    Deeper than Symptoms: Heterogeneity as a Challenge to Network Models in Psychiatry

    According to the Network Models account (NM), mental disorders arise from the interplay among symptoms in a network structure. Crucially, some versions of NM rest on the idea that symptoms should be taken as the causally relevant unit of analysis in the study of mental disorders. In this paper we offer some reasons to resist such an idea, by arguing that the exclusive focus on symptoms and their connections fails to provide us with the appropriate level of analysis to understand mental disorders. Drawing on evidence from cultural psychiatry and medical anthropology, we show that heterogeneity raises serious issues for NM. Through the discussion of three significant cases, we suggest that some versions of NM risk missing out on crucial aspects of the person’s pathology. Proponents of NM should therefore amend their account so as to make it more responsive to different aspects of heterogeneity

    Fino a qualche anno fa quasi tutti i bambini sordi, indipendentemente dalle loro specifiche o generiche variabilità, venivano protesizzati e seguiti in logopedia. Oggi la diffusione dell’Impianto Cocleare (IC) fornisce ulteriori variabili che rendono maggior- mente complesso e difficoltoso il percorso verso un’informazione completa e accessibile da parte delle persone sorde, delle famiglie, degli operatori del settore. Riteniamo pertanto fondamentale avviare una seria riflessione anche interna all’ENS su tale delicato e complesso tema, fornendo ulteriori strumenti, dati, idee e pro- spettive di analisi. Viene esposta qui la sintesi di questo primo lavoro condotto dalle due vincitrici della borsa, Valentina Foa e Cristiana Varuzza, nel corso dell’anno de- dicato all’attività di ricerca. Si tratta di un progetto in evoluzione, aperto a ogni col- laborazione futura, con l’augurio che questo lavoro e i suoi sviluppi possano contribuire ad una maggiore informazione, invitino alla riflessione e al confronto e supportino le scelte e gli orientamenti delle persone sorde, delle famiglie, del personale medico e di tutti quelli che operano nel complesso e variegato mondo della sordità.Until a few years ago almost all deaf children, regardless of their specific or generic variability, were fitted with prostheses and followed up in speech therapy. Today, the diffusion of the Cochlear Implant (CI) provides further variables that make the path towards complete and accessible information by deaf people, families and operators in the sector more complex and difficult. We therefore believe it is essential to start a serious reflection, also within the ENS, on this delicate and complex issue, providing further tools, data, ideas and analysis perspectives. Here is the summary of this first work conducted by the two winners of the scholarship, Valentina Foa and Cristiana Varuzza, during the year dedicated to research activity. It is an evolving project, open to any future collaboration, with the hope that this work and its developments can contribute to greater information, invite reflection and comparison and support the choices and orientations of deaf people, of families, medical personnel and all those who work in the complex and variegated world of deafness

    Genome sequences of segmented filamentous bacteria in animals Implications for human research

    Experimental studies indicate that segmented filamentous bacteria (SFB) can recapitulate fundamental immune responses, particularly in reference to Th 17 and regulatory T-cell activity. The recent description of whole genomic DNA sequences of mouse and rat SFBs and the comparison between these sequences opens new important perspectives. In particular this knowledge allows to perform quali-quantitative studies in human beings regarding these yet unculturable anaerobic bacteria. The comparative analysis of rat and mouse SFB 16S rRNA sequences suggests the existence of highly conserved regions that could represent SFB-specific molecular targets potentially useful to develop PCR probes for study SFB in humans. These molecular tools may be of inestimable value to evaluate the possible role played by SFB in the immune system physiology, as well as its potential involvement in human chronic inflammatory and autoimmune diseases

    HHV-6A infection induces amyloid-beta expression and activation of microglial cells

