histologique a permis de mieux caractériser l’anatomie de l’ATM.• Une étude tribologique a permis d’étudier le frottement du cartilage contre une céramique.Toutes ont été menées pour proposer une alternative aux prothèses actuelles.Trapeziometacarpal joint arthritis, called rhizarthrosis, is prevalent in elderly persons, especially in menopausal women. This painful pathology is changing progressively to disability with loss of strength and range of movement. The current total prostheses are a surgical option with most of the time earlier complications in active and young patient. In front of this problem we decided to study the anatomy and the kinematic of this joint, in order to find an alternative to this problem and increase the lifespan of the prostheses. In consequence:• An kinematic study have been realized to better understand the movement of the thumb• An histological study allowed to better characterized the ligament structure of the joint• An tribological study have been performed to characterized the wear of the cartilage against a ceramicAll these studies have been realized to propose an alternative to the current total prostheses
Lower limb 3D imaging in weightbearing position : knee, foot, and ankle 3D biometrics : preoperative assessment
L’imagerie médicale en 3 dimensions (scanner, IRM) a permis d’améliorer la compréhension des pathologies et la planification des actions de prise en charge de ces pathologies. Dans la planification des opérations de chirurgie orthopédique, l’utilisation des images scanner permet la mise en place d’instrumentation patient-spécifique (PS). Cette instrumentation guide le chirurgien dans sa pratique et assure au patient une conformité des résultats postopératoires avec les prévisions. Cependant, par rapport aux standards d’imagerie radiologique en 2D (Rayon X), la 3D apporte des informations supplémentaires. Ces informations amènent à repenser les pratiques et à créer de nouveaux standards entièrement en 3D. Il faut disposer de systèmes de coordonnées (SdC) et de méthodes de mesures spécifiques à la fois à la 3D et à la partie anatomique considérée. Pour être en mesure de planifier une opération chirurgicale en orthopédie, il faut comprendre les phénomènes biomécaniques mis en jeu, les interactions entre les structures osseuses et les tissus mous. Les travaux présentés dans ce manuscrit s’articulent autour de ces 2 enjeux. J’ai d'abord cherché à comprendre les relations entre les déformations du membre inférieur et les déformations du pied. Le but étant de savoir si, lorsqu’une planification est faite pour une opération du genou, il est nécessaire de prendre en compte les conséquences qu’elle pourrait avoir sur le pied, et inversement. Ensuite, j’ai défini un SdC PS et des mesures de déformation pour le tibia distal en 3D, mis en place semi-automatiquement grâce à un algorithme. Une étape préliminaire à la mise en place d’un SdC pour le pied est également présentée.Three-dimensional medical imaging (CT,MRI) has allowed to improve the understanding of pathologies and their management. In the surgical planning in orthopedics, the use of scanner images allows the implementationof patient-specific (PS) instrumentation. This instrumentation guides the surgeon in his practice and ensures that the postoperative results are conform with the forecasts.However, compared to the standards of radiological imaging in 2D (X-ray), 3D provides additional information. This information leads to rethink practices and create new standars entirely in 3D. It is necessary to have coordinate systems and measurement methods specific to both the 3D and the anatomical part considered. To be able to plan an orthopaedic surgery, it is necessary to understand the biomechanical phenomena involved, the interactions between bone structures and soft tissues.The work presented in this manuscript revolves around these 2 issues. First, I sought to understand the relationships between lower limb and foot deformities. The goal is to know whether, when planning for a knee operation, it is necessary to take into account the consequences it could have on the foot, and vice versa. Then, I defined a PS coordinate system (CS) and deformationmeasurements for the distal tibia in 3D, set up semi-automatically thanks to an algorithm. A preliminary step to the implementation of a CS for the foot is also presented
Altérations de la structure osseuse de l'extrémité proximale du fémur : Analyse en imagerie médicale, étude biomécanique, et application à la prédiction du risque fracturaire.
La fracture ostéoporotique du col fémoral et la coxarthrose constituent aujourd'hui deux enjeux majeurs de santé publique chez les sujets âgés. L'ostéoporose est définie par une diminution de la masse osseuse et une altération de la structure osseuse. La coxarthrose d'autre part associe la perte progressive du cartilage d'encroûtement articulaire, une sclérose osseuse sous-chondrale, des géodes sous-chondrales, et des ostéophytes marginaux. Bien qu'une masse osseuse élevée semble prédisposer à l'arthrose, les relations métaboliques et biomécaniques entre ostéoporose et coxarthrose sont complexes. L'évaluation de l'architecture osseuse de l'extrémité proximale du fémur est un des enjeux majeurs de recherche actuels visant à la caractérisation des modifications du tissu osseux liées au vieillissement. Notre travail de thèse s'est inscrit dans un projet pluridisciplinaire de recherche sur les altérations de la structure osseuse de l'extrémité proximale du fémur, reposant sur une analyse en imagerie médicale et une étude biomécanique de cette région anatomique fondamentale.Fracture of the proximal femur and hip osteoarthritis are nowadays a major public health problem in elderly persons. The current definition of osteoporosis is a low bone mass associated with microarchitecture deterioration. On the other hand, osteoarthritis corresponds to progressive articular cartilage loss, subchondral bone sclerosis, subchondral bone cysts, and marginal osteophytes. Although a higher bone mass may increase the risk of osteoarthritis, osteoporosis and hip osteoarthritis present a complex metabolic and biomechanical relationship. The proximal femur architectural evaluation and characterization of age-related osseous alterations are currently one of the main challenges in bone and mineral research. Our work was based on a multidisciplinary project which aimed at evaluating the age-related structural deterioration of the proximal femur using medical imaging and biomechanical testing in this crucial anatomical region
