As a laser user, I would like to take advantage of the strong data you reported on this widely debated and discussed topic, that is the photodynamic therapy in chronic periodontitis, in order to arouse interest among researchers and clinicians in performing and publishing similar studies on the subject, and also to make clear the need of special issues on authoritative journals like Lasers Med Sci to pose a milestone on this old subject over 30 years. The two oldest publications available in PubMed are both dated 1983 and are listed below [1, 2]. Thank you again for publishing this extremely interesting paper

    Several conditions such as age, gender, systemic diseases, smoking, consuming vitamin rich foods, denture wearing may play a role in aetiology of sublingual varices. However, few studies investigating these associations. Nevertheless, prevention of potential bleeding of sublingual varices especially in patients in antithrombotic therapy should led clinicians to treat them by diode laser, since it allows a minimally invasive treatment of photocoagulation. We report on a large sample of patients (706) showing single or multiple SVs on antithrombotic therapy, highlighting the possibility to treat lesions without drug discontinuation and without intra- and post -operative complications.     The utilization of computer-assisted design and computer-assisted manufacturing (CAD/CAM) techniques in the creation of complete dentures (CDs) has piqued the interest of many people. This article seeks to provide a comprehensive, critical, and objective analysis of the current knowledge of CDs and related technology. The aim of this study is to assess existing literature concerning 3D-printed complete dentures, covering aspects like innovative biomaterials, manufacturing methods and processes, workflow, and clinical effectiveness. The design of the current study included an initial review of 172 titles, an appraisal of abstracts, and finally a selection of articles for rigorous textual analysis. Inconsistencies discovered throughout the selection process were amicably resolved through discourse, culminating in the identification of 65 items. The publications retrieved from a thorough search of the PubMed, Scopus, and Embase databases spanned the years 1994 to 2023. Contemporary digital technology provides evident advantages, but its successful incorporation necessitates meticulous preparation.     Clinical Outcomes of Monolithic Zirconia Crowns on Posterior Natural Abutments Performed by Final Year Dental Medicine Students: A Prospective Study with a 5-Year Follow-Up

    The conventional metal–ceramic is still considered the gold standard in fixed prosthetics especially in terms of longevity. Among alternative materials used, Monolithic Zirconia has shown the capability to reconcile excellent biomechanical properties with acceptable aesthetic performance and to overcome several inconveniences related to veneer restorations. This study aims to clinically evaluate Monolithic Zirconia prosthetic crowns on natural abutments in the posterior sectors, performed by final-year dental medicine students (undoubtedly with less experience in the management of such material) by the standardized California Dental Association score system evaluation, to better understand the viability of Monolithic Zirconia. This prospective study was carried out at the Dental School of the University of Bari “Aldo Moro”, Italy. Prosthetic rehabilitation included single crowns or a short pontic prosthesis with maximum one intermediate. Final-year dental students performed tooth reduction under the supervision of three expert tutors. The California Dental Association systematics (based on color, surface, anatomical shape, and marginal integrity) were adopted to evaluate the prosthetic maintenance status over time. Annual follow-up visits were re-evaluated by the same parameters each year. Univariate logistic regression analysis was performed to evaluate outcomes and the Kaplan–Meier plot to report survival. The sample consists of 40 crowns performed on 31 patients, 15 males (48.4%) and 16 females (51.6%) with an average age of 59.3 years. The clinical cases subjected to experimental study were found to be “Excellent” (1a/2a/3a/4a) in 34 cases (85%), “Acceptable” in 4 cases (10%), and “To be re-done” in 2 cases (failures) (5%). Our conclusive data support the predictability of Monolithic Zirconia restorations on natural posterior abutments at a long-term follow-up of five years, even when performed by less-experienced clinicians

    Targeted Diode Laser Therapy for Oral and Perioral Capillary-Venous Malformation in Pediatric Patients: A Prospective Study

    Background: This study describes the management protocol for capillary-venous malformations in pediatric patients and reports the epidemiology of diagnosed and treated cases at the Unit of Odontostomatology of the Aldo Moro University of Bari from 2014 to 2022. Methods: The authors classified the intraoral and perioral capillary-venous malformations by superficial diameter (<1 cm, 1–3 cm, >3 cm) and ultrasonographical depth extension (≤5 mm, >5 mm). All patients underwent pulsed-mode diode laser transmucosal photocoagulation (8–12 W/cm2); those with malformations that were wide (>3 cm) and deep (>5 mm) received intralesional photocoagulation, too (13 W/cm2). The children received general anesthesia based on their compliance and lesions’ extension. The follow-up lasted six months. Results: A total of 22 females and 14 males (age range 4–18 years) presented 63 capillary-venous malformations. Five patients with Sturge–Weber syndrome, seven with hereditary hemorrhagic telangiectasia, and five with angiomatosis showed multiple malformations. The authors found no intraoperative or postoperative complications. Seventeen patients with lesions >1 cm and >5 mm deep required multiple laser sessions to heal. Conclusion: The results of the current study support diode laser photocoagulation as the gold standard for the treatment of intraoral and perioral capillary-venous malformations in pediatric patients

