Retrospektive Vergleichsuntersuchung zu verschiedenen Anästhesieregimen bei der Anlage von tension-free-vaginal tapes (TVT)
Introduction
The tension free vaginal tape (TVT) is a surgical procedure to treat female stress incontinence. Spinal anesthesia, general anesthesia and different regimes of analgosedation are possible approaches to provide anesthesia. This study was designed to compare these anesthesia procedures in terms of immediate perioperative medical and logistic parameters.
Patients and methods
150 patients undergoing the TVT procedure were included in a monocentric and retrospective study. Groups were formed by the method of anesthesia (spinal anesthesia, general anesthesia and analgosedation). From the anesthesia and surgery records data were obtained regarding perioperative time periods, unforeseen intraoperative events, the patient’s ability to cooperate during the cough stress test and the immediate postoperative therapy. The main anesthesia regimes spinal anesthesia, general anesthesia and analgosedation were compared. In one subgroup analysis the different regimes of analgosedation were investigated, a second analysis the different application forms of Propofol were compared (target controlled infusion (TCI) versus propofol total intravenous anesthesia (TIVA)).
Results
There were differences regarding logistic and medical parameters between the three compared methods of anesthesia, spinal anesthesia, general anesthesia and analgosedation. Spinal anesthesia leads to non-significant shorter process times and fewer immediate postoperative pain medication was necessary. All analgosedation regimens included remifentanil. Additional administration of Esketamin resulted in shorter process times but a non- significant increase of perioperative episodes of hypertension and postoperative nausea and emesis was observed. The use of propofol during analgosedation was associated with longer process times which was reduced significantly by application via TCI.

All anesthesia regimens are suitable for the TVT procedure. Each shows specific advantages and disadvantages. Propofol administration during analgosedation might lead to longer process times which could partially be compensated by TCI application. Ein Verfahren zur operativen Therapie der weiblichen Stressinkontinenz stellt die Implantation eines tension-free-vaginal Tapes (TVT) dar. Als mögliche Anästhesieverfahren gelten unter anderem die Spinalanästhesie, die Allgemeinanästhesie und unterschiedliche Regime der Analgosedierung. Ziel dieser Arbeit war der Vergleich der genannten Verfahren hinsichtlich unmittelbar perioperativer medizinischer und logistischer Parameter.
Patienten und Methoden
In eine monozentrische und retrospektive Studie konnten die Anästhesie- und Operationsprotokolle von 150 Patientinnen, die sich an der Universität Erlangen im Beobachtungszeitraum vom 2002 bis 2009 einer TVT-Operation unterzogen hatten, eingeschlossen werden. Aus den Protokollen wurden Daten zu den Anästhesie- und Operationszeiten, dem Auftreten intraoperativer Ereignisse, der Möglichkeit zur intraoperativen Kontrolle des TVT („Cough-Test“) sowie Daten zur postoperativen Therapie gewonnen. Die verschiedenen, während der TVT-Operationen angewandten Anästhesieverfahren, die Analgosedierung, die Spinal- und die Allgemeinanästhesie, wurden anhand der erhobenen Daten miteinander verglichen. Zudem wurden in Subgruppenanalysen die unterschiedlichen Analgosedierungsregime sowie die unterschiedlichen Applikationsformen von Propofol (target controlled infusion (TCI) und total intravenösen Anästhesie (TIVA) einander gegenübergestellt.
Ergebnisse und Beobachtungen
Alle drei Anästhesieverfahren zeigten erwartungsgemäß spezifische Vor- und Nachteile hinsichtlich der analysierten Parameter. Die Spinalanästhesie
zeichnete sich durch signifikant kürzere Prozesszeiten und durch einen tendenziell geringeren Bedarf an postoperativer Analgesie aus. Alle Analgosedierungsregime hatten Remifentanil als Bestandteil. Der zusätzliche Einsatz von Esketamin verkürzte die Prozesszeiten, führte jedoch tendenziell zu einer Zunahme von intraoperativen hypertensiven Entgleisungen und postoperativer Übelkeit und Erbrechen. Eine Anästhesie mit Propofol bewirkte signifikant längere Prozesszeiten, insbesondere dann, wenn Propofol nicht als TCI, sondern als TIVA appliziert wurde.
