Inflammationsinduzierte Vaskulopathien bei Patienten mit Cystischer Fibrose
Introduction: Cystic fibrosis is a disease that is primarily associated with pulmonary involvement. Frequent infections and increased pulmonary arterial pressure are prominent in this disease, leading to alterations of the bronchi and bronchioli, the interstitial tissues and the pulmonary vessels. Structural changes of the vessels are rarely considered in the current literature.
Methods: Retrospective analysis and microscopic examination of explanted lungs from cystic fibrosis patients in the period from 06/1998 - 12/2016, at the Institute of General and Special Pathology, Saarland University. Intrapulmonary arteries were qualitatively and quantitatively analyzed focussing on remodeling and clinical parameters.
Results and discussion: Explanted lungs of 35 cystic fibrosis patients were analyzed. All had a double lung transplantation. The average age at transplantation was 28.5 8.5 (SD) years. The average duration of disease was 23.2 8.4 years. Most of the patients were in a markedly reduced clinical condition and had limited lung function. More than 900 vessels were histologically examined and structural changes of the intima recorded. Intimal thickening accounted for up to 51.7% of the total wall thickness. There was a significant difference in intimal thickness in patients whose transplantation took place before the age of 18. In addition, there was a significantly greater intimal thickness in patients who had received azithromycin therapy prior to transplantation. No significant results could be found for genotype of the CFTR gene, preoperative immunosuppression, preoperative condition of the patients or demographic differences.
Conclusions: Intimal changes of the pulmonary vessels could be found in varying degrees in cystic fibrosis patients. The age at lung transplantation and azithromycin therapy correlated significantly with arterial intimal remodeling. In univariate analysis it was not possible to detect other significantly correlation parameters. The changes were based on multifactorial causes and influences. Possible therapy effects showed no significant influence as well.Einleitung: Die Mukoviszidose ist eine Erkrankung, die vor allem auch mit einer pulmonalen Beteiligung einhergeht. Häufige Inflammationen und ein erhöhter pulmonal arterieller Druck sind bei dieser Erkrankung ausgeprägt und können strukturelle Veränderungen der pulmonalen Gefäße bedingen oder aggravieren. Das pulmonale „remodelling“ findet in der aktuellen Literatur kaum Beachtung.
Methoden: Retrospektive Analyse und mikroskopische Untersuchung aller explantierter Lungen von Mukoviszidosepatienten, die im Zeitraum von 06/1998 - 12/2016, im Institut für Allgemeine und Spezielle Pathologie der Universität des Saarlandes untersucht worden sind.
Ergebnisse und Diskussion:
In dieser Studie wurden die explantierten Lungen von 35 Mukoviszidosepatienten aufgenommen. Bei allen lag eine Doppellungentransplantation vor. Das durchschnittliche Alter bei Transplantation lag bei 28,5 8,5 (SD) Jahren. Die durchschnittliche Erkrankungsdauer lag bei 23,2 8,4 Jahren. Die meisten Patienten waren in einem reduzierten klinischen Zustand und hatten eine reduzierte Lungenfunktion. In mehr als 900 histologisch analysierten Gefäßen, fanden sich Veränderungen der Intima im Sinne einer Intimahyperplasie. Diese Intimahyperplasie machte bis zu 51,7% der Gesamtwanddicke aus. Es fand sich ein signifikanter Unterschied der Intimadicke bei Patienten, deren Transplantation bereits vor dem 18. Lebensjahr stattfand. Zusätzlich zeigte sich eine signifikant größere Intimadicke bei den Patienten, die vor Transplantation eine Azithromycintherapie erhalten hatten. Zu den übrigen in dieser Studie untersuchten potentiellen Einflussfaktoren wie Genotyp, Erregerspektrum der Keimbesiedelung, präoperativer klinischer Zustand der Patienten, präoperative Immunsuppression, sowie demographische Unterschiede, konnten keine signifikanten Ergebnisse nachgewiesen werden.
