Congenital imprinting disorders: application of multilocus and high throughput methods to decipher new pathomechanisms and improve their management

    No full text
Imprinting disorders (IDs) are a group of congenital diseases affecting growth, development and metabolism. They are caused by changes in the allele-specific regulation (“epigenetic mutation”) or in the genomic sequence (“genetic mutation”) of imprinted genes. Currently molecular tests in ID patients are generally restricted to single loci classically associated with the disease, but this approach limits diagnostic yield, because of the molecular and clinical heterogeneity between IDs. From the technical point of view, these limitations are aggravated by the lack of standardization in testing methodology, in the DNA sequences tested, and in clinical inclusion criteria prompting testing. However, an increasing number of new studies show that these problems can be addressed by the use of new tests targeting multiple loci and/or a total exome and genome analysis. The rapid development of efficient and high-throughput molecular techniques and their applications in research and diagnostics in the last decade have led to an impressive increase of knowledge on IDs and their basic pathomechanisms. In combination with the improvement of data recording and documentation, the diagnostic strategies are increasingly based on standardized protocols, and thereby provide the backbone for directed counseling, more personalized management, and new therapeutic approaches

Disturbed genomic imprinting and its relevance for human reproduction: causes and clinical consequences

    No full text
Background: Human reproductive issues affecting fetal and maternal health are caused by numerous exogenous and endogenous factors, of which the latter undoubtedly include genetic changes. Pathogenic variants in either maternal or offspring DNA are associated with effects on the offspring including clinical disorders and nonviable outcomes. Conversely, both fetal and maternal factors can affect maternal health during pregnancy. Recently, it has become evident that mammalian reproduction is influenced by genomic imprinting, an epigenetic phenomenon that regulates the expression of genes according to their parent from whom they are inherited. About 1% of human genes are normally expressed from only the maternally or paternally inherited gene copy. Since numerous imprinted genes are involved in (embryonic) growth and development, disturbance of their balanced expression can adversely affect these processes. Objective and Rationale: This review summarises current our understanding of genomic imprinting in relation to human ontogenesis and pregnancy and its relevance for reproductive medicine. Search Methods: Literature databases (Pubmed, Medline) were thoroughly searched for the role of imprinting in human reproductive failure. In particular, the terms 'multilocus imprinting disturbances, SCMC, NLRP/NALP, imprinting and reproduction' were used in various combinations. Outcomes: A range of molecular changes to specific groups of imprinted genes are associated with imprinting disorders, i.e. syndromes with recognisable clinical features including distinctive prenatal features. Whereas the majority of affected individuals exhibit alterations at single imprinted loci, some have multi-locus imprinting disturbances (MLID) with less predictable clinical features. Imprinting disturbances are also seen in some nonviable pregnancy outcomes, such as (recurrent) hydatidiform moles, which can therefore be regarded as a severe form of imprinting disorders. There is growing evidence that MLID can be caused by variants in the maternal genome altering the imprinting status of the oocyte and the embryo, i.e. maternal effect mutations. Pregnancies of women carrying maternal affect mutations can have different courses, ranging from miscarriages to birth of children with clinical features of various imprinting disorders. Wider Implications: Increasing understanding of imprinting disturbances and their clinical consequences have significant impacts on diagnostics, counselling and management in the context of human reproduction. Defining criteria for identifying pregnancies complicated by imprinting disorders facilitates early diagnosis and personalised management of both the mother and offspring. Identifying the molecular lesions underlying imprinting disturbances (e.g. maternal effect mutations) allows targeted counselling of the family and focused medical care in further pregnancies.</p

ANGLICKÁ ANOTACE Title: Patristical ideas in the works of Matthias of Janov The thesis treats of the identification of ideas of church fathers in the work and thinking of Matthias of Janov. The thesis consits of three main parts. First part deals with Matthias of Janov in broader context of church tradition. Major political and theological developments that could have an impact on Matthias theological work are presented in this section. Second part introduces life of Matthias of Janov and identifies his major theological contributions. In the third part of the thesis, following explanation of methodology used, the findings of the author are presented. Three areas were selected by the author and researched in view of the identification of possible influance of church fathers in the Matthias of Janov work: the idea of Antichrist, frequent eucharisty and condemnation of idolatry. The findings confirm a strong influance of Augustin and Hieronymus in particular and other early church fathers on the formation of the theology of Matthias of Janov in all three areas of the study. It was also noticed that through the church fathers Matthias of Janov was strongly influenced by neoplatonism which was documented on his idea of "The First Thruth" Key words: Matthias of Janov, church fathers, antichrist, eucharisty,..

(1477). U petom poglavlju autor daje kratki pregled Matijaševih odnosa s papinstvom, a u šestom pregled ratovanja s Osmanlijama tijekom sedamdesetih i osamdesetih godina XV. stoljeća. Deseto poglavlje daje pregled političkih aktivnosti tijekom posljednjih godina njegove vladavine. Jedanaesto poglavlje analizira Matijaševu vojsku. Rad završava kratkim pregledom posljednica kraljeve burne politike. This article is the second part of a longer paper in which the author reviews the very complex foreign policy of Hungarian-Croatian king Matthias Corvinus. In this paper the author analyses major events from Matthias’ coronation in the spring of 1464 until his sudden death in Vienna in the spring of 1490. The whole paper and analysis of events are based on research of various sources and literature. In the first chapter the author describes the failed attempt of Pope Pius II to organize a crusade against the Ottoman Empire in 1464 and Matthias’ own actions against the Turks with the main goal of strengthening his southeastern borders. In the second chapter the author analyses the conflict between Matthias and his ex-father-in-law, George of Poděbrady, and the war between them (1468–1471). In the third chapter the author gives a short overview of the uprising of magnates and the aristocracy against the central rule of King Matthias (1472). The fourth chapter deals with the war for the Bohemian throne from 1471 to 1479 and the war against Frederick III in 1477. In the next chapter the author gives a brief overview of the diplomatic relations between popes and Matthias Corvinus over a longer period of time, from 1464 to 1490. The main subject of the sixth chapter is the war against the Ottoman Empire during the seventh and eighth decade of the 15th century. In the seventh chapter the author describes Matthias’ search for his next wife and his marriage to Beatrice of Aragon. In the following chapter the author reviews Matthias’ new war against Frederick III for the title of the German Empire. The ninth chapter is a brief overview of Matthias’ foreign policy in the Italian states and Venice during the last decade of his life. The tenth chapter is dedicated to the final years of Matthias’ life. The next chapter analyses one of the most important segments of his foreign policy – his army. This article ends with short overview of the events which happened after the death of King Matthias and the consequences of his stormy policy