Praktische Schlussfolgerungen
Alle Verfahren sind mögliche Anästhesieformen für die TVT-Operation mit jeweils spezifischen Vor- und Nachteilen. Ist Propofol Bestandteil eines Analgosedierungsregimes, muss mit längeren Prozesszeiten gerechnet werden. Eine Applikation von Propofol per TCI scheint diesen unerwünschten Umstand teilweise umgehen zu können Hinsichtlich der Verwendung von Esketamin sind die guten Prozesszeiten gegen ein erhöhtes Nebenwirkungsspektrum abzuwägen
Untersuchung des Nutzens und der unmittelbaren Auswirkung von Schrittmacherimpulsen nach Defibrillation bei Kammerflimmern
Background.
Ventricular fibrillation of the ischemic heart is the most common reason for sudden cardiac death. Recurrent ventricular fibrillation is not uncommon in the ischemic heart. Treatment of ventricular fibrillation is defibrillation. During open heart surgery stabilizing of the heart rhythm through pacing is a widespread measure. For out-of-hospital resuscitation pacing is recommended only for bradycardia. The aim of the present study is to evaluate the effects of pacing after defibrillation of ventricular fibrillation under the idea of an “electric-stabilizing” mechanism of the pacemaker on the ischemic heart after ROSC.
Methods.
Similar to an open-heart cardiac surgery model 20 anesthetized pigs (35,7 ± 2,3 kg) underwent 5 minutes of untreated cardiac arrest after induction of ischemic ventricular fibrillation. Afterwards resuscitation according to the ERC-Guidelines was initiated. Animals of the PM-group (n=10) were treated with epimyocardial pacing after defibrillation, animals of the control-group (n=10) did not receive any pacing. Beside the incidence of recurrent ventricular fibrillation in both groups hemodynamic parameters including invasive cardiac output monitoring was registered. Arterial, central-venous and mixed-venous blood samplings were collected for blood gas analysis. After sustaining ROSC and a phase of 60 minutes follow-up the pigs were euthanized in deep anesthesia.
Results.
In 9 of 10 pigs in each group resuscitation was successful. The mean duration from induction of cardiac arrest to the first ROSC was 19:19 ± 06:40 minutes. After ROSC in 6 pigs in each group recurrent ventricular r fibrillation was observed. There was no difference in the incidence of recurrent ventricular fibrillation or duration till the reappearance of cardiac arrest in both groups. After ROSC, atrial fibrillation was the most common rhythm registered. Proarrhythmogenous effects with induction of ventricular fibrillation through the use of the pacemaker were observed. There was no difference in the hemodynamic parameters in both groups. The use of a continuous cardiac output pulmonary artery catheter with a thermal filament revealed grossly false-high values of cardiac output in comparison to the other invasive cardiac output monitoring systems used. Short time after induction of ventricular fibrillation high arterial potassium concentrations were observed in all pigs. There was no influence of hyperkalemia on the resuscitation success, no significant difference in the potassium concentrations was found in each group.
Conclusions.
There is no rhythm-stabilizing effect in the use of epimyocardial pacing after successful defibrillation of ischemic ventricular fibrillation in an open-heart porcine resuscitation model. Atrial fibrillation after ROSC was common and proarrhythmogenous effects of the use of a DDD-pacer were observed. There is no significant influence of pacemaker use on post-ROSC hemodynamics. High plasma levels of potassium in the arterial blood were observed short time after induction of cardiac arrest. There was no influence of hyperkalemia on resuscitation success in the present study. Further research is needed to evaluate a possible harmful influence on the heart rhythm of pacing in cardiac surgery under ischemic conditions. Additional studies are necessary to determine the presence of similar electrolyte disturbances in cardiac arrest in humans and if this might influence therapeutic implications in resuscitation efforts.Hintergrund und Ziele.
Kammerflimmern auf dem Boden einer kardialen Ischämie stellt die Hauptursache des plötzlichen Herztodes dar. Das ischämische Herz neigt zu rekurrierendem Kammerflimmern. Kammerflimmern wird am effektivsten mittels Defibrillation behandelt. Schrittmachertherapie zur Stabilisierung des Herzrhythmus auch nach erfolgreicher Defibrillation ist im Rahmen herzchirurgischer Operationen Routine. Die Anwendung von Schrittmacherimpulsen bei präklinischen Reanimationsbemühungen hat sich nur für bradykarde Rhythmusstörungen als effektiv erwiesen. Der Zweck der vorliegenden Studie ist es, den Nutzen und die unmittelbaren Auswirkungen von Schrittmacherimpulsen nach Defibrillation bei Kammerflimmern zu untersuchen unter der Vorstellung eines möglichen „elektrisch stabilisierenden“ Effektes der geordneten Schrittmacherimpulse auf das ischämische Herz unmittelbar nach Rückkehr zu einem spontanen kardialen Auswurf nach Defibrillation.
