Maxillary sinusitis caused by dental implants extending into the maxillary sinus and the nasal cavities.
This report describes the case of a patient who underwent osseointegrated dental implant placement. The implants were misplaced inside the nasal fossae and in the right maxillary sinus, causing chronic purulent sinusitis. CT scan without contrast showed signs of right maxillary sinusitis and confirmed the misplacement of four dental implants that surfaced into the nasal cavities. The imaging also revealed the presence of another implant that emerged inside the maxillary sinus. The patient underwent functional endoscopic sinus surgery with complete symptom remission at the long-term follow-up. We propose that sinusitis caused by protrusion of implants and by sinus floor lift procedures could share common physiopathological patterns and predisposing factors

Tutti i pazienti hanno avutorisultati funzionali ed estetici soddisfacenti. La morbilità del sito donatore è stata trascurabile. Il lembo sovraclaveare potrebbe non soloessere un’alternativa alla ricostruzione con lembi liberi, ma anche un’opzione di prima scelta nella chirurgia ricostruttiva cervico faccialeHead and neck surgery has witnessed an increase in microvascular reconstructive procedures with free flaps over the last 20 years as theyoffer efficient functional recovery. Nevertheless, under certain circumstances they may be contraindicated or cannot be used. We presentthe use of supraclavicular pedicled flap in three patients with different recipient sites. All patients had acceptable functional and aestheticoutcomes. Donor-site morbidity was satisfactory. Supraclavicular pedicled flap is not only an alternative to free flap reconstruction, but alsoa first-choice option in head and neck reconstructive surgery
"MIning" the sentiment during Covid19 pandemic
Influenced by the health emergency caused by the Covid-19 pandemic, many brands have changed their communication strategy, introducing more or less explicit references to principles of solidarity and fraternity in their TV commercial, to enhance trust and hope in Italian families during the lockdown. The traditional settings of the advertising format, focused on the product quality, have been moved to the background, in favour of the strengthening of the "brand image" by means of specific words, characters, hashtags and music, spreading empathetic messages to all those who need to regain hope and confidence, in a period of extreme emotional fragility. In this scenario, the paper aims to detect the emotions and the awareness raised by the brands during the lockdown period, compared to external periods, and to measure, through Text mining techniques, the sentiment of customers expressed on the social network Twitter. After a careful phase of extraction and pre-processing of tweets by means of Natural Language Processing algorithms, a dataset of 20,982 tweets was analysed, considering three different periods: pre-lockdown, during lockdown and post lockdown. The frequency analysis through the TF-IDF algorithm, firstly highlighted not only an increase in activity on the Twitter social network during the lockdown period of 3 times and half higher than the previous period, but, above all, a completely different use of the terms ‘Italia’, ‘Grazie’ and ‘Nuovo’, the most used in tweets, along with the hashtags '#iorestoacasa' and '#andràtuttobene’. The novelty in the advertising approach of the brands towards consumers, therefore, seems to have generated a certain sense of appreciation and gratitude, as well as a strong sense of belonging that is absent in the two other periods, in which it emerged that the TV commercial was commented on Twitter mostly for the brand's product or as an element of "disturbance" for other television events. The analyses of bi-grams and of co-occurrences highlighted that the commercials mostly drawing consumers’ interest were Lavazza and Barilla. In general, three reversals characterized the trend of the average daily “sentiment score”: a drop in “sentiment” just before the lockdown period, a strong resumption of positivity throughout it and a decline in the immediately following period. These results show how, by moving away from the traditional advertising approach, brands have managed, in order to enhance the amount of people talking about them, to reach the difficult goal of arousing an important positive mood in an extremely emotionally draining period; following, the trend has been reversed probably due to fatigue, and the first tastes of freedom

JOURNAL OF ANXIETY DISORDERS. 2018;53:30-38.The extent to which emotionally significant stimuli capture visual attention remains elusive because a preference for reporting or choosing emotionally significant stimuli could mimic attentional capture by these stimuli. We conducted two prior-entry experiments to disentangle whether phobic and fear-relevant stimuli capture attention or merely produce a response bias in spider-fearful participants. Prior entry denotes the effect that attended stimuli are perceived earlier than unattended stimuli as indicated by temporal order judgments. We presented phobic (spiders), fear-relevant (snakes) and neutral stimuli in pairs with varying temporal onset. The participants' task was to indicate which stimulus was presented first (Experiment 1) or second (Experiment 2). In the first experiment, spider-fearful but not control participants indicated that they had perceived spiders as occurring earlier in time, suggesting a prior-entry effect for spiders in this group. But surprisingly, in the second experiment, spider-fearful participants indicated more frequently that they had seen spiders as being presented second. This finding rules out a genuine prior-entry effect and instead suggests a strong preference for the response option associated with the feared animal. This response bias may result from a hypervigilance toward the feared stimulus and contribute to maintaining avoidance behavior in individuals with specific phobias

The use of near-infrared autofluorescence during total laryngectomy with hemi-or total thyroidectomy
Purpose: The aim of this study is to describe the use of near-infrared autofluorescence (NIR-AF) to identify and preserve parathyroid glands (PGs) in a group of patients with advanced hypopharyngeal/laryngeal cancer undergone total (pharyngo)laryngectomy with hemi- or total thyroidectomy. Methods: At San Raffaele Hospital, Milan (Italy), from January 2021 to May 2021, 7 patients affected by cT4a laryngeal squamous cell carcinoma (SCC) underwent surgery using an autofluorescence detection system (Fluobeam–Fluoptics®). For proper surgical planning, the demolition phase envisaged extension of the intervention to 4 hemithyroidectomies and 3 total thyroidectomies associated, respectively, with homolateral or bilateral CCND. Serum calcium, ionized calcium, and parathyroid hormone (PTH) levels at post-operative day (POD) 1 and 2 and at 2 weeks after surgery were monitored. Finally, we compared the data obtained with a cohort of patients who underwent surgery without the adoption of NIR-AF. Results: With the use of NIR-AF, 18/20 PGs were identified, of which 7/18 were preserved exclusively thanks to the use of autoflorescence. The technique also made it possible to identify and isolate three PGs from the surgical specimen, which were subsequently transplanted only after intraoperative histological confirmation. On POD-1, 3/7 patients (42.8%) were hypocalcemic; on POD-2 and after 2 weeks only 1/7 patient (14.2%) was hypocalcemic. Comparing the two groups, we highlighted that the utilization of NIR-AF was related to a significant decrease of median serum (p = 0.026) and ionized calcium levels (p = 0.017) 2 weeks after surgery. Using this new technology, in no case did definitive histological examination reveal the presence of PGs in the surgical specimen, reaching an accuracy of 100%. Conclusions: In our cohort of patients who underwent total (pharyngo)laryngectomy with hemi- or total thyroidectomy, the use of near-infrared autofluorescence improved medium term postoperative hypocalcemia rates. This new technology helps to achieve a better calcemic outcome compared to the standard naked eye approach, since it helps the surgeon to identify and preserve parathyroid glandular tissue with a lower incidence of post-operative hypocalcemia

Dispelling the Myths Behind First-author Citation Counts
We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued
use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation
counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more