Schlussfolgerung:
Bei Mukoviszidosepatienten lassen sich Intimaveränderungen der intrapulmonalen Arterien in unterschiedlichem Ausmaß finden. Die Benennung eines einzelnen auslösenden Faktors war nicht möglich. In der Analyse der Daten konnte keine signifikante Korrelation zu demographischen Unterschieden, verschiedenen Genotypen, Erregerspektrum der Keimbesiedelung, sowie zur präoperativen Immunsuppression nachgewiesen werden. Prognostic relevance of EGFR and KRAS mutation burden in adenocarcinomas of the lung
Lungenkrebs gehört mit ca. 57500 Neuerkrankungen im Jahr zu den häufigsten Krebserkrankungen in Deutschland. Da die Tumorerkrankung in über 50 % der Fälle erst diagnostiziert wird, wenn bereits eine Fernmetastasierung vorliegt, ist eine komplette Tumorentfernung oft nicht mehr möglich. Bei diesen Patienten kommt der medikamentösen Therapie eine besondere Bedeutung zu. Noch vor wenigen Jahren stellte die Chemotherapie die einzig mögliche medikamentöse Therapieform dar. Mittlerweile ist bekannt, dass sich die Tumore im Hinblick auf ihre genetischen Eigenschaften unterscheiden und mehrere Angriffspunkte für neue zielgerichtete Medikamente bieten. Beim pulmonalen Adenokarzinom, das geschlechtsabhängig ca. 36-46 % der malignen pulmonalen Neoplasien ausmacht, liegen je nach Ethnizität der Betroffenen und Studie in ca. 10-50 % der Fälle Mutationen im EGFR-Gen und in ca. 15-30 % der Fälle Mutationen im KRAS-Gen als onkogene Treiber vor. Während sich für Patienten mit Treibermutation im EGFR-Gen bereits prognoseverbessernde zielgerichtete Medikamente etabliert haben, befinden sich zielgerichtete Medikamente für Patienten mit KRAS-Mutation derzeit noch in Entwicklung. Zum Auffinden von Mutationen werden die 1977 vorgestellte Sanger-Sequenzierung und die 2005 eingeführte Hochdurchsatz-Sequenzierung (Next-Generation-Sequenzierung) verwendet. Die Hochdurchsatz-Sequenzierung ermöglicht erstmalig auch eine quantitative Bestimmung von Mutationen im Tumor. Aktuelle Studien weisen darauf hin, dass diese quantitative Bestimmung von Mutationen für die Prognose und Prädiktion von Bedeutung sein kann. Mit der in der vorliegenden Arbeit eingesetzten Technik soll anhand von in pulmonalen Adenokarzinomen vorliegenden EGFR- und KRAS-Mutationen herausgefunden werden, ob auch die semiquantitative Bestimmung von Mutationen – die durch die Sanger-Sequenzierung retrospektiv ermöglicht wird – von diagnostischer und klinischer Relevanz ist. In dem Kollektiv von 376 Patienten mit EGFR-Mutationsanalyse wurden bei 35 Patienten (9 %) aktivierende Mutationen im EGFR-Gen nachgewiesen. Die semiquantitative Mutationslast konnte bei 34 dieser Patienten ermittelt werden. Eine hohe EGFR-Mutationslast ging bei Patienten in den UICC-Stadien IIIB-IV mit einem längeren krankheitsspezifischen Überleben einher. Am häufigsten waren nicht-synonyme Mutationen im KRAS Exon 2 auffindbar. In dem Kollektiv von 104 Patienten mit KRAS-Mutationsanalyse konnten bei 31 Patienten (30 %) KRAS-Mutationen nachgewiesen werden. Die semiquantitative Mutationslast konnte bei 23 dieser Patienten ermittelt werden. Eine hohe KRAS-Mutationslast traf signifikant häufig mit dem Status „N0“ zusammen. Die Arbeit weist darauf hin, dass es durch die semiquantitative Analyse möglich ist, Mutationsträger mit pulmonalem Adenokarzinom noch differenzierter zu betrachten: Demnach ähneln die klinisch- histopathologischen Standardparameter und die Prognose von Patienten mit niedriger Mutationslast mehr denen von Patienten ohne Mutation als denen von Patienten mit hoher Mutationslast. Dieses Ergebnis könnte durch Mutationslastanalysen multipler prognoserelevanter Gene bestätigt werden, sodass auch eine prädiktive Wertung möglich ist.With approximately 57500 new cases a year lung cancer is one of the leading malignant neoplasms in Germany. Extrapulmonary metastases are present in over 50 % of cases at initial diagnosis. Therefore complete surgical excision is often not