Methoden.
In Anlehnung an einen routinemäßigen herzchirurgischen Eingriff wurden bei 20 anästhesierten Schweinen (35,7 ± 2,3 kg) im Thorakotomie-Modell ischämisches Kammerflimmern von fünf Minuten Dauer induziert. Es schloss sich eine leitliniengerechte Reanimation mit Defibrillationen an. In der PM-Gruppe (n=10) erfolgte unmittelbar nach Defibrillation die Applikation von Schrittmacherimpulsen über epimyokardiale Elektroden, die Kontrollgruppe (n=10) wurde ohne Schrittmacheranwendung therapiert. Neben der Häufigkeit des Auftretens von rekurrierendem Kammerflimmern nach ROSC wurden das Herzzeitvolumen mit einer Dopplersonde, erweitertes hämodynamisches Monitoring und engmaschig arterielle, zentral-venöse und gemischt-venöse Blutgasanalysen durchgeführt. Nach erfolgreicher Reanimation schloss sich eine 60-minütige Nachbeobachtungszeit an. Hiernach wurden die Tiere in tiefer Narkose euthanasiert.
Ergebnisse und Beobachtungen.
Bei je 9 von 10 Tieren der PM- und Kontrollgruppe gelang eine erfolgreiche Wiederbelebung. Im Mittel dauerte es 19:19 ± 06:40 Minuten von der Induktion bis zum ersten ROSC. Hiernach trat bei je 6 Tieren der PM- bzw. Kontrollgruppe erneut Kammerflimmern auf. Es konnte kein Unterschied in der Häufigkeit von rekurrierendem Kammerflimmern oder der Zeitspanne bis zum Wiederauftreten eines Herz-Kreislaufstillstandes nach ROSC für beide Gruppen nachgewiesen werden. Bei einem Großteil der Tiere lag nach ROSC Vorhofflimmern vor. Es konnte ein proarrhythmogener Effekt des Schrittmachereinsatzes mit Induktion von Kammerflimmern beobachtet werden. Die hämodynamischen Parameter unterschieden sich für beide Gruppen nicht. Der Einsatz eines Pulmonaliskatheters mit Thermofilament lieferte unplausibel falsch-hohe Werte für das Herzzeitvolumen im Vergleich zu den anderen eingesetzten Messmethoden. Schon kurz nach Induktion des Kammerflimmerns zeigten sich bei allen Tieren ausgeprägte Hyperkaliämien. Dies hatte keinen Einfluss auf den Reanimationserfolg und unterschied sich für die PM- oder Kontrollgruppe nicht.
Schlussfolgerungen.
Der Einsatz von epimyokardialen Schrittmacherimpulsen nach erfolgreicher Defibrillation von ischämischem Kammerflimmern hat im vorliegenden Schweinemodell keinen protektiven rhythmusstabilisierenden Effekt. Darüber hinaus zeigten sich bei häufig vorliegendem Vorhofflimmern nach ROSC sogar proarrhythmogene Effekte durch den Einsatz eines DDD-Schrittmachers. Die Hämodynamik der Post-Reanimationsphase bleibt durch den Einsatz des Schrittmachers unbeeinflusst. Nach fünf Minuten unbehandeltem ischämischen Kammerflimmern zeigen sich im Schweinemodell ausgeprägte Hyperkaliämien ohne Einfluss auf den Reanimationserfolg. Weitere Untersuchungen sind notwendig, die Rahmenbedingungen eines schädlichen Einflusses beim epimyokardialen Schrittmachereinsatz und ein mögliches Vorliegen sowie therapeutische Folgerungen gestörter Elektrolyt-Homöostase beim Herz-Kreislaufstillstand des Menschen zu untersuchen
Arbeitsbelastung und ihr zugrunde liegende Einflussfaktoren im qualifizierten Krankentransport: Multiple lineare Regressionsanalyse einer Querschnitts- Fragebogenstudie.
Background
Human workload is a key factor for system performance, but data on emergency medical services (EMS) are scarce. We investigated paramedics’ workload and the influencing factors for non-emergency medical transfers. These missions make up a major part of EMS activities in Germany and are growing steadily in number.
Methods
Paramedics rated missions retrospectively through an online questionnaire. We used the NASA-Task Load Index (TLX) to quantify workload and asked about a variety of medical and procedural aspects for each mission. Teamwork was assessed by the Weller teamwork measurement tool (TMT). With a multiple linear regression model, we identified a set of factors leading to relevant increases or decreases in workload.