    BACKGROUND: The control of viral infections in the brain involves the activation of microglial cells, the macrophages of the brain that are constantly surveying the central nervous system, and the production of amyloid-beta (Aβ) as an anti-microbial molecule. Recent findings suggest a possible implication of HHV-6A in AD. We evaluated the effect of HHV-6A infection on microglial cell expression Aβ and the activation status, determined by TREM2, ApoE, cytokines, and tau expression. METHODS: We have infected microglial cells (HMC3, ATCC®CRL-3304), in monolayer and human peripheral blood monocyte-derived microglia (PBM-microglia) spheroid 3D model, with HHV-6A (strain U1102) cell-free virus inocula with 100 genome equivalents per 1 cell. We collected the cells 1, 3, 7, and 14 days post-infection (d.p.i.) and analyzed them for viral DNA and RNA, ApoE, Aβ (1-40, 1-42), tau, and phospho-tau (Threonine 181) by real-time immunofluorescence and cytokines by immunoenzymatic assay. RESULTS: We observed a productive infection by HHV-6A. The expression of Aβ 1-42 increased from 3 d.p.i., while no significant induction was observed for Aβ 1-40. The HHV-6A infection induced the activation (TREM2, IL-1beta, ApoE) and migration of microglial cells. The secretion of tau started from 7 d.p.i., with an increasing percentage of the phosphorylated form. CONCLUSIONS: In conclusion, microglial cells are permissive to HHV-6A infection that induces the expression of Aβ and an activation status. Meanwhile, we hypothesize a paracrine effect of HHV-6A infection that activates and induces microglia migration to the site of infection

    Triglyceride-glucose index predicts outcome in patients with chronic coronary syndrome independently of other risk factors and myocardial ischaemia

    AIMS: The triglyceride-glucose (TyG) index, a surrogate marker of insulin resistance (IR), is a prognostic risk factor in the general population. We aimed to assess whether it is an independent predictor of outcome also in patients with chronic coronary syndrome (CCS). METHODS AND RESULTS: TyG index was evaluated in 1097 consecutive patients (75% men, median age 72 years) with known (26%) or suspected coronary artery disease (CAD), undergoing stress-rest myocardial perfusion scintigraphy, and coronary angiography and followed up for a median of 4.5 years. Moderate/severe perfusion abnormalities during stress (summed stress score >7) were documented in 60% of patients, obstructive CAD in 74%, and 36% underwent early revascularization. TyG index was 8.9 (median, interquartile interval 8.6–9.2). Cardiac death or myocardial infarction occurred in 103 patients and all-cause death in 65. After correction for clinical risk factors, LV function and common bio-humoral variables, TyG index (HR 2.42, 95% CI 1.57–3.72, P < 0.001), and moderate/severe stress perfusion abnormalities (hazard ratio (HR) 2.17, 95% confidence interval (CI) 1.25–3.77, P < 0.001) independently predicted cardiac events. TyG index (HR 3.64, 95%CI 2.22–5.96, P < 0.001) and high-sensitivity C-reactive protein (HR 1.11, 95% CI 1.04–1.19, P = 0.002) independently predicted all-cause death. CONCLUSION: In patients with CCS, the TyG index identifies a cardiometabolic profile associated with an additional risk of cardiac events, over the presence of myocardial ischaemia and independently of other clinical, common bio-humoral or imaging risk determinants

Las mismas fueron descriptas como lavas andesíticas y basandesíticas que se interdigitan con rocas del Complejo Volcánico Domuyo (Pleistoceno) En el presente trabajo se describe esta unidad a partir del levantamiento de cartografía de detalle y una serie de perfiles, y paneles realizados con VANT, además del análisis de facies. Estas rocas en algunos sectores no presentan deformación, pero en otros muestran estructuras de bandeamiento subverticales y convolutas, de gran escala, asociadas a pequeñas disyunciones columnares perpendiculares al bandeado y a brechas. Además de ubicar litoestratigráficamente a esta unidad, se presenta un modelo depositacional de las corrientes piroclásticas densas concentradas que le dieron lugar
