Etude épidémiologique de la malnutrition en milieu nomade au Tchad : diagnostic et approche d'interventions
Abstract: Malnutrition is associated with 35-53% of child mortality. It is widespread in developing countries and particularly rampant in the remote rural communities of Africa. In these communities, the situation embedded in a context of food insecurity is aggravated by the seasonal variation, micro-nutrient deficiencies and intestinal parasitic infections.
The goal of this study was to strengthen the knowledge about the food and nutritional situation in a nomadic pastoralist zone and to provide to decision-makers baseline information to better response to the Millennium goals. The objective was to assess the nutritional status and the main risk factors of malnutrition, among children under 5 years of age and women in nomadic pastoralist and rural sedentary communities at the southern shores of Lake Chad. Specifically it aimed at: i) Measuring the prevalences of malnutrition among children and theirs mothers according to seasonal variation; ii) Determining the vitamin A deficiencies among children and theirs mother; iii) Measuring the prevalences of intestinal parasitic infection; anaemia and malaria among women and children under five; iv) Estimating the level of food security and access to health care, and v) Proposing adapted interventions.
Repeated cross-sectional surveys were conducted at the end of the dry season (May/June) and at the end of the wet season (October) in 2007 and 2008. Malnutrition and food security were assessed with anthropometric measurements and semi-structured interviews. Vitamin A and anaemia were evaluated with a portable devices, fluorometer (iCheckTM) and HemocueTM, respectively. Intestinal parasitic infections were assessed with stool microscopy and the Kato-Katz method and malaria was evaluated by ParacheckTM rapide test for Plasmodium falciparum.
A total of 653 nomadic and 579 sedentary children under 5 years of age and their mothers (398 and 376) were included in May/June and the same numbers in October 2007. Three hundred specimens were further collected in October 2008. As to women, 48% (95% CI: 42-53%) of nomadic and 16% (95% CI: 12-20%) of sedentary women (statistically significant at p<0.01) were malnourished, but also 4% (95% CI: 2.4-7%) of sedentary women were obese which was not observed among nomadic women. There was no significant difference between seasons for women. However, acute malnutrition varied from 17.9 % at the end of dry season to 13.7% at the end of wet season (p = 0.03) for children, with no difference between nomadic and sedentary children. Household dietary diversity scores were very low in both groups. High prevalences of intestinal parasitic infections were observed among women and children, 75% (95% CI: 68-83%) and 60% (95% CI: 53-66%), respectively. Malaria prevalences for P. falciparum were very low, less than 5% in both groups. The proportions of the blood retinol equivalent deficiencies were 19% (95% CI: 6-32%) among sedentary women and 25% (95% CI: 15-35%) among sedentary children. Nomadic communities had higher retinol levels than sedentary communities. Regression analysis showed a strong relationship between blood retinol level, and retinol levels in the consumed livestock milk and retinol in the mothers’ milk. Multivariable logistic regression analysis of malnutrition in children showed highly significant association (p<0.01) with anaemia and age and, among women, with anaemia.
Although the landscape bordering Lake Chad offers rather good food availability and accessibility to cereals and animal products, with a strong seasonal variation remains. The lack of regulative institutions, the social insecurity and the abuse of rights of administrator and military forces accentuate the fragile position of nomadic and sedentary rural communities. In the light of our results, both nomadic and rural sedentary communities were vulnerable. They had high malnutrition proportions, particularly among children during the dry season, high prevalences of parasitic infections and low household dietary diversity scores. Vitamin A deficiencies were important among the sedentary community, whereas anaemia prevalence was frequent in nomadic communities.
To improve this situation, some recommendation can be given for actions at the national and at household levels. At the national level, we propose a holistic and intersectoral approach with interventions aiming at the provision of a minimal service package in nomadic pastoral zones: i) Strengthen the institutional framework; ii) Provide potable water in nomadic communities; iii) Assure a minimal health package; and iv) Disseminate information on good practices. At the household level, facilitate community-based actions: i) Diversify the diet and promote the consumption of green vegetables and fruits; ii) Promote hygiene with use of latrines or burying of feces; iii) Give colostrum to newborns. These recommendations cannot be dissociated from the problems of accessibility to social services and pastoral development (education, health, accessibility to natural resources, and security of pastoral system). They can only be effective within holistic and intersectoral approaches that deal with the overarching problems influencing malnutrition and by involving the targeted communities in solution finding. ---------- Résumé: La malnutrition est directement ou indirectement associée à 35-53% aux causes de la mortalité des enfants dans les pays en développement, particulièrement dans les zones rurales difficilement accessibles. Cette situation est aggravée par les variations saisonnières, les déficits en micronutriments et les infestations parasitaires dans un contexte d’insécurité alimentaire chronique au Sahel.
Le but de cette étude est de contribuer à une meilleure connaissance de la situation alimentaire et nutritionnelle en milieu nomade et de fournir aux décideurs les informations de base pour mieux répondre aux objectifs du millénaire. L’objectif est d’étudier l’épidémiologie de la malnutrition en milieu nomade et sédentaire rural sur la rive sud-est du lac Tchad.