feasible and drug therapy is of great importance. Just a few years ago chemotherapy was the only possible form of drug therapy. It has recently been proven that tumors differ in genetic characteristics and thus offer multiple points of attack for new, targeted drugs. Pulmonary adenocarcinoma accounts, depending on gender, for about 36-46 % of all cases of pulmonary neoplasia. Depending on ethnicity and study, it harbors EGFR mutations in approximately 10-50 % of cases and KRAS mutations in approximately 15-30 % of cases. Nowadays prognostic-improving targeted drugs have already been established for patients with driver mutations in the EGFR gene, whereas targeted drugs for patients with KRAS mutations are still in development. In order to detect mutations, the Sanger sequencing method, which was developed in 1977, as well as the next-generation sequencing method, which was introduced in 2005, are used. For the first time, next- generation sequencing also allows a quantitative determination of mutations in the tumor. Recent studies indicate that this quantitative determination of mutations could be important for prognosis and prediction. The technique used in the present work aims to find out, on the basis of EGFR and KRAS mutations in pulmonary adenocarcinomas, whether the semiquantitative determination of mutations - which is also made possible retrospectively by Sanger sequencing - is of diagnostic and clinical relevance.
In a collection of 376 patients with EGFR mutation analysis, activating mutations were detected in 35 patients (9 %). The semi-quantitative mutation burden could be determined in 34 of these patients. High EGFR mutation burden was associated with longer tumor-specific survival in patients in UICC stages IIIB-IV. Exon 2 was analyzed for KRAS non-synonymous mutations. In the tumor group mutations were detected in 31 of 104 patients (30 %). The semi-quantitative mutation burden could be determined in 23 of these patients. High KRAS mutation burden significantly frequently coincided with "N0" status.
This work indicates that the semiquantitative analysis of mutation status allows prediction of clinical outcome. This study shows that the prognosis of patients with low mutation burden is more similar to that of patients without mutations than to that of patients with high mutation burden. This conclusion could be confirmed by mutation load analyses of multiple prognosis-relevant genes in terms of their predictive value
PI3K Inhibitors in the Treatment of PIK3CA-Mutated Breast Cancer: A Methodological Comparison of Molecular Pathology Analysis Techniques for the Detection of PIK3CA Variants in Long-Term Preserved Tumor Tissue
Einleitung
Das Mammakarzinom ist das häufigste Karzinom der Frau. Neben den konventionellen Therapieansätzen steht, nach der SOLAR1 Studie von Andre et al. aus dem Jahr 2019, mit dem PI3K-Inhibitor Alpelisib in Kombination mit Fulvestrant seit 2020 eine weitere Behandlungsmöglichkeit für das PIK3CA-mutierte, Hormonrezeptorpositive, HER2neu-negative, endokrin vortherapierte, lokal fortgeschrittene oder metastasierte Mammakarzinom zur Verfügung. Voraussetzung für den Einsatz dieser Medikamentenkombination ist der vorherige Nachweis einer PIK3CA-Mutation durch „liquid-biopsy“, oder aus einer Gewebeprobe. Aufgrund der Tatsache, dass es sich bei Alpelisib um eine Zweitlinientherapie handelt, welche erst 2020 zugelassen wurde, findet eine solche Testung aktuell vor allem nachträglich aus asserviertem, formalin-fixierten und paraffin-eingebetteten Tumorgewebe statt. Vor diesem Hintergrund ist es Ziel dieser methodischen Arbeit die Validität der drei gängigsten Methoden, Sequenzierung nach Sanger, Echtzeit-PCR und das „Next-Generation-Sequencing“, für den Nachweis oder Ausschluss von PIK3CA-Mutationen im Langzeit-archiviertem Tumorgewebe zu vergleichen und somit möglicherweise eine präferenzielle Empfehlung für die molekularpathologische Diagnostik zu geben.
