Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. Delayed hemolysis after treatment with parenteral artesunate in African children with severe malaria
Malaria ist weiterhin eine der bedeutendsten parasitären Erkrankungen weltweit, an der 2015 laut Schätzungen der WHO 212 Millionen Menschen erkrankten. Bei 429 000, insbesondere Kindern unter fünf Jahren führte sie zum Tod. P. falciparum ist für den Großteil der schweren Verlaufsformen verantwortlich.
In den letzten Jahren hat sich das Medikament Artesunat bei der Therapie der schweren Malaria als Mittel der Wahl durchgesetzt, es fehlen allerdings noch Untersuchungen zu Langzeitfolgen. Seit 2011 wurde wiederholt von Fällen berichtet, bei denen es zu verzögert auftretenden Hämolysen etwa 14 Tage nach Artesunat- Therapie kam. Ihr Auftreten wurde im Rahmen einer Pilotstudie in der endemischen Region Ghana untersucht. Es wurden hierfür über sechs Monate Kinder zwischen 5 Monaten und 10 Jahren mit schwerer Malariainfektion rekrutiert, mit Artesunat therapiert und über vier Wochen nachverfolgt. Dabei lag der Fokus auf dem Auftreten einer hämolytischen Anämie. Es wurden insgesamt 53 Patienten rekrutiert, von denen vier an Tag 14 Anzeichen einer Hämolyse hatten. Probanden, die Hämolysen entwickelten, hatten höhere Parasitendichten und waren jünger. Insbesondere bei einem Patienten war die Blutbildveränderung therapiebedürftig. Alle Patienten, bei denen laut aktueller Studienlage bisher verzögerte hämolytische Episoden auftraten und beschrieben wurden, überlebten diese. Einige Patienten entwickelten schwere therapiebedürftige Anämien und benötigten Bluttransfusionen. Ohne Behandlung wären lebensbedrohliche Komplikationen aufgetreten. Ein solcher Fall ist auch in dieser Studie beschrieben.
Die genauen pathophysiologischen Ursachen, die für die verzögert auftretenden Hämolysen verantwortlich sind, sind noch nicht bekannt. Es werden verschiedene Mechanismen diskutiert. Unter anderem wird angenommen, dass das sog. ‚pitting’ in der Milz eine entscheidende Rolle spielt. Dabei kehren ehemals infizierte Erythrozyten zunächst in den Blutkreislauf zurück, bevor sie dann mit verkürzter Lebenszeit mehr oder weniger synchron zugrunde gehen. Dadurch könnte eine verzögerte Hämolyse zumindest teilweise erklärt werden. Die Bedeutung von Artesunat als Therapie der schweren Malaria und seine nachgewiesene Überlegenheit gegenüber Chinin sollte dadurch nicht in Frage gestellt werden. Allerdings sollten alle Patienten in endemischen sowie nicht-endemischen Regionen innerhalb von vier Wochen nach der Therapie engmaschig nachkontrolliert werden.Malaria still ist one of the most significant parasitic infections worldwide. According to the WHO, an estimate of 212 million cases occured in 2015. About 429 000 malaria deaths were reported, most of them iconcerning children aged 5 and under. Malaria infection due to P. falciparum ist the most deadly.
Intravenous Artesunate is recommended as first-line therapy for severe malaria. Data on long-terme treatment safety still is incomplete.
Cases of delayed haemolysis about 14 days after artesunate treatment have been reported repeatedly since 2011. A pilot study was conducted in the endemic region Ghana. Children between 5 months and 10 years of age with severe malaria infection were included, treated with parenteral Artesunate and a followed up for 28 days. The primary outcome was the incidence of therapy associates haemolytic anemia.
A total of 53 children were included. Four oft them showed signs of haemolysis on day 14 after treatment. Probants developing hemolyses had higher parasite counts and were younger. One oft hem especially was in need of therapy.
All patients developing delayed haemolysis according to current research survived. Some patients however sufferd severe anemia and required further treatment such as blood transfusions. Without treatment life threatening complications would have occurred. One of these cases is described in this study.
The exact patophysiological mechanisms causing delayed haemolysis aren’t finally researched. Different mechanisms have been discussed. One oft them ist so called ‚pitting’ in the spleen. Once infected erythrocytes returne to the blood circulation with a shortened lifespan untill they are eliminated synchroneosly. This could explain delyed hemolysis at least partly.
The importance of Artesunate for treatment of severe malaria and its superiority over Quinine shouldn’t be questioned. In conclusion prolonged follow-ups should be established for all patients recieving Artesunate
Cardiovascular Dyfunktion in HIV patients in Ghana
35,3 Millionen Menschen leben weltweit mit HIV, 69% von ihnen im subsaharischen Afrika. Dank der antiretroviralen Therapie sind die Überlebenszeiten der betroffenen Patienten heutzutage verlängert. Jedoch treten immer mehr Späterscheinungen der HIV-Infektion auf.
Das Ziel dieser Arbeit war die Prävalenz und das Spektrum von kardiovaskulären Erkrankungen bei HIV-positiven Patienten in Ghana zu determinieren. Es wurden 200 Teilnehmer von Juli bis Dezember 2010 in der HIV-Ambulanz des Komfo Anokye Teaching Hospital, Kumasi, Ghana eingeschlossen.
