Interdependence of the dimensions of the cochlea and scalar position of the electrode array
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
Long-time results of children with Cochlear Implant
Das CI ist ein entscheidendes Rehabilitationsinstrument von höchstgradig schwerhörigen oder tauben Patienten. Besonders für taub geborene Kinder stellt das Cochlear Implant nicht nur eine passive Teilhabe an der Umwelt sicher, sondern bildet über die frühe Wahrnehmung von Sprache die wesentliche Grundlage zum Spracherwerb. Im Rahmen der CI-Versorgung ist daher die Frage des „richtigen Zeitpunktes“ der Implantation von großer Bedeutung: Es gilt die operativen Risiken und eventuelle Entwicklungsfenster gegeneinander abzuwägen.
Im Rahmen dieser Auswertung betrachteten wir daher die Hörtestergebnisse von Patienten, die in Freiburg zwischen 1993-2004 operiert und rehabilitiert wurden. Um Entwicklungseffekte auszuschließen, lagen zwischen OP und Audiometrie mindestens 5 Jahre. Zur Testung wurden Standardsprachtests für Erwachsene verwandt.
Vor Vollendung des 2. Lebensjahres (im Median: 1,6) implantierte Kinder zeigen ein deutlich besseres Sprachverständnis. Dieser Unterschied besteht sowohl im Vergleich zu den später implantierten Kindern als auch den postlingual ertaubten Erwachsenen. Entscheidende Vorteile – putativ aufgrund der höheren Plastizität der Hörbahn – zeigen sich gegenüber den im Erwachsenenalter implantierten Patienten im Freiburger Einsilbertest. Dieser Vorteil verliert sich bei Anwendung des OLSA, der deutlich mehr Redundanz beinhaltet, die vermutlich den postlingual ertaubten Erwachsenen – aufgrund des zuvor weitgehend normal abgelaufenen Spracherwerb und daher besserer Kenntnis der Sprachsuprastruktur – eine Kompensation ermöglicht.
Der Vergleich mit der international publizierten Literatur liefert ähnliche Ergebnisse – wobei das dargestellte Kollektiv das zum Zeitpunkt der Erstellung dieser Arbeit das weltweit größte Kollektiv darstellt. Die Anwendung der Erwachsenensprachtests scheint die Kindern nicht benachteiligt zu haben. Bezüglich des Einflusses der Art und Entstehung der Schwerhörigkeit kann im Rahmen dieser Betrachtung keine Aussage gemacht werden.
Zusammenfassend gehen wir von einem deutlichen Vorteil früh implantierter Kinder in Hinsicht auf die Sprachperzeption aus. Im Zusammenspiel mit der flächendeckenden Einführung des Neugeborenen-Hörscreenings ist daher durch eine raschere Früherkennung auch eine frühere Implantation und damit ein insgesamt besseres Gesamtergebnis bei mehr Kindern zu erwarten.Regarding patients with deafness or profound hearing loss, cochlear implants are the best possibility to achieve hearing rehabilitation. This is especially true for congenital deaf children as cochlear implants constitute the link to the sounding environment and promote speech development. The right timeframe for implantation is critical: Perioperative risks are to be weighed against critical periods of development.
This study analyses speech perception results achieved by patients implanted and rehabilitated in Freiburg 1993-2004. The interval between surgery and audiometric testing spans at least 5 years. Perception was tested using standard speech tests for adults (Freiburg monosyllables, Freiburg numbers, Oldenburg sentence test).
Childres implanted before the age of 2 (median 1,6) achieved higher scores than children implanted at a later age. Compared to adults, children implanted early achieved significantly higher monosyllabic discrimination scores. Considering the OLSA results, early implanted children and adults achieved comparable scores. These effects may be attributed to higher plasticity of the auditory pathway in children respectively the higher familiarity with speech structure and context in adults.
Similar results have been published internationally. The collection described in this study comprised the largest number of subjects at time of publication. Although the audiologic tests were drafted for adults, children were able to perform well. Cause of deafness could not be detected to be a predictor of audiologic outcome.
Children implanted early show clear benefits in respect to the development of speech perception. Universal newborn hearing screening enables early detection and early treatment of congenital deafness and ensures better results for a growing group of children
Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