Material und Methodik
Untersucht wurde in neutral-gepuffertem Formalin fixiertes und in Paraffin eingebettetes Überschussmaterial von Mamma-Resektaten aus den Jahren 2006 bis 2011. Das Untersuchungsmaterial besteht in dieser Arbeit ausschließlich aus DNA-Proben, welche aus nicht-spezifischer-Typ- (NST) lobulären und kombinierten Karzinomen der Mamma isoliert wurden. Das verwendete Probenmaterial wurde ursprünglich in der Klinik für Frauenheilkunde, Geburtshilfe und Reproduktionsmedizin (Leitung: Prof. Dr. E.-F. Solomayer) am Universitätsklinikum des Saarlandes (UKS) gewonnen und im Institut für Allgemeine und Spezielle Pathologie am UKS (Leitung: Prof. Dr. Rainer M. Bohle) nach leitliniengerechter Diagnostik untersucht. In dieser Arbeit wurden die Proben mittels Sequenzierung nach Sanger, Quantitativer Echtzeit-PCR und Next-Generation-Sequencing (NGS) untersucht. Anschließend wurden die molekulargenetischen Untersuchungsmethoden unabhängig voneinander im Hinblick auf die Mutationsfrequenz unter Berücksichtigung der Mutationsloki Exon 7, Exon 9 und Exon 20 des PIK3CA und der intrinsischen Subtypen Luminal A, Luminal B, HER2-positiv und Triple-negativ ausgewertet. Zusätzlich wurde mittels Cohens´s Kappa die Reliabilität der drei verwendeten PIK3CA-Mutationsanalyseverfahren statistisch ermittelt.
Ergebnisse
Von den 59 Patientinnen gehören 22 zu der Gruppe HR-positiv und HER2/neu-negativ. Das Vorhandensein eines PIK3CA-Pseudogens auf dem Chromosom 22 erschwerte die Mutationsanalyse mittels Sequenzierung nach Sanger. Bei circa 75% der Proben konnte mittels Sequenzierung nach Sanger das Exon 9 des PIK3CA nicht vollständig analysiert werden. In der Echtzeit-PCR konnte eine Probe nicht analysiert werden. Mit der NGS konnten alle Patientinnen-Proben analysiert werden. Die Sensitivität der Sequenzierung nach Sanger lag in dieser Arbeit bei 33%. Die Spezifität liegt in dieser Arbeit für die Sequenzierung nach Sanger bei 100%. Die Quantitative Echtzeit-PCR und das Next-Generation-Sequencing hatten nach Altman eine sehr gute und nach Landis und Koch eine fast perfekte Reliabilität.
Schlussfolgerung
Zusammenfassend ergibt sich, dass für eine primär separate PIK3CA-Mutationsanalyse aus mehrere Jahre als Paraffinblock asserviertem Tumorgewebe zunächst die Sequenzierung nach Sanger verwendet werden sollte. Falls die Sequenzierung nach Sanger den Wildtyp oder ein inkomplettes Ergebnis nachweist, sollte eine erneute Analyse mittels NGS stattfinden. Detektiert die Sequenzierung nach Sanger eine Mutation, ist keine weitere Analyse notwendig.Introduction
Breast cancer is the most common cancer in women. In addition to conventional treatment approaches, the PI3K inhibitor alpelisib in combination with fulvestrant has been available since 2020 as a further treatment option for PIK3CA-mutated, hormone receptor-positive, HER2-negative, endocrine-pretreated, locally advanced or metastatic breast cancer following the SOLAR1 study by Andre et al. in 2019. The prerequisite for the use of this drug combination is the prior detection of a PIK3CA mutation by liquid biopsy or from a tissue sample. Since alpelisib is a second-line therapy that was only approved in 2020, such testing is currently mainly carried out retrospectively from preserved excess tissue. Against this background, the aim of this methodological work is to practically compare the three established methods, sequencing according to Sanger, real-time PCR and next-generation sequencing, for this application and thus provide a recommendation for practice.