Results
A total of 194 non-emergency missions were analysed. Global workload was rated low (Mean = 27/100). In summary, 42.8% of missions were rated with a TLX under 20/100. TLX subscales revealed low task demands but a very positive self-perception of performance (Mean = 15/100). Teamwork gained high ratings (Mean TMT = 5.8/7), and good teamwork led to decreases in workload. Aggression events originating from patients and bystanders occurred frequently (n = 25, 12.9%) and increased workload significantly. Other factors affecting workload were the patient’s body weight and the transfer of patients with transmittable pathogens.
Conclusion
The workload during non-emergency medical transfers was low to very low, but performance perception was very positive, and no indicators of task underload were found. We identified several factors that led to workload increases. Future measures should attempt to better train paramedics for aggression incidents, to explore the usefulness of further technical aids in the transfer of obese patients and to reconsider standard operating procedures for missions with transmittable pathogens
Detektion von Vitalparametern mit nicht-linearer Schwingkreistechnologie: Evaluation eines nichtinvasiven Sensorsystems im porcinen Reanimationsmodell
Abstract
Background
Sudden cardiac death is still one of the leading causes of death in industrialized countries
[28]. Early recognition and prompt initiation of resuscitation measures is essential
to improve survival from cardiac arrest. A non-linear resonant circuit technology could
help lay bystanders to decide early if resuscitation is required. This noninvasive sensor
detects pulse and respiration on the surface of the skin in the jugular region. The sensor
detects the small movements in the surface of the skin and underlying tissue caused by
pulse waves and respiration. Therefore, the technology could be further used for singlepoint
detection of vital signs. First test results gave information about the practicality of
the sensor system and showed the dermal correlates of pulse and respiration in a raw
signal from the sensor [25]. The objective of the animal studies that followed was the
validation and evaluation of the sensor system regarding different levels of blood pressure
and clinical scenarios.
Methods
After approval of the regional council of Mittelfranken (AZ No. 54-2532.2-1/10) 20
German pigs with an age of 12 weeks were included in the study. The investigation was
part of a porcine resuscitation study, thus the animals were used for multiple purposes.
The non-linear resonant circuit sensor was fixed on the skin of the jugular region on
every pig using an adhesive pad. During the whole period of the resuscitation experiment
the sensor recorded a raw signal. Together with the sensor data, vital signs were
recorded and saved as wave forms and as absolute values. In the following analysis, 10
second periods of the sensor signal were compared with vital signs. The signal was visually
analysed and described, with special regard to the correlates of pulse and respiration,
their morphology and perceptibility at different hemodynamic levels.
Results
Cumulatively, in all 20 pigs, 5321,24 minutes of traces were recorded and could be analyzed.
The raw signal of the sensor system superimposes pulse and respiration curves.
The pulse artifacts were detected as short peaks in the curve, mostly broader than the
arterial pulse and displaying several little peaks. Possibly, venous pulses were recorded
as well. The morphology of the pulse correlates was very heterogeneous and highly in4
dividual. However, the respiration correlates were much more homogeneous. At least,
there were 3 patterns representing respiration movements: a sinusoidal baseline undulation,
an amplitude-modulated signal and periodical negative defections. Residual blood
flow, especially in the initial phase of cardiac arrest due to ventricular fibrillation depicted
as a lot of small, irregular peaks in the sensor recordings. During that period, the
smallest delimitable blood pressure amplitude was 2 mmHg. There was a typical sensor
signal during manual cardiac compressions. Both internal and external compressions
were shown as large, uniform and regular peaks. Correlates of respiration during cardiac
compressions were difficult to distinguish. A successful defibrillation was always reliably
detectable, as well as pulse correlates of return of spontaneous circulation. Arrhythmias,
e.g. extrasystoles, were represented. There was no correlation between varying
blood pressure levels and the amplitude of the pulse correlates in the sensor signal traces
based on visual analysis of the sensor signal. Additionally, the sensor signal was
prone to artifacts.
