    Long-term outcome of pattern-sensitive epilepsy

    Purpose: To evaluate the long-term outcome of patients with pattern-sensitive epilepsy. Methods: We prospectively studied 35 patients (21 females and 14 males) with pattern-sensitive epilepsy (follow up ≥5 years; mean 13.9; range 6.4 - 27.6). All cases had regular clinical examinations and serial electroencephalographic (EEG) recordings. Photosensitivity and pattern sensitivity were ascertained neurophysiologically in all cases. Outcome was evaluated according to the following variables: duration of photosensitivity, rate of remission (seizure-free period ≥ 2 years), withdrawal of therapy, and recurrence after drug discontinuation. Results: The epilepsy was generalized in 18 cases (17 idiopathic, one symptomatic) and partial in 17 (13 idiopathic, four symptomatic). Sixteen patients (46%) had a family history of seizures. The mean age at the last examination was 21.4 years (range 11.2-35.5 years). Five patients (14%) had only reflex seizures. The most common type of spontaneous seizures was generalized (60%), whereas reflex seizures were more frequently partial (74%). Mean epilepsy duration was 8.7 ± 6.3 years. Patients with only reflex seizures were instructed to avoid precipitating stimuli and were not treated with antiepileptic drugs. Treatment was gradually withdrawn in 10 out of 30 treated patients, with relapse in only two cases. At the end of follow up, 28 patients (80%) were seizure-free. Conclusion: The long-term outcome of patients with pattern-sensitive epilepsy indicates a good prognosis with a favorable course for both spontaneous and reflex seizures. © 2006 International League Against Epilepsy

    Lateralization and outcome of EEG abnormalities in benign epilepsy with centro-temporal spikes.

    L’ Epilessia parziale Benigna a punte Centro-Temporali (BECTS) è stata messa in relazione con altre forme di epilessia parziale idiopatica per il riscontro di analogie morfologiche e topologiche nei patterns EEG. Tali ipotetiche relazioni derivano da osservazioni di singoli casi, non essendo mai stati effettuati studi sistematici su campioni omogenei di BECTS. Lo studio si basa sulla valutazione longitudinale dei patterns EEG (follow up medio 6,3 anni), in 36 pazienti con BECTS. Le anomalie EEG all’ esordio erano prevalentemente unilaterali e regredivano nel 50% dei casi al termine del follow up. Patterns EEG extrarolandici erano presenti in 8 pazienti, in tutti la sede era occipitale; 5 di loro presentavano anche una risposta fotoparossistica alla SLI; 4 di loro presentavano anche altri tipi di crisi con sintomi tipici delle Epilessie Idiopatiche del Lobo Occipitale (Sindrome di Panayotopoulos e Sindrome di Gastaut). E’ probabile che alcuni pazienti con BECTS abbiano una predisposizione genetica a uno spettro più ampio di espressività elettroclinica età-dipendente. Ulteriori informazioni da studi clinici e genetici sono necessarie per capire il tipo di relazione che intercorre tra le diverse forme di Epilessia Parziale Idiopatica dell’ infanzia

    “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

    Treatment of multiple myeloma: A randomized study of three different regimens

    The results of an Italian multicentric trial for treatment of symptomatic Multiple Myeloma (MM) are reported. One hundred and thirty-three previously untreated patients were singled out at random for three different chemotherapy schedules: Melphalan plus Prednisone (M.P.) x 6 monthly cycles; Vincristine plus Melphalan plus Cyclophosphamide plus Prednisone (VMCP) x 6 monthly cycles; Peptichemio, Cyclophosphamide, BCNU. Drugs in this latter schedule were administered sequentially, for a period of six months. Criteria for response, progression and relapse were those of the Southwestern Oncology Group. Fifteen patients in MP chemotherapy (35%) and 20 patients in VCMP chemotherapy (46%) achieved an objective response (decrease of at least 50% in the synthesis index of Monoclonal Component (M.C.)), While only 3 out of the other 21 patients assigned to the third schedule responded to treatment. No significant differences were noted in the survival curves in either of the three treatment groups. The 38 responding patients did not receive maintenance therapy; no significant difference was found in remission duration between patients in MP and VCMP arms, with a median duration of 16 months for the whole group. No statistical difference was observed between survival and remission curves of patients with a 'response' (M. spike reduction > 75%) and those with 'improvement' (M. spike reduction between 75 and 50%). The authors conclude that the inclusion of Vincristine in a combination chemotherapy does not produce clear survival benefits; a longer induction period (12 cycles) could allow a better differentiation between MP and VMCP regimens