Methods and Material
Excess material from breast resections obtained between 2006 to 2011. The tissue analyzed was formalin-fixed and paraffin-embedded. The test material in this study consists exclusively of DNA samples isolated from lobular, NST and combined carcinomas of the breast. The sample material used was originally obtained at the Department of Gynaecology, Obstetrics and Reproductive Medicine (Head: Prof. Dr E.-F. Solomayer) at Saarland University Hospital (UKS) and examined at the Institute of General and Special Pathology at UKS (Head: Prof. Dr Rainer M. Bohle) according to guideline-based diagnostics. In this study, the samples were analysed using Sanger sequencing, quantitative real-time PCR and next-generation sequencing. The molecular genetic examination methods were then analysed independently of each other regarding detection of mutation frequency at loci in the PIK3CA gene in exon 7, exon 9 and exon 20 in the intrinsic breast cancer subtypes luminal A, luminal B, HER2-positive and triple-negative. In addition, the reliability of the three PIK3CA mutation analysis methods used was calculated and statistically analysed using Cohen's Kappa.
Results
Of the 59 patients, 22 belonged to the HR-positive and HER2/neu-negative group. The presence of a PIK3CA pseudogene on chromosome 22 made it difficult to analyse the mutation using Sanger sequencing. In approximately 75% of the samples, exon 9 could not be fully analysed by Sanger sequencing. One sample could not be analysed using quantitative real-time PCR. All patient samples could be analysed using NGS. The sensitivity of Sanger sequencing in this study was 33%. The specificity for Sanger sequencing in this study was 100%. Next-generation-sequencing and real-time PCR showed a very good reliability according to Altman and almost perfect reliability according to Landis and Koch.
Conclusion
In summary, Sanger sequencing should initially be used for a PIK3CA mutation analysis from long time preserved paraffin-embedded tumour tissue. If Sanger sequencing is negative or detects only the wild type gene, a new analysis should be performed using NGS. If Sanger sequencing detects a mutation, no further analysis is necessary
MicroRNA-Expression von primären Plattenepithelkarzinomen der Lunge in Abhängigkeit vom histologischen Grading
Hintergründe:
MicroRNAs sind einsträngige, kurze, nicht (für ein Protein) kodierende Ribonukleinsäuren, welche die Proteintranslation regulieren. Sie sind dadurch in der Lage in eine Vielzahl von molekularbiologischen als auch -pathologischen Prozessen einzugreifen. Im Hinblick auf maligne Erkrankungen konnte für zahlreiche Karzinome eine aberrante microRNA-Expression im Vergleich zu tumorfreiem Gewebe nachgewiesen werden. In nicht kleinzelligen Bronchialkarzinomen sind sie durch Dysregulation ihrer Zielproteine an Entstehung, Progression und Metastasierung beteiligt. Für das pulmonale Adenokarzinom konnte zudem ein Zusammenhang zwischen einer aberranten microRNA-Expression und der Tumordifferenzierung nachgewiesen werden. Ziel dieser Arbeit war es, zu überprüfen, ob sich für das primäre Plattenepithelkarzinom der Lunge ein Zusammenhang zwischen der Dysregulation bestimmter microRNAs und der Tumordifferenzierung sowie weiteren klinischpathologischen Parametern ergibt.
Material und Methoden:
Insgesamt wurden N = 45 Patienten, bei denen ein primäres Plattenepithelkarzinom der Lunge reseziert wurde, pseudonymisiert in diese Studie eingeschlossen. Die für die Diagnostik verwendeten und asservierten Schnittpräparate wurden erneut mikroskopiert. Die histopathologischen Befunde wurden, wenn nötig, entsprechend der zum Zeitpunkt dieser Arbeit gültigen TNM-Klassifikation (8. aktualisierte Auflage, 2020) aktualisiert. Zudem wurde der Differenzierungsgrad (sogenanntes „Grading“) der Plattenepithelkarzinome anhand zweier unterschiedlicher Schemata ermittelt: Neben dem bislang in der pathologischen Routinediagnostik angewandten Schema, das im Wesentlichen die Zell- und Zellkernmorphologie bewertet, wurde ein neueres Schema eingesetzt, welches anhand der Phänomene „tumor budding“ und „tumor nesting“ das Wachstumsmuster der Plattenepithelkarzinome an der Invasionsfront beurteilt. Nach RNA-Extraktion aus kryoasservierten, gepaarten Tumor- und Lungenproben wurden mittels Microarray-Analyse (N = 2550 microRNAs) an einer kleineren, zufällig ausgewählten Subgruppe (N = 18) die microRNA-Expressionsunterschiede zwischen Lungen- und Tumorgewebe untersucht. Für neun der dysregulierten microRNAs, welche nach Literaturrecherche in verschiedenen Karzinomen mit Tumorinvasion und -metastasierung assoziiert sind, wurde die Expression mittels quantitativer Polymerasekettenreaktion am Gesamtkollektiv analysiert.