    Oral lichenoid lesions in HIV-HCV-coinfected subjects during antiviral therapy: 2 cases and review of the literature

    Some dental materials and certain drugs may induce epithelial alterations, which clinically resemble oral lichen planus (OLP), on oral mucosa. But these alterations do not have all the clinical and/or the histological features of OLP; these lesions are known as oral lichenoid lesions (OLLs). Some researchers describe the onset and/or the worsening of OLL/OLP after the administration of anti-hepatitis C virus (HCV) therapy. In this article, we describe the development of symptomatic OLLs, as a consequence of anti-HCV therapy (interferon-alpha and ribavirine), in 2 human immunodeficiency virus-HCV-coinfected subjects. An immunological cause related to coinfection and administration of different medications could be responsible for the onset of OLLs. These new cases, together with the previous reports of a possible association between OLP and/or OLL and anti-HCV therapy, highlight the absolute need to monitor carefully the human immunodeficiency virus-HCV-coinfected subjects who are about to start the anti-HCV therapy and to define better the clinical and histopathological criteria to distinguish OLP from OLL

    Periodontal diseases are a consequence of the host’s inflammatory and immune mechanisms against dysbiotic bacterial plaque. Given the role of probiotics in biofilm control and modulation of dysbiosis, this study assessed the efficacy of a specific strain of Lactobacillus Reuteri, DSM 17938, in the treatment of stage II and III periodontitis. 36 patients were randomly allocated into two groups: group A, the treated group; and Group B, the control group. The treated group and the control group both underwent initial periodontal debridement. Patients received medications after undergoing periodontal debridement. Clinical parameters were assessed at baseline and at 21 days. All parameters evaluated, Probing Depth (PD), Full Mouth Bleeding score (FMBS), and Full Mouth Plaque Score (FMPS) showed a reduction over time in both groups. The treated group showed a better reduction (p = 0.05) for PD. As far as the depth of probing is concerned, the decrease observed between the control group and the group treated with probiotics is such as to be considered statistically significant and since the average of the values for the treated group is higher than that of the control group, the use of probiotics has an efficacy of medium statistical importance in the treatment of periodontal disease

    Gingival Cyst of the Adult: A Case Description with a Relevant Literature Analysis

    Gingival cysts of the adult are rare and benign odontogenic lesions of the oral cavity, accounting for almost 0.3% of all odontogenic cysts. Their differential diagnosis is still challenging for surgeons as it includes other gingival inflammatory or non-inflammatory lesions and peripheral odontogenic tumors. The aim of this paper is to report a new case occurring in an adult, analyzing the clinical, radiographic, and histopathological features as guidelines for a differential diagnosis. We report a 49-year-old patient complaining of a small, pigmented lesion localized on the attached gingiva with no history of trauma, which was surgically excised and histologically diagnosed as a gingival cyst. A differential diagnosis may be challenging for clinicians it includes a wide spectrum of inflammatory and non-inflammatory lesions, so a correct diagnostical–therapeutical approach is needed to avoid possible overtreatment and minimize the recurrence rate

    Comparison between Conventional PMMA and 3D Printed Resins for Denture Bases: A Narrative Review

    The aim of the current paper is to review the available literature reporting on comparative studies of heat-cured resins and three-dimensionally printed biomaterials for denture bases in terms of their composition, properties, fabrication techniques and clinical performance. The methodology included applying a search strategy, defining inclusion and exclusion criteria, selecting studies to summarize the results. Searches of PubMed, Scopus, and Embase databases were performed independently by three reviewers to gather literature published between 2018 and 2021. A total of 135 titles were obtained from the electronic databases, and the application of exclusion criteria resulted in the identification of 42 articles pertaining to conventional and 3D printed technology for removable dentures. The main disadvantages of the heat-cured resins for removable dentures are that they require lots of special equipment, skilled personnel and time. Emerging technologies, such as 3D printed dentures, have the potential to alleviate these problems allowing for faster patient rehabilitation. With the development of digital dentistry, it is becoming increasingly necessary to use 3D printed resin materials for the manufacturing of removable dentures. However, further research is required on the existing and developing materials to allow for advancement and increase its application in removable prosthodontics
