    Improved survival in metastatic germ-cell cancer

    Background: The prognostic score of the International Germ Cell Cancer Collaborative Group (IGCCCG) in metastatic germ-cell cancers (mGCC) relies on treatments delivered before 1990. It is unclear, if this score is still relevant to contemporary cohorts of patients who receive modern-type chemotherapy and supportive care. Patients and Methods: All patients who underwent cisplatin/etoposide based first-line chemotherapy for mGCC at the University Hospital Zurich (USZ) between 1991 and 2016 were identified retrospectively. Clinical characteristics were extracted from medical charts and patients classified according to the IGCCCG score (J Clin Oncol 1997;15:594). Progression-free survival (PFS) and overall survival (OS) probabilities at 5 years served as outcome parameters. Results: The study cohort consisted of 204 patients at a median age of 32 years and a median follow-up of 4.2 years. According to the IGCCCG score, PFS in the contemporary USZ cohort was 71% overall; 83% for good risk, 69% for intermediate risk and 30% for poor risk patients, p < 0.001. OS for the entire cohort was 88%. In respect to OS, we observed no difference between good risk and intermediate risk patients (94% vs. 91%, p = 0.62), but a statistically significant difference between those two risk groups and poor risk patients, who had an OS of only 65%, p < 0.001. Conclusions: Within the contemporary USZ cohort of mGCC patients no improvements in PFS probabilities were observed compared to the ones predicted by the IGCCCG score for any prognostic category, but marked improvements in OS probabilities for intermediate risk and poor risk patients, possibly due to better salvage treatments

    "Seit einigen Jahren wird Unterrichtsforschung vermehrt videobasiert angegangen. Die methodologische Diskussion, die die Weiterentwicklung des Instrumentariums begleitet, kreist dabei zentral um die Frage der Invasivität der Aufzeichnungstechnik und möglicher Kameraeffekte. Je nach methodologischer Ausrichtung wird diesem Problem anders begegnet: So versucht die abbildtheoretische Unterrichtsvideografie, die Invasivität zu kontrollieren. Qualitative Methoden wie die Kameraethnografie dagegen machen mögliche Kameraeffekte wie die Performanz vor der Kamera oder die Blickrichtung, die durch das Kameraauge fixiert wird, zum Ausgangspunkt neuer, ästhetischer Verfahrensweisen. Der vorliegende Artikel geht einen dritten Weg: Anhand eines Rückgriffs auf den Wissenschaftssoziologen LATOUR und anhand der methodologischen Reflexion eines forschungspraktischen Beispiels sollen grundsätzliche Probleme der Unterrichtsforschung herausgearbeitet werden, die sich mit dem Einsatz der Videografie besonders deutlich zeigen und mit der sich die Unterrichtsforschung vermehrt auseinandersetzen müsste." (Autorenreferat)"In the last few years videography has been used for research on classroom teaching. The methodological discussion accompanying the development of this technique centers on the question of the invasiveness and influence of filming on instructors and students. Depending on methodological orientation, this problem is met in different ways. Quantitative methods attempt to control the invasiveness. Qualitative methods, such as ethnography, use possible camera effects, such as performance whilst being filmed, or the position of the camera as a starting point of new (aesthetical) procedures. In this article the author outlines an alternative approach. Drawing upon the work of Latour, and reflecting on an example of research practice, fundamental problems of research on classroom teaching are elaborated. The problems noted above become particularly apparent with the use of videography. It will be argued that the benefits of using videography should be addressed in research methods courses and in the practice of research on teaching." (author's abstract

    Protection versus retreat: the economic costs of sea-level rise

    The author analyses the relative role of protection (or damage mitigation) expenditures within the total costs associated with raised sea levels induced by climate change. A rule of thumb is derived to approximate the optimal level of protection. Economic efficiency requires that protection expenditures are designed such that the sum of protection costs plus remaining land-loss damage is minimised. The optimal protection level will depend on the relative importance of dryland loss compared with the costs of accelerated wetland loss plus protection expenditures. This framework is then used to estimate the damage-cost functions associated with a sea-level rise for the countries of the OECD.