Ergebnisse:
In der Microarray-Analyse ließen sich insgesamt 79 differenziell exprimierte microRNAs nachweisen. In der nachfolgenden Polymerasekettenreaktion konnte für acht microRNAs eine Hoch- (miR-31-5p, miR-205-5p, und miR-210-3p) beziehungsweise Herabregulation (miR-126-3p, miR-144-3p, miR-338-3p und miR-375-3p) bestätigt werden. Für miR-574-5p ließ sich eine Dysregulation nicht bestätigen. Pulmonale Plattenepithelkarzinome mit histologischem Nachweis einer Hämangiosis carcinomatosa zeigten eine signifikant niedrige Expression von miR-126-3p. Zudem ergab sich für das neuere Grading-System mittels der Phänomene „tumor budding“ und „tumor nesting“ an der Invasionsfront ein signifikanter negativer Zusammenhang zwischen einer Expression von miR-338-3p und miR-126-3p und der Tumordifferenzierung. Für die weiteren klinisch-pathologischen Parameter konnte am untersuchten Kollektiv kein signifikanter Zusammenhang mit der microRNA-Expression nachgewiesen werden.
Schlussfolgerung:
Die Phänomene „tumor budding“ und „tumor nesting“ gelten als morphologisch erfassbare Korrelate der sogenannten epithelialen-mesenchymalen Transition. Die damit bezeichnete Lösung von Karzinomzellen aus dem Zellverband und Akquirierung mesenchymaler Eigenschaften gilt als wesentlicher Bestandteil der Invasion und Metastasierung. Die Ergebnisse zeigen an einem Kollektiv resezierter Plattenepithelkarzinomen der Lunge, dass die Herabregulation von miR-338-3p und miR-126-3p unabhängig von Tumorstadium, Alter und Geschlecht der Patienten mit einer prognostisch ungünstigen Differenzierung der Plattenepithelkarzinome und für miR-126-3p zusätzlich mit einer Hämangiosis carcinomatosa assoziiert ist. So scheinen diese microRNAs, als Suppressoren der epithelialen mesenchymalen Transition, die Tumorinvasion und -metastasierung zu beeinflussen. Ihre Herabregulation im Vergleich zum Normalgewebe stellt somit einen weiteren, einfach messbaren prognostischen Faktor und gegebenenfalls einen potenziellen Therapieansatz dar.Background:
MicroRNAs are single-stranded, short, non-coding ribonucleic acids that regulate protein translation. Therefore, they are able to control a multitude of molecular biological and pathological processes. Regarding malignant diseases, aberrant microRNA expression is known for many carcinomas. For non-small cell lung cancer they are involved in cancer initiation, progression, and metastasis through dysregulation of their target proteins. In addition, a connection between dysregulated microRNA expression and tumor differentiation could be demonstrated for pulmonary adenocarcinomas. The aim of this work was to check whether there is a connection between the expression of certain microRNAs and tumor differentiation or other clinical-pathological parameters in primary squamous cell carcinomas of the lung.
Material and methods:
A total of N = 45 patients with a resected primary squamous cell carcinoma of the lung were included in this study. The preserved specimens, which were used for diagnostics, were again microscopically evaluated. The histopathological findings were updated in accordance with the latest TNM classification (8th updated edition, 2020) if necessary. In addition, the degree of differentiation (so-called grading) was recorded using two different grading schemes: in addition to the grading scheme used at the time of initial diagnosis, a newer scheme was also used. This grading scheme assesses the tumor growth pattern at the invasion front by quantifying the phenomena of “tumor budding” and “tumor nesting”. After RNA extraction from paired tumor and tumor free lung tissue, microRNA expression was investigated by microarray analysis (N = 2550 microRNAs) on a smaller, randomly selected subgroup (N = 18). Nine of the dysregulated microRNAs, which are associated with tumor invasion and metastasis according to literature research were analyzed by quantitative polymerase chain reaction in the overall collective.
