    Étude des troubles du sommeil et de leurs repercussions chez des patients atteints d'une Maladie de Ménière

Introduction : la maladie de Ménière est une pathologie vestibulaire chronique dans laquelle la qualité de vie des patients est fortement altérée avec un impact psycho-physiologique majeur, à la source d’un handicap certain. L’objectif de notre étude est de décrire le sommeil des patients présentant une maladie de Ménière et de rechercher d’éventuelles associations entre la plainte cochléo-vestibulaires et les troubles du sommeil. Matériel : les patients atteints de maladie de Ménière ont bénéficié à partir du mois de mai 2019, suite aux recommandations de la société française d’ORL, à titre systématique d’une consultation au service d’exploration du sommeil à Strasbourg. Nous avons évalué de manière rétrospective la qualité du sommeil de ces patients, de juin 2019 à janvier 2020, et de son éventuel lien sur la symptomatologie vertigineuse. Résultats : nous avons inclus 26 patients qui présentaient des critères diagnostiques de maladie de Ménière avérée. L’âge moyen des patients était de 56 ans et les femmes représentaient 62% de notre effectif. Concernant le sommeil de nos patients, 58% se disaient insatisfaits et on retrouvait 73% de patients souffrant d’insomnie et/ou de syndrome d’apnées du sommeil au moins modéré. Le score moyen de Pittsburgh, qui évalue le sommeil dans sa globalité, était de 8.2 en moyenne et 76% des patients avaient un score pathologique, supérieur à 5. On retrouvait un syndrome d’apnées du sommeil modéré à sévère chez 42% de nos patients et nous avions retenu chez 38% des patients une indication à un appareillage par ventilation PPC. Enfin, 58% de nos patients présentaient les critères diagnostiques d’insomnie. Les patients avec un score fonctionnel d’invalidité des vertiges AAO-HNS - à 3 par rapport aux autres étaient davantage migraineux, 50% contre 8% (p = 0,03), insomniaques, 83% contre 31% (p - 0,01), et fatigués. La macroarchitecture du sommeil ne différait pas entre les deux groupes. L’insomnie a un lien d'association avec une maladie de Ménière davantage invalidante : Odd-ratio de 11.2 [1.91; 100], p = 0,014. Les patients insomniaques atteints de maladie de Ménière sont davantage dépressifs comparés aux patients non insomniaques : 40% contre 0% (p = 0.024). Les patients atteints d’un syndrome d’apnées –hypopnées modéré et sévère avaient un examen clinique vestibulaire plus pathologique, et présentaient une moins bonne audition dans les graves. Nous avons identifié un facteur de corrélation entre la perte auditive à 500Hz et l’IAH : coefficient de corrélation de Spearmann à 0.480 [0.00894; 0.951], p= 0,046. Les patients apnéiques modérés et sévères n’avaient pas de score fonctionnelle vertigineux AAO-HNS plus pathologique. Néanmoins nous avons identifié 5 nouveaux cas de patients apnéiques modérés et sévères nécessitant un traitement par PPC nocturne. Tous avaient reçu au-delà d’un simple traitement médical, contre seulement 32% des autres patients (p=0.01). Conclusion : près de 70% de nos patients souffrent cliniquement de troubles du sommeil, qui sont encore aujourd’hui sous diagnostiqués chez les malades de Ménière. Une consultation systématique de ces malades, ou du moins une considération poussée sur les troubles du sommeil, nous parait un point important de la prise en charge de ces patients, à plus forte raison devant le fait qu’un sommeil de mauvaise qualité est associé à une invalidité plus importante sur le plan vertigineux.SUMMARY: Study of sleep disorders and their repercussions in patients with Ménière's disease. Introduction: Meniere's disease is a chronic vestibular pathology in which the quality of life of patients is altered with a major psycho-physiological impact. The objective of our study is to describe the sleep of patients with Ménière's disease and to investigate possible associations between cochleo-vestibular complaints and sleep disorders. Material: Patients suffering from Ménière's disease have benefited from May 2019, following the recommendations of the French ENT society, from a systematic consultation at the sleep exploration service in Strasbourg. We retrospectively evaluated the quality of sleep of these patients, from June 2019 to January 2020, and its possible link on vertiginous symptomatology. Results: We included 26 patients who had diagnostic criteria for proven Ménière's disease. The average age of the patients was 56 years old and women represented 62% of our workforce. Regarding the sleep of our patients, 58% were dissatisfied and 73% had insomnia and/or at least