Comparing traditional and “in-motion” intense pulsed light techniques for hair removal. a split study
Background and Objectives: Hair removal is a common aesthetic concern for patients referred to dermatologists and aesthetic physicians. Lasers and lights are one of the mainstays in the management of this condition. Among these devices, intense pulsed lights (IPLs) are broadly used in order to reduce the number and width of the hair present. Currently used techniques are associated with a high risk of side effects, such as hyper or hypopigmentation. Materials and Methods: Thirty patients seeking hair removal in one or more body areas with skin phototypes 1 to 4 were recruited to perform this study. All areas to be treated were divided into two equal regions; one side was treated with the standard IPL hair removal procedure, while the other was treated with a new “in-motion” IPL technology. Results and hair removal rates were evaluated six months after the last treatment. Results: Out of the 30 patients treated, all patients experienced hair reduction. No statistically significant difference in hair removal was noted among the two sides. A statistically significant reduction in pain during the procedure was observed in the side treated with the “in-motion” technique. Conclusions: Traditional and “in-motion” IPL techniques have similar results in hair removal; the “in-motion” technology seems to guarantee a better safety profile than the traditional technique, as well as maintains the same results over time and a faster treatment time. A more extensive clinical study will be necessary to confirm our study’s results
Q-switched Nd:YAG laser to manage hyperpigmentation in asians. a multicenter study
(1) Introduction: In cosmetic dermatology, benign hyperpigmentation is a prevalent issue. Solar lentigo and other melanoses are bringing patients to the attention of physicians more and more often. The Q-switched 1064/532-nanometer (nm) laser is the gold standard device to treat these conditions. A new generation of these machines, able to shorten pulse duration to hundreds of picoseconds, has emerged showing better preliminary results. In order to treat hypermelanoses in Asians, we used a Q-switched laser with a picosecond pulse. (2) Methods: A retrospective recruitment of 31 Asian individuals requesting treatment for melanosis was conducted at Catanzaro’s Magna Graecia University, University of Rome Tor Vergata and Villa Bella Antiaging. Although they might have varied, the treatment settings were typically as follows: for dermic lesions, a pulse wavelength of 1064 nm with a duration of 450 picoseconds (ps) was used, and for epidermal lesions, a pulse wavelength of 532 nm with a duration of 370 ps was used. Up to four laser treatments were carried out, each at least 30 days apart. During a three-month follow-up after the final session, patient satisfaction was evaluated using a visual analogue scale (VAS). Images taken prior to laser treatments and thereafter were compared, and the aesthetic effect was scored on a five-point scale by two blinded specialists. (3) Results: Of the 31 patients recruited, 25 (80.6%) were female and 6 were male (19.4%). The average stated age was 48.96 ± 13.68. During the dermatological examination, all patients were found to have nearly completely removed melanosis, with a mean VAS score of 7.03 ± 1.35 and a five-point evaluation scale of 2.71 ± 0.78. Conclusions: The Q-switched 1064/532 nm laser can be seen as a safe and effective option to treat benign hypermelanosis in Asian patients. Compared to other technologies, the use of a picosecond pulse appears to promise superior outcomes. To validate the results of this investigation, however, a clinical trial comparing the many commercially accessible therapy options is required
Autologous platelet-rich fibrin injections in the management of facial cutaneous sinus tracts secondary to medication-related osteonecrosis of the jaw
Medication-related osteonecrosis of the jaws (MRONJ) is an infectious complication of antiresorptive or antiangiogenic drug therapies. In severe stages of this disease cutaneous sinus tracts may be observed. Platelet-rich fibrin (PRF) is a second-generation platelet concentrate used in medicine and dentistry for to promote tissue healing. This report describes the management of facial cutaneous sinus tracts secondary to MRONJ with autologous PRF injections. Eight patients with the diagnosis MRONJ and facial sinus tracts were enrolled in this study and received treatment. MRONJ lesions underwent pharmacological and surgical treatment. Sinus tracts received 1-mL injections of PRF around the fistula using an insulin syringe once a week for four times starting from the day of the surgical treatment. After 4 weeks, six patients showed healing of the fistula and bone lesions, only one patient showed healing of the fistula, and no remission was reported in another one. All patients reported an improvement of the symptoms in the first 2 days after the treatment session. Patients were also satisfied from an aesthetic point of view. Further studies will be needed to determine if PRF is a valid therapeutic option in dermatology

CO2 laser treatment in idiopathic scrotal calcinosis. a case series
Introduction: Idiopathic scrotal calcinosis (ISC) is a benign and uncommon condition characterized by the presence of multiple and calcified nodules of the scrotum without disorders in the calcium/phosphorous metabolism. The condition is usually asymptomatic. Various treatments are available. Surgery is the traditional treatment for ISC, as it allows a histopathological exam. However, newer treatments, such as ablative lasers, have been proposed with very good results. Methods: In this article, we report our experience in the treatment of ISC with a CO2 laser. Five patients affected by ISC were enrolled for CO2 laser treatment. Results: All patients had excellent healing. Four patients reported minimal scarring. No bleeding was reported after the procedure. Two patients reported itching and were treated with oral antibiotics and antihistamine drugs. Conclusion: The CO2 super pulsed laser is a fast and effective way to treat ISC and may be an alternative to traditional surgery in this condition