Results:
In the microarray analysis, a total of 79 differentially expressed microRNAs could be detected. In the subsequent polymerase chain reaction, the downregulation (miR-31-5p, miR-205-5p, and miR-210-3p) or upregulation (miR-126-3p, miR-144-3p, miR-338-3p and miR-375-3p) was confirmed for eight microRNAs. For miR-574-5p, dysregulation was not proven. Primary squamous cell carcinomas of the lung with histological evidence of blood vessel invasion showed a significantly lower expression of miR-126-3p. In addition, a significant correlation between downregulation of miR-338-3p and miR-126-3p and lower tumor differentiation was found for the new grading system using "tumor budding" and "tumor nesting". For the other clinico-pathological parameters, no significant connection with microRNA expression could be demonstrated.
Conclusion:
The phenomena of "tumor budding" and "tumor nesting" are regarded as morphological correlates of the so-called epithelial-mesenchymal transition. This molecularpathological process, which describes detachment of an individual carcinoma cell out of the cell group and the acquisition of mesenchymal properties, is considered an essential part of invasion and metastasis. The results of the presented study show in a group of resected specimens that the downregulation of miR-338-3p and miR-126-3p is associated with a poorer prognostic tumor grade of squamous cell carcinomas regardless of tumor stage, age and sex of the patients. Downregulation of miR-126-3p is also associated with blood vessel invasion. Thus, these microRNAs, as suppressors of the epithelial mesenchymal transition, might be able to inhibit tumor invasion and metastasis. Therefore, their underexpression is an easily measurable prognostic factor and possibly a potential therapeutic approach
Development of an miRNA signature for individual prognostic assessment in seminoma patients.
645 Background: Due to the risk of long-term toxicities in young non-metastatic seminoma patients, surveillance regimens nowadays are the standard of care. However, up to 20% of patients will relapse and undergo further treatment, requiring even greater efforts in terms of adjuvant therapy. miRNAs are known to be very stable and robust biomarkers. This study aims to establish a prognostic miRNA profile utilizing bioinformatics tools to differentiate between metastatic (met) and non-metastatic (nmet) seminomas. Methods: MiRNA was extracted from formalin-fixed paraffin-embedded sections. Using microarray analysis (Agilent Technology), two cohorts of patients with seminoma (24 patients each, 12 nmet and 12 met) were examined. The following criteria were used to preselect relevant miRNAs: absolute median log2 fold change >0.5, distance correlation >0.5, accuracy in single-miRNA classifier trained with leave-one-out cross-validation (met vs. nmet) >0.7. Validation was performed in 88 patients (47 nmet and 41 met) through qPCR using the TaqMan assay, with RNU48, miR-191-5p, and miR-361-5p serving as references. Statistical and bioinformatics analyses were performed using the Mann-Whitney U test and machine learning algorithms. Results: The bioinformatics analysis of microarrays identified 16 miRNAs exhibiting an accuracy >70% in differentiating met tumors from nmet ones. The validation of 6 miRNAs by qPCR revealed significant expression differences between met and nmet tumors for miRNAs miR-371a-5p, miR-512-3p, miR−509−3−5p, miR-517a-3p (p<0.05), dependent on the reference used. All 4 miRNAs are higher expressed in met tumors. Synchronous met tumors exhibit stronger differences in the expression of the above-mentioned miRNAs compared to metachronous met tumors, except for miR-509-3-5p. miR-371a-5p was found to be the most robust miRNA to distinguish both synchronous and metachronous met from nmet seminomas. Classifiers trained on the PCR data for different combinations of the 6 investigated miRNAs using leave-one-out cross-validation show a medium performance (average accuracy of 0.59) including one model based on 4 miRNAs with high accuracy of 0.99. Conclusions: By using machine-learning algorithms on microarray data, we have established a miRNA signature that can differentiate metastatic from non-metastatic seminomas with high reliability. Validation of 4 miRNAs by qPCR confirms the high prognostic value of miRNAs in seminoma patients. Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.