C’est une étude transversale répétée à la fin de la saison sèche (mai/juin) et à la fin de la saison des pluies (octobre) en 2007 et 2008. Les données pour l’évaluation de la malnutrition et l’alimentation ont été collectées par des interviews semi structurés et des mesures anthropométriques. Les évaluations de la vitamine A et de l’hémoglobine ont été réalisées par la fluorométrie portable iCheckTM et par l’HémocueTM respectivement. La recherche des parasites intestinaux a été faite à l’observation directe et par la méthode de Kato-Katz au microscope et enfin le paludisme a été indirectement évalué par le ParacheckTM test rapide du paludisme à Plasmodium falciparum.
Au total 653 enfants nomades et 579 enfants sédentaires de moins de 5 ans et leurs mères (398 et 376) ont été impliqués dans cette étude en mai/juin et le même nombre a été répété en octobre 2007. Les données sur les analyses biologiques ont été collectées en octobre 2008 sur un échantillon de 300 enfants de 0-5 ans et leurs mères. Chez les femmes nomades, 48% (95% IC : 42-53%) ont été malnutries contre 16% (95% IC : 12-20%) des femmes sédentaires (statistiquement significative à p<0,01). Par ailleurs, nous avons observé chez les femmes sédentaires un taux d’obésité de 4% (95% IC: 2,4-7%) et cela n’a pas été observé chez les nomades. Par contre, il n’y a pas de différence au sein de chaque groupe selon la variation saisonnière. Cependant, la malnutrition aigüe globale chez les enfants a varié de 17,9% à la fin de la saison sèche à 13,7% à la fin de la saison des pluies (p = 0,03) pour les enfants nomades, sans une différence significative entre les enfants nomades et sédentaires. La diversité alimentaire au niveau des ménages a été faible dans les deux groupes et parmi les mauvaises pratiques, apparaît le rejet du colostrum par les femmes nomades foulbés. Des prévalences élevées de parasitoses intestinales ont été observées aussi bien chez les nomades que chez les sédentaires. Elles sont de 75% (95% IC : 68-83%) chez les femmes et 60% (95% IC: 53-66%) chez les enfants. Les prévalences du paludisme à P. falciparum ont été très faibles en cette période et sont inférieures à 5% dans les deux groupes. Les proportions d’équivalent rétinol dans le sang étaient plus élevées chez les sédentaires. Elles sont de 19% (95% CI: 6-32%) chez les femmes sédentaires et 25% (95% CI: 15-35%) chez les enfants sédentaires. Les femmes et les enfants nomades ont eu un niveau appréciable en équivalent rétinol ; il est inférieur à 6%. Il y a une corrélation positive entre le niveau de rétinol dans le sang et le lait de consommation ainsi qu’avec le taux de rétinol retrouvé dans le lait des mères. Les analyses multivariées ont montré une association entre la malnutrition, l’anémie et l’âge chez les enfants et entre l’état nutritionnel et l’anémie chez les femmes (p<0,01).
Bien que la rive sud-est du lac Tchad offre une disponibilité et une accessibilité alimentaire plus ou moins appréciable en matière de la production animale et céréalière, elle reste tout de même très variable avec la saisonnalité. La faiblesse des institutions de régulation, l’insécurité sociale et les abus des autorités politico-militaires accentuent la fragilité des groupes nomades et sédentaires ruraux. Ainsi les pasteurs nomades et les sédentaires ruraux apparaissent à la lumière de ces résultats comme vulnérables, car les deux groupes ont une situation nutritionnelle non appréciable surtout en saison sèche chez les enfants. Ils ont présenté également des prévalences d’infestations parasitaires élevées et des diversités alimentaires très faibles. Si les carences en vitamine A sont très marquées chez les sédentaires, l’anémie quant à elle est plus accentuée chez les nomades.
Pour pallier à cette situation un certain nombre de propositions peuvent être faites au niveau national et au niveau des ménages. Au niveau national, nous proposons une approche holistique intersectorielle avec des interventions qui visent à fournir un paquet minimum de service en milieu nomade: i) Renforcer les cadres institutionnels ; ii) Favoriser l’accès à l’eau de qualité ; iii) Assurer un paquet minimum d’activités sanitaires et iv) Promouvoir l’éducation et la communication.
Au niveau des ménages, il faut faciliter les activités communautaires, plus spécifiquement, il faut : i) Inciter à la diversification des aliments de consommation selon la disponibilité régionale et de promouvoir la consommation des légumes verts et des fruits; ii) Mettre l’accent sur l’hygiène avec l’utilisation des latrines d’une part, et l’enfouissement des selles d’autres part, iii) Donner le colostrum au nouveau-né. Ces propositions d’amélioration ne peuvent être dissociées de toute la problématique de l’accessibilité aux services sociaux de base et du développement pastoral (éducation, santé, accessibilités aux ressources, et sécurisation des systèmes pastoraux). Elles ne peuvent être efficaces et efficientes que dans une approche holistique qui traite la globalité des problèmes qui influencent la malnutrition avec des interventions conjointes qui impliquent d’avantage les populations cibles
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued
use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation

We have done our best to complete the author checklist relating to the use of animals in the hut study. Note that the objective for the hut study was to evaluate the IRS treatment applications for residual efficacy against Anopheles mosquitoes, including the local An. coluzzii mosquito population. Cows were only used to attract mosquitoes into the huts and no tests were carried out directly on the cows. The author checklist is intended for use with studies where experiments are carried out on animals, which is why we have had such difficulty in completing this for the hut study, as many of the questions do not relate to how the cows were used