In dieser Studie konnte gezeigt werden, dass kardiovaskuläre Erkrankungen bei HIV-infizierten Patienten auch in Kumasi ein ernstzunehmendes gesundheitliches Problem sind. 21,1% der Patienten unserer gesamten Studienkohorte wiesen eine oder mehrere kardiale Dysfunktionen auf. Die häufigsten kardialen Diagnosen waren Perikarderguss (24,0%), linksventrikuläre systolische Dysfunktion (20,6%) und pulmonaler arterieller Hochdruck (20,0%). Diese Ergebnisse stehen im Einklang mit ähnlichen Studien, die in anderen afrikanischen Ländern durchgeführt wurden. Daher empfehlen sich ein erhöhtes Bewusstsein für die Problematik und die Einrichtung eines Screening-Programmes von HIV-positiven Patienten zur frühzeitigen Erkennung kardialer Probleme. So könnte eine adäquate Behandlung früher erfolgen und möglicherweise eine Reduktion der Morbidität und Mortalität erreicht werden.
Im Gegensatz zu anderen Studien bestand zwischen den sozio-demografischen Angaben der Patienten und deren kardiovaskulären Dysfunktionen kein Zusammenhang. Weiterhin zeigten unsere Patienten kein einheitliches Bild bei Betrachtung der kardialen Dysfunktionen und dem Immunstatus. In der Literatur ist das Auftreten einer kardialen Erkrankung meist mit einem niedrigen Immunstatus assoziiert
Dispelling the Myths Behind First-author Citation Counts
We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued
use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation
counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more

Beeinträchtigung der Herzfunktion von Kindern mit schwerer Malaria tropica
Nach wie vor ist die Malaria tropica in Kumasi und Zentral- Ghana eine der Hauptursachen für viele Todesfälle im Kindesalter. Schwere Infektionen wie die Sepsis beeinflussen generell Herzfunktion und hämodynamischen Status, jedoch ist konkret bei schwerer Malaria über ähnliche Auswirkungen bisher wenig bekannt. Resultate von kürzlich durchgeführten Studien mit Biomarkern weisen auf eine Beeinflussung der Herzfunktion hin, aber es existierten keine klinischen Untersuchungen, die dies beweisen.
Aufgrund dessen wurde 2008 und 2009 eine groß angelegte Studie durchgeführt. Es wurden 183 Kinder im Alter von 12 und 108 Monaten aufgenommen und klinische sowie Blutparameter an den Tagen Null und 42 (+/- 2 Tage) erhoben. Bedside tests für biochemische Marker wie Herzenzyme, Kreatinin, Blutgasanalysen, Elektrolyte und Blutauszählungen, sowie Funktionsuntersuchungen wie Echokardiographie und Elektrokardiogramm wurden mit klinischen Symptomen und Parasitendichte während einer Erkrankung verglichen. Uni- und multivariate Korrelationsanalysen sowie intra- individuelle Ergebnisse wurden zwischen den Tagen Null und 42 ermittelt.
Das Herzminutenvolumen war am Aufnahmetag im Vergleich zu Tag 42 signifikant erhöht (5.8 ml/m2 (SD +/- 1.8) zu 4.7 ml/m2 (SD +/- 1.4); P< 0.001). Ein gesteigertes Herzminutenvolumen korrelierte negativ mit den Hämoglobinwerten, aber nicht mit Parametern die auf eine eingeschränkte Gewebeperfusion oder eine metabolische Azidose hinweisen. Die Parasitendichten hatten einen signifikanten Einfluss in Richtung einer metabolischen Azidose, aber nicht auf das Herzminutenvolumen. Verschlechterte Herzfunktion, niedrige Hämoglobinwerte und eine metabolische Azidose wurden vor allem bei Kindern unter zwei Jahren gefunden.Mortality and morbidity due to Plasmodium falciparum malaria remain high. While severe infectious diseases like sepsis alter cardiac function and hemodynamic status, little is known on cardiac dysfunction in severe malaria. Results of recent studies with biomarkers indicate impaired cardiac function in severe malaria but no larger clinical investigation is available to support this.
To get more results about the cardiac function in children with severe malaria a study to investigate cardiac function among children with severe malaria in Ghana was conducted. In 2008 and 2009, 183 children aged 12-108 months were included and clinical as well as blood parameters were obtained upon admission and at day 42 (+/- 2 days). Bedside tests for biochemical markers like cardiac enzymes, creatinine, blood gas analyses, electrolytes and blood counts as well as functional analyses like standardized cardiac ultrasound and electrocardiograms were compared with clinical symptoms and conditions as well as blood parasite levels. Uni- and multivariate correlation analyses as well as intra-individual pair-wise comparisons between day 0 and 42 were performed.
Cardiac index (CI) was significantly increased on day 0 compared to day 42 (5.8 ml/m2 (SD +/- 1.8) versus 4.7 ml/m2 (SD +/- 1.4); P< 0.001). Increased CI correlated negatively with haemoglobin levels but not with parameters indicating impaired tissue perfusion or metabolic acidosis. Parasite levels had a significant influence on metabolic acidosis but not on CI. Alterations related to cardiac function, haemoglobin levels and metabolic acidosis were most prominent in children younger than two years