Conclusion
The results of this study show that the non-linear resonant circuit is able to detect correlates
of pulse and respiration even under extreme hemodynamic conditions. The large
morphological diversity and numerous movement artifacts preclude a reliable visual
analysis and evaluation of the raw signal. Providing an algorithm for raw analysis and
processing, multiple vital sign recording and monitoring solutions are conceivable.Zusammenfassung
Hintergrund und Ziele
Der plötzliche Herztod gehört nach wie vor zu den Haupttodesursachen in den Industrienationen
[28]. Die Prognose hängt ganz wesentlich von der schnellen Erkennung und
dem frühzeitigen Beginn von Reanimationsmaßnahmen ab. Als frühe Entscheidungshilfe
zur Reanimation steht insbesondere für Laien eine nicht-lineare Schwingkreistechnologie
zur Verfügung. Sie detektiert nicht invasiv Puls und Atmung mittels eines kleinen
Sensors auf der Hautoberfläche der Fossa Jugularis indem die durch Puls und Atmung
entstehenden Bewegungskorrelate abgeleitet werden. Deshalb könnte die Technologie
zusätzlich zur single-point Vitalparameterdetektion eingesetzt werden. Aus Labormodellen
ließen sich erste Aussagen über die Funktionalität ableiten und in einer Rohdatenkurve
Puls- und Atmungskorrelate darstellen [25]. Ziel der anschließenden, tierexperimentellen
und hier dargestellten Untersuchung war die Validierung und Evaluierung
des Sensorsystems unter verschiedenen physiologischen Kreislaufbedingungen.
Methoden
Nach der Genehmigung durch das Regierungspräsidium Mittelfranken (Aktenzeichen
Nr. 54-2532.2-1/10) wurde an 20 zwölf Wochen alten deutschen Hausschweinen (Rasse
„Deutsches Landschwein“) im Rahmen einer Mehrfachverwendung der Tiere in einem
Reanimationsversuch ein nicht-linearer Schwingkreissensor auf die Jugularregion des
Tieres geklebt. Während des gesamten Versuchsablaufes wurde die Rohdatenkurve zusammen
mit den Vitalparametern aus den Überwachungsmonitoren als Kurvenform und
in Absolutwerten aufgezeichnet und gespeichert. Die anschließende Bewertung des
Sensorsignals erfolgte rein visuell. Dabei wurden immer 10 Sekunden des Signals betrachtet
und mit den Vitalparametern verglichen. Das Hauptaugenmerk lag in der Beschreibung
der Puls- und Atmungskorrelate und deren Morphologie, sowie deren Erkennbarkeit
in den verschiedensten Kreislaufsituationen.
Ergebnisse
Alle Aufzeichnungen über 5321,24 Minuten Kurvenmaterial der 20 Versuchstiere konnten
in die Analyse einbezogen werden. Das Rohsignal des Sensorsystems bestand
grundsätzlich aus einer Überlagerung von Puls- und Atmungssignal. Die Pulse waren
als kurze Ausschläge meist nach oben zu erkennen, sie waren meist breiter als die arte2
riellen Pulsausschläge und wiesen oft mehrere Gipfel auf, die möglicherweise durch den
Venenpuls mit bedingt waren. Die sehr heterogene Morphologie ließ kein einheitliches
Muster beim Puls und eine hohe Individualität des Signals erkennen. Die Atmung zeigte
ein deutlich homogeneres Bild. Hier ließen sich 3 Muster voneinander abgrenzen: eine
sinuskurvenartige Grundlinienschwankung, ein amplitudenmoduliertes Signal und regelmäßige
Ausschläge nach unten. Während Kammerflimmerphasen waren im Sensorsignal
viele kleine, unregelmäßige Ausschläge zu erkennen. Diese deuteten auf kleine
intravasale Restflüsse während des Kammerflimmerns hin. Dabei war die niedrigste
abgebildete Blutdruckamplitude 2 mmHg. Die Herzdruckmassage zeigte sich sowohl
bei externer als auch bei interner Durchführung in sehr einheitlichen, großen und regelmäßigen
Ausschlägen. Atmungskorrelate ließen sich während der Herzdruckmassage
nur schwer abgrenzen. Nach erfolgreicher Defibrillation konnte das Sensorsystem einen
Spontankreislauf stets zuverlässig detektieren. Auch Arrhythmien wie Extrasystolen
waren im Signal sichtbar. Eine Abhängigkeit des Signals vom arteriellen Blutdruck
konnte durch rein visuelle Bewertung nicht erkannt werden. Zusätzlich zeigte sich das
Signal als anfällig für Artefakte.
Schlussfolgerungen
Die Ergebnisse dieser Arbeit zeigen, dass das Sensorsystem in der Lage ist, Korrelate
von Puls und Atmung auch in extremen Kreislaufsituationen zu detektieren. Allerdings
ist durch die große Heterogenität und Anfälligkeit für Artefakte eine visuelle Analyse
und Auswertung des Rohsignals zu unzuverlässig. Mit einer technischen Prozessierung
des Signals wären verschiedene Anwendungen in der Vitalparameterdetektion und -
überwachung denkbar
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. Dispelling the Myths Behind First-author Citation Counts
We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued
use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation
counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases.
