    Laparoscopic Cholecistectomy in day surgery: preliminary experience of first 80 consecutive cases.

    Laparoscopic Cholecystectomy (LC) represents today the gold standard for the treatment of cholelithiasis (CL). The reported average post-operative hospital stay is 2.6 days. Day Surgery (DS) LC has been therefore suggested. Methods: Our treatment protocol for LC in DS includes age up to 65 years, BMI _ 35, no major contraindications for the general anesthesia (ASA I or II) and no acute complications of CL. Between July 2004 and July 2007, 80 consecutive patients (pt) underwent DS LC. They were 27 males and 53 females, age ranging between 15 and 63 years. Patient motivations and home support was also investigated. They were admitted at 07:00. A 3-trocar technique was always used. By 14:00 a soup is served and pt is allowed to walk. At 16:00 serum RBC, total and direct bilirubin, amylase and lipase are checked. At 18:00 the patient is discharged with dietary instruction, drug prescription, and the telephone numbers of the operative unit. Result: All the procedures were completed laparoscopically with a mean time of 38 min. Only 3 (4.1%) required overnight hospitalization for important nausea and vomiting. All patient but one expressed their satisfaction for this care regimen. Discussion: The ambulatory LC is justificated by simplification of the procedure, reduction of management costs even over one-day surgery regimen and also by reduct impact upon patients’ lifestyle

    Treatment of congenita1 and non-neoplastic trachea1 stenosis with the Nd-YAG laser. - The use of Nd-YAG laser in trachea1 surgery is recent. It makes possibile to perform endoscopic photoresection of nonneoplastic stenoses of the trachea. The Authors report their experience using laser endoscopy in four patients. Results confim the safety and usefulness oof thie new method in treatment of obstructive lesions of the trachea

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed

    “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

    Appropriate Similarity Measures for Author Cocitation Analysis

    We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.     Case clinical and biological features of an intranodal palisadet myofibroblastoma

    Abstract. Intranodal palisaded myofibroblastoma (IPM) is a rare benign tumor of the lymph nodes probably arising from smooth muscle-like cells. The tumor is characterized by intranodal proliferation of spindle cells. Neoplastic spindle-cell proliferation is most often of metastatic repetition which is very important in the recognition of IPM, because it may be mistaken for metastasis or other tumors such as Kaposi’s sarcoma. We report a novel case of IPM that confirms the myofibroblastic differentiation of the tumor. The onset of IPM has been associated with Epstein-Barr virus (EBV). In addition, recently reported cases of IPM have been seen with cyclin 1 overexpression and also with human herpes virus (HHV)-8 and EBV DNA sequences. In our case, there was no evidence of HHV-8 and EBV DNA sequences and we were not able to find cyclin 1 overexpression

    Surgery and intraperitoneal hyperthermochemotherapy in the treatment of malignant gastrointestinal tumors. In the last two years the Authors have treated 4 patients affected with malignant tumors of large bowel with very poor prognosis. In every cases they found large spreading to peritoneal cavity. These patients underwent to intraperitoneal iperthermochemotherapy using a special device conceived by Authors. This treatment produced promising results as related to life quality in these cases.Chirurgia associata ad ipertermochemioterapia intraoperatoria nel trattamento dei tumori maligni dell’apparato digerente con carcinosi peritoneali.Nell arco di due anni sono stati sottoposti ad ipertermochemioterapia intraperitoneale 3 pazienti affetti da neoplasia maligna del grasso intestino ed 1 da neoplasia maligna gastrica con disseminazioni peritoneali multiple. I pazienti inclusi nello studio sono stati sottoposti a trattamento ipertermico e chemioterapico della cavita peritoneale mediante l'uso di un dispositive meccanico da noi messo a punto. Il trattamento ha prodotto prometcenti risultati per quanto riguarda la qualita di vita det pazienti giunti a questo stadio di malatta
