Unexpected deaths in medical wards during night-shifts: a narrative analysis of nursing experiences.

ntroduction. In Italy, children's nursing care is provided also by general nurses who previously worked in adult settings. Knowing the nurses' role transition experiences may help Health-Care Organizations to optimize their human resources management and improve the quality of care. Methods. A phenomenological study was conducted. Eighteen nurses were enrolled with a snowball sampling. Results. Participants' workplace demotivation and physical limitations were the main triggers of the role transition from adult care to children care. During this process nurses have experienced conflicting emotions. Their inexperience in the pediatric field and the perception of a limited utility of their skills made their integration into the new teamwork difficult. Generally, the role transition occurred in an unstructured way. Discussion. Children's nursing care requires specific skills. The results of this study encourage Health-Care Organizations to increase the level of attention on transition role in pediatric settings. Conclusions. The transition role from the adult to pediatric nursing care should be adequately governed also through formal mentoring programs. Future research should be directed to documentation of the association between transition process and nursing care outcomes

Indications for CAR-T cell therapy and management of the complications: an update. Introduction. The engineering of T lymphocytes with the chimeric antigen receptor (CAR-T) opened a new pattern for the treatment of malignant neoplasms and it was pivotal for the treatment of some haematological malignancies. Aim. To describe the therapy with CAR-T, the mechanism, the management process, the role of the multidisciplinary team and high-light the main complications and management, follow-up, the impact on quality of life and the role of nurse. Method. A literature review was conducted. Secondary studies published between 1 January and 17 October 2022, in English and Italian language, on adult population undergoing CAR-T, were included. Of the 335 articles, 64 were, finally, in-cluded. Results. New CAR-T products have been tested for the treatment of acute myeloid leukaemia, multiple myelo-ma and some kind of solid tumours. The two main toxici-ties are the cytokine release syndrome and the neurotoxi-city. Alternative drugs have been tested for minor adverse effects. The multidisciplinary team and the nurse are fun-damental, both in the clinical care and in the organization; an emphasis was put on the correct patients' information. Quality of life after CAR-T treatment is still poorly inves-tigated. Conclusions. The knowledge on CAR-T is in continuous and rapid growth and several questions are still without answer, requiring a continuous update of the transplant centres
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
STRESS E IMPLICAZIONI DI SALUTE NEGLI INFERMIERI ASSOCIATE ALLA TURNISTICA
OBIETTIVI: In letteratura è stato dimostrato che il lavoro notturno o il lavoro con turni negli infermieri è
spesso associato a ripercussioni sulla salute psico-fisica e sulle relazioni sociali. Lo scopo di questo studio è di
valutare se vi sia un’associazione tra le tipologie di turnistica e lo stress lavoro correlato, la soddisfazione sul
lavoro e la salute percepita.
METO DI: Lo studio è stato condotto durante il periodo Giugno-Settembre 2011 presso un’Azienda Ospedaliera
Universitaria. È stato somministrato agli infermieri, un questionario, derivato principalmente dagli studi
di Sveinsdottir, composto da nove sezioni comprendenti le caratteristiche socio-demografiche e professionali, le
abitudini relative al sonno, la percezione della salute, la soddisfazione sul lavoro, gli stili di vita, l’uso di farmaci,
i giorni di assenza per malattia, l’accesso alle cure e l’ambiente fisico percepito. Inoltre, sono state inserite
24 domande derivate dal “The Job Content Questionnaire” (JCQ) di Karasek. Le analisi statistiche sono state
effettuate utilizzando Excel di Microsoft Office 2003 e STATA versione 12 . È stata eseguita un’analisi logistica
multi-level utilizzando come variabili dipendenti il terzo terzile degli score di ogni outcome e di ogni dimensione
del questionario JCQ: decision latitude = 72 (skill discretion= 37, decision authority = 35) e job demand = 38
(psychological demand = 25, physical demand = 15).
RISULTATI : Il questionario è stato completato da 455 infermieri (56,5%) del campione. Il turno di lavoro
prevalente era quello “night-shift” (mattino/pomeriggio/notte) 300 infermieri (68,2%), seguito dal turno “shiftnot-
night“ (mattino/ pomeriggio alternati) 94 infermieri (21,4%) e dal turno “not-shift”(solo mattino) 46 infermieri
(10,4%). È stata evidenziata un’associazione tra la turnistica e le seguenti dimensioni: il “night-shift” risulta
associato ad un basso livello di decision authority e un’alta richiesta di job demand rispetto al “not-shift”; sia il
“night-shift sia il “shift-not-night“ risultano associati a un maggiore job strain rispetto al “not-shift”. Inoltre, sia
il “night-shift sia “shift-not-night“ risultano associati a un maggiore stato di esaurimento ed di stanchezza rispetto
al “not-shift”. Lo studio non ha identificato differenze tra la turnistica e il numero di visite dal medico, le assenze
per malattia. Infine, il tipo di turno non modifica la soddisfazione sul lavoro.
CONCLUSIONI: I risultati del presente studio devono essere considerati dalla direzione infermieristica, per
poter mitigare l’eccesso di stress lavoro correlato associato ai turni di lavoro che potrebbe causare il born-out di
questi lavoratori
Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. Dispelling the Myths Behind First-author Citation Counts
We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued
use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation
counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more
sophisticated methods