moderate sleep apnea syndrome. Pittsburgh's average score, which evaluates sleep as a whole, was 8.2 and 76% of patients had a pathological score, greater than 5. Moderate to severe sleep apnea syndrome was present in 42% of our patients and 38% of our patients had an indication for CPAP ventilation. Finally, 58% of our patients had diagnostic criteria for insomnia.Patients with an AAO-HNS Dizziness Functional Disability Score - 3 compared to others had more migraine history, 50% versus 8% (p = 0.03), were more insomniacs, 83% versus 31% (p - 0.01), and fatigued. The macroarchitecture of sleep did not differ between the two groups.Insomnia is associated with a more disabling Meniere's disease: Odd-ratio of 11.2 [1.91; 100], p = 0.014. Insomniac patients with Ménière's disease are more depressed compared to non-insomniac patients: 40% versus 0% (p = 0.024).Patients with moderate and severe apnea-hypopnea syndrome had a more pathological vestibular clinical examination, and had poorer hearing in the severe ones. We identified a correlation factor between hearing loss at 500Hz and AHI: Spearmann's correlation coefficient at 0.480 [0.00894; 0.951], p= 0.046. Moderate and severe apneic patients did not have the more pathological AAO-HNS vertiginous functional score. Nevertheless, we identified 5 new cases of moderate and severe apneic patients requiring nocturnal CPAP treatment. All had received more than just medical treatment, compared to only 32% of the other patients (p=0.01). Conclusion: Nearly 70% of our patients clinically suffer from sleep disorders, which are still under-diagnosed in Ménière's patients. A systematic consultation of these patients, or at least a thorough consideration of sleep disorders, seems to us to be an important point in the management of these patients.     Associées à la vision et à la proprioception, les informations sensorielles provenant du vestibule de l'oreille interne assurent à l'homme de maintenir sa position d'équilibre statique et dynamique. Un dérèglement de la physiologie du système vestibulaire peut provoquer d'importantes crises de vertiges (comme dans la maladie de Menière), un problème de santé publique mal maîtrisé en France. Aujourd'hui demeurent une méconnaissance sur le comportement de fines structures présentes dans le labyrinthe de l'oreille interne, et une incompréhension au niveau des interactions entre ces fines structures et les fluides présents. Nous engageons une méthode pour caractériser le tissu vestibulaire membraneux et modélisons par éléments finis et de manière originale le canal semi-circulaire latéral et l'utricule, en vue d'étudier l'influence de modifications anatomiques et des propriétés tissulaires sur la physiologie du système.Combined with vision and proprioception, the sensory inputs provided by the vestibule of the inner ear allow the human body to maintain static and dynamic balance. Dysfunction of the vestibular system’s physiology can lead to major attacks of vertigo and dizziness (as in Menière's disease), a poorly controlled public health problem in France. Today, there remains a lack of knowledge about the behavior of fine structures of the inner ear labyrinth, and a lack of understanding of the interactions between these fine structures and the fluids present. We are developing a method for characterizing membranous vestibular tissue and modeling in an original way a finite element model of the lateral semicircular canal and utricle, in order to study the influence of anatomical changes and tissue properties on the physiology of the system

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed.     “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

    Appropriate Similarity Measures for Author Cocitation Analysis

    We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.     Dispelling the Myths Behind First-author Citation Counts

    We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more sophisticated methods

    We have done our best to complete the author checklist relating to the use of animals in the hut study. Note that the objective for the hut study was to evaluate the IRS treatment applications for residual efficacy against Anopheles mosquitoes, including the local An. coluzzii mosquito population. Cows were only used to attract mosquitoes into the huts and no tests were carried out directly on the cows. The author checklist is intended for use with studies where experiments are carried out on animals, which is why we have had such difficulty in completing this for the hut study, as many of the questions do not relate to how the cows were used
