Score and application for surgical risk stratification in patients with malignant tumors of the digestive tract
Os tumores malignos do trato digestório são doenças graves que acometem pacientes já idosos em sua maioria, que para seu tratamento curativo ou paliativo, devem se submeter a procedimentos cirúrgicos em muitos casos. Tais pacientes são frágeis pela idade e por comorbidades que podem ou não estar associadas às neoplasias, fazendo com que o risco de complicações pós-operatórias seja maior. Apesar de haver muitos escores para predição de risco cirúrgico, não existe um que seja específico para este grupo de pacientes. O objetivo deste trabalho foi desenvolver um escore conjuntamente com seu aplicativo para uso em smartphones para auxiliar a abordagem inicial de pacientes com cânceres do trato gastrointestinal no período pré-operatório de sua cirurgia oncológica, a fim de predizer o risco de morbidade e mortalidade no pós-operatório. Como mecanismo do aplicativo, foram utilizados fatores prognósticos embasados na literatura médica destinados ao atendimento destas situações clínicas. Os referidos fatores foram pesquisados nas bases de dados Pubmed e Scielo. O aplicativo funciona como uma calculadora, na qual dados sobre o paciente são incluídos em sequência pelo usuário até que uma faixa de risco provável de complicação pós-operatória seja exibida como resultado juntamente com sugestões de medidas, destinadas a orientar o médico assistente a otimizar os cuidados peri-operatórios para minimizar o risco cirúrgico. Em testes utilizando simulação de situações clínicas com grupo de cirurgiões especializados no tratamento de cânceres do trato gastrointestinal, a usabilidade do aplicativo obteve uma média de 97,5%, o aplicativo foi considerado de fácil uso em 100%. Contudo, faz-se necessário estudo clínico para validação do escore antes de utilizá-lo na prática clínica. Os resultados não tiveram distinção com a idade ou tempo de prática profissional. Concluiu-se que o escore e o aplicativo desenvolvidos poderão contribuir em predizer complicações pós-operatórias, auxiliando o médico a diagnosticar, conduzir e instituir as primeiras medidas que melhorem o performance do paciente.Malignant tumors of the digestive tract are serious diseases that affect elderly patients, in the majority of cases, that, for curative or palliative treatment, must undergo surgical procedures in many cases. Such patients are fragile due to their age and comorbidities that may or may not be associated with neoplasms, making the risk of postoperative complications high. Although there are many scores for predicting risk, there is no specific one for this group of patients. The objective of this study was to develop a score together with its app for use on smartphones to assist in the initial approach of patients with cancers of the gastrointestinal tract in the pre-operativ set of their oncological surgery, in order to predict the risk of morbidity and mortality in the post-operative period. As app’s mechanism, prognostic factos based on the medical literature were used to address these clinical situations. These factors were researched in the Pubmed and Scielo databases. The application works like a calculator, in which data about the patient is inserted in sequence by the user until a range of probable risk of postoperative complication is displayed as a result, associated with suggested measures designed to guide the attending physician to optmize the patient care or prevention to minimize surgical risk. Tests were performed in surgeons with experience in treating cancers of the gastrointestinal tract using simulated clinical situations, the application’s usability obtained na average of 97.5%, the app was considered easy to employ in 100%. However, a clinical study is necessary to validate the score before using it in clinical practice. Results did not differ according to age or time of professional practice. It is concluded th initial management recommendations in 92.8% plus partially appropriate at 7.2%. As conclusion, the developed score and app can contribute to predicting postoperative complications, helping the attending physician to diagnose, manage and initiate measures that improve the patient’s performance
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. Dispelling the Myths Behind First-author Citation Counts
We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued
use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation
counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. We have done our best to complete the author checklist relating to the use of animals in the hut study. Note that the objective for the hut study was to evaluate the IRS treatment applications for residual efficacy against Anopheles mosquitoes, including the local An. coluzzii mosquito population. Cows were only used to attract mosquitoes into the huts and no tests were carried out directly on the cows. The author checklist is intended for use with studies where experiments are carried out on animals, which is why we have had such difficulty in completing this for the hut study, as many of the questions do not relate to how the cows were used