Evaluation of the Economic and Environmental Sustainability of Livestock Farms in Inland Areas
The present research aimed to evaluate the economic and environmental sustainability of livestock farms in inland areas of the Cilento, Vallo di Diano and Alburni National Park (Southern Italy) and the convenience and possibility of activating forms of local economies. The study involved three types of grazing husbandries: one with only sheep and goats; one with only cattle; and one mixed, namely with cattle, sheep and goats. The profitability of the analyzed farms was compared through their gross profit and the net income of the farmer. To evaluate the convenience of the farms under study to activate forms of a short supply chain, the transformation value of the milk was used as the reference parameter. The environmental impact per farm and per adult bovine unit was assessed through the LCA methodology. The economic analysis showed that the survival of the analyzed farms is essentially linked to public subsidies, which in some cases represent more than 75% of the total output. Family enterprise plays a fundamental role in management decisions, in the size of animal breeding, and in investment decisions. Referring to environmental impacts, the analysis showed a lower sustainability of cattle farming, mainly due to the higher methane emissions during enteric fermentation. Despite all this, the ecosystem services provided by these semi-extensive farms in inland areas are significant, and therefore economic and environmental analyses should take them into account to enhance them and encourage farmers to remain in these often marginal areas
Local chemotherapy as an adjuvant treatment in unresectable squamous cell carcinoma. what do we know so far?
Background: Squamous cell carcinoma (SCC) is one of the most common cancers involving skin and oral mucosa. Although this condition’s gold-standard treatment is the surgical removal of the lesions, the physician must propose alternative treatments in some cases due to the patient’s ineligibility for surgery. Among the available alternative therapies, local chemotherapy may represent an initial treatment in combination with radiotherapy or systemic chemotherapy due to the low frequency of side-effects and the lack of necessity for expensive devices. Methods: In this paper, we review all available literature in various databases (PubMed, Scopus-Embase, Web of Science), proposing local chemotherapy as a treatment for cutaneous and oral SCC. Exclusion criteria included ocular lesions (where topical treatments are common), non-English language, and non-human studies. Results: We included 14 studies in this review. The majority were case reports and case series describing the treatment of non-resectable localized SCC with either imiquimod or 5-fluorouracil. We also analyzed small studies proposing combination treatments. Almost all studies reported an excellent clinical outcome, with a low risk of relapses in time. Conclusions: Resection of the lesion remains the gold-standard treatment for SCC. When this approach is not feasible, local chemotherapy may represent a treatment alternative, and it may also be associated with radiotherapy or systemic chemotherapy
An innovative dual-wavelength laser technique for atrophic acne scar management. a pilot study
Background and Objectives: Acne scars are one of the most disturbing and long-term symptoms of acne vulgaris, having a negative impact on a person’s physical, emotional, and social well-being. Aim: the purpose of the study was to evaluate the efficacy and post-treatment outcomes of a dual-wavelength system combining the irradiation of two wavelengths at 10,600 nm and 1540 nm in the management of facial atrophic acne scars. Materials and Methods: Four healthy adult volunteers aged 24–53 years were enrolled. The areas treated were the full face (two patients), cheeks (one patient), and forehead (one patient). A dual-wavelength system (1540 nm and 10,600 nm) was used for this study. Patients underwent 2–4 treatment sessions, and the treatments were performed once every 45–90 days. All possible side effects such as burning sensation, dyschromia, mild to moderate post-treatment erythema, bleeding, itching, edema, and crusting were checked. The index to assess edema and erythema was based on a four-point scale (none, mild, moderate, and severe) and was applied before and at 3-month follow-up (3 MFU) after the last treatment session. In addition, a patient assessment was conducted before treatment and at 3 MFU after the last treatment session. Results: For all patients examined, the edema index was mild, while for the erythema index, 3/4 patients experienced moderate and 1/4 patients experienced mild symptoms. The mean patient downtime was 5.8 ± 0.5 days. Concerning the patient assessment, 2/4 subjects showed excellent improvement, 1/4 patients showed good improvement, and 1/4 patients showed slight improvement. As shown by the photographic assessment, a noticeable improvement in skin texture and a substantial reduction in acne scars were observed at the end of treatment. Conclusions: This dual-wavelength laser technology has the potential to be an interesting and safe approach for acne scar treatment, with a low risk of scarring/hypopigmentation and a shorter healing time
Emerging role of anti-IL23 in the treatment of psoriasis. when humanized is very promising
Risankizumab is a novel anti-interleukin 23 humanized antibody developed to treat severe psoriasis. In this paper, we analyze the key information about this new drug and the results of phase 3 clinical trials already present in literature in order measure its safety and effectiveness in every day practice. Risankizumab seems to be one of the best performing drugs in the treatment of psoriasis, with a good safety profile and a dosage regimen less frequent than all other biologic agents, although head-to-head randomized clinical trials with other anti-interleukin treatments will be necessary in order to confirm these preliminary results
