Malignant melanoma Clustering with some familial/hereditary tumors
No full textGermline mutations of the CDKN2 and CDK4 genes that are involved in the cell cycle regulation, have been associated with malignant melanoma (MM). However, these mutations have been detected only in some cases of familial MM, particularly in Sweden and in Italy, which suggests that other genes are involved in the pathogenesis of hereditary MM. Few data are available about the clustering between MM and familial or hereditary tumours (e.g. breast cancer, ovarian cancer, colon cancer, pancreatic cancer). In this respect, the Breast Cancer Linkage Consortium reported that carriers of germline mutations in the BRCA2 predisposing gene have a significantly increased risk for MM (RR = 2.58; 95% CI = 1.28-5.17). The aim of the present study was to determine the family history of 342 consecutive cases of MM observed from 1994 to date. Specifically, we looked for: MM associated with other malignant neoplasias; familial clustering of MM; and a family history of cancer. Malignant melanoma was associated with other tumours in 20/342 patients (5.84%); a history of MM was associated to a positive family history of cancer in 31/342 cases (9.06%); and familial clustering of MM was detected in 22/342 (6.4%) of the consecutive series of patients analysed. CALCIFICAZIONI CORONARICHE IN PAZIENTI DIABETICI CON INSUFFICIENZA RENALE CRONICA
No full textIntroduzione. La malattia cardiovascolare rappresenta una delle principali cause di morte nei pazienti con insufficienza renale cronica (IRC). Nei pazienti in dialisi (ESRD-ptz) esiste una stretta associazione tra calcificazioni vascolari (presenti in 80% dei soggetti) e rischio cardiovascolare. Tale associazione è maggiormente evidente in ESRD-ptz da diabete nei quali le CAC progrediscono ancor più rapidamente. Recentemente CAC sono state evidenziate anche in stati precoci dell’IRC. Non ci sono dati relativi a diabetici con IRC in pre-dialisi (CKD-ptz). Scopi. In questo studio è stato verificato se i disordini del metabolismo minerale contribuiscono alla prevalenza ed alla severità delle CAC nei pazienti diabetici. Metodi. Pazienti ambulatoriali di età compresa tra 18-70 anni con funzione renale normale e CKD-ptz erano arruolati nello studio. Criteri di esclusione erano: malattia coronarica sintomatica, aritmia, pregresso infarto del miocardio, angioplastica o by-pass coronario, stroke, malattia renale progressiva. In ogni paziente erano raccolti i fattori di rischio “tradizionali” e “non tradizionali”. Calcemia, fosforemia, PTH, omocisteina, proteina C-reattiva, trigliceridi, colesterolo totale, HDL, LDL erano misurati all’ingresso nello studio. Lo score del calcio (TCS) veniva misurato mediante TC spirale. I dati dei ptz con e senza diabete (indipendentemente dal grado di CKD) erano messi a confronto. Risultati. Storia di ipercolesterolemia e/o ipetrigliceridemia era riportata da tutti i ptz diabetici e dal 75% dei non diabetici. Ipertensione arteriosa era presente nell’81% di entrambi i gruppi. Eventi cardiovascolari familiari erano riportati nel 49% dei diabetici e 45% dei non diabetici. Dati ulteriori dello studio sono riportati nella tabell

La Farfalla di Dinard (1952) è analizzata nella prospettiva di un racconto giallo, incentrato
sulla «farfallina color zafferano» (che viene qui identificata nella Colias crocea,
Fourcroy, 1785, detta in Francia “Le Souci”) prossima a morire in Bretagna e latrice
di un «messaggio segreto» per Montale da parte di un «tu». Avendo l’autrice avvistato
(estate 2016) numerose “farfalline color zafferano” sui sentieri delle Cinque Terre, nonché
su quello di Monterosso costeggiante le ville Delle Due Palme e Vecchiona, dimostra
che il «tu» incarnato dalla farfalla di Dinard è Anna degli Uberti – altrove da Montale
enigmaticamente detta “morta a vent’anni”, nonché «maestra» di enigmistica –, già
simboleggiata da una farfalla in altri testi monterossini (Vecchi versi, L’estate, Dov’era
il tennis..., Annetta) e altresì “nascosta” nelle Storie naturali [II] (1950; Fuori di casa)
a Saint-Malo, di fronte a Dinard, nella Bretagna avente come protettrice Sant’Anna.

Abstract
La Farfalla di Dinard (1952) is analyzed here from the perspective of a mystery story,
centered on the “saffron-colored butterfly” (identified with Colias crocea, Fourcroy,
1785, also known in France as “Le Souci”) , that is close to dying out in Brittany, bringing
a “secret message” from a “you” to Montale. The author of this essay saw numerous
“saffron-colored butterflies” on Cinque Terre footpaths (summer 2016), as well as on
those of Monterosso, coasting the villas Delle Due Palme and Vecchiona. This shows
that the “you” embodied by the Dinard butterfly is Anna degli Uberti – elsewhere called
enigmatically by Montale “dead at the age of twenty”, as well as “master of enigmas”.
She was already symbolized by a butterfly in other texts revolving around Monterosso
(Vecchi versi, L’estate, Dov’era il tennis..., Annetta) and “hidden” in Storie Naturali [II]
(1950; Fuori di casa) in Saint-Malo, opposite Dinard, in Brittany, under the protection
of Saint Anne.
