Asuhan Keperawatan Bronkopneumonia pada An.Z Dan An.S dengan Masalah Keperawatan Ketidakefektifan Bersihan Jalan Napas di Ruang Bougenville RSUD Dr.Haryoto Lumajang Tahun 2018
No full textInsiden penyakit bronkopneumonia pada negara berkembang hampir 30% pada anak-anak di bawah umur 5 tahun dengan resiko kematian yang tinggi. Dari hasil studi pendahuluan yang dilakukan penulis pada bulan april 2018 di Ruang Bougenville RSUD dr. Haryoto Lumajang didapatkan 136 pasien anak yang mengalami bronkopneumonia dari bulan Januari 2018 hingga awal April 2018. Pada klien dengan Bronkopneumonia sering mengalami ketidakefektifan bersihan jalan napas yang disebabkan karena banyaknya lendir di bronkus/paru. Pada bayi dan anak-anak tidak mampu mengeluarkan sekret secara mandiri jadi jika masalah bersihan jalan nafas ini tidak ditangani secara cepat maka bisa menimbulkan masalah yang lebih berat saperti pasien akan mengalami sesak yang hebat bahkan bisa menimbulkan kematian.
Tujuan penulisan karya tulis ilmiah ini adalah untuk mengeksplorasi asuhan keperawatan Bronkopneumonia pada An.Z dan An.S dengan masalah keperawatan ketidakefektifan bersihan jalan napas. Desain yang digunakan adalah laporan kasus terhadap pasien Bronkopneumonia dengan menggunakan pengumpulan data berdasarkan lembar WOD (wawancara, observasi dan dokumentasi). Partisipan terdiri dari dua klien anak yang memenuhi criteria partisipan. Intervensi pemantauan pernapasan dilakukan selama perawatan klien diruangan dengan memantau kecepatan, irama, kedalaman, adanya pergerakan dinding dada abnornmal, pola napas dan auskultasi suara napas tambahan sebagai bukti adanya akumulasi cairan atau sekret di saluran napas. Intervensi yang dilakukan untuk mempercepat perbaikan jalan napas kedua klien adalah mengatur posisi kepala klien lebih tinggi dari badan, anjuran minum air hangat ataupun pemberian ASI, pemberian terapi nebulisasi dan fisioterapi dada. Intervensi ini dilakukan tiga hari selama perawatan di ruangan.
Implementasi keperawatan yang dilakukan kepada klien 1 dan 2 untuk perbaikan pernapasan adalah dengan mengatur posisi klien dengan posisi kepala lebih tinggi dari badan. Hal ini berguna untuk memaksimalkan ventilasi. Kedua klien diberikan terapi nebulisasi, tujuan utama penggunaan nebulizer adalah untuk menghilangkan obstruksi sekresi dan memperbaiki hygiene bronchus. Setelah diberikan terapi nebulisasi, tindakan selanjutnya adalah dilakukannya terapi fisioterapi dada dengan teknik clapping dada menggunakan dua atau tiga jari. Tujuan dari terapi clapping ini adalah jalan nafas bersih, secara mekanik dapat melepaskan sekret yang melekat pada dinding bronkus dan mempertahankan fungsi otot-otot pernafasan. Kedua klien juga dianjurkan diberi minum air hangat atau pemberian ASI guna mengurangi kekentalan dahak melalui proses induksi yang menyebabkan arteri pada area sekitar leher vasodilatasi dan mempermudah cairan dalam pembuluh darah dapat diikat oleh sekret atau mucus.
Hasil yang didapatkan setelah dilaksanakan implementasi keperawatan pada kedua klien adalah pada hari ketiga perawatan diruangan, dari 6 kriteria hasil didapatkan 4 kriteria yang dapat dikatakan mengalami pencapaian dan 2 kriteria dalam perbaikan. Klien 1 maupun klien 2 sama-sama telah mengalami pencapaian kriteria hasil yaitu tidak adanya keluhan sesak dengan dibuktikan frekuensi pernapasan dalam rentang normal, tidak ada otot bantu napas, tidak terjadi penurunan bunyi napas, dan sputum sudah dapat dikeluarkan pada klien 2 dan pada klien 1 sudah keluar namun di telan lagi. Pada klien 1 dan klien 2 juga terjadi perbaikan kondisi dari yang semula memiliki keluhan batuk grok-grok hingga sesak napas, dan adanya suara ronchi di beberapa lobus paru saat auskultasi, kini mengalami perbaikan kondisi meskipun masih adanya keluhan batuk, dan suara napas tambahan ronchi sudah berkurang di beberapa lobus paru.
Dari hasil tersebut, bagi peneliti selanjutnya mengenai Bronkopneumonia dengan masalah keperawatan yang sama diharapkan untuk memperpanjang waktu penelitian guna mengevaluasi kriteria hasil yang masih belum tercapai sepenuhnya atau dikatakan masih dalam perbaikan kondisi. Bagi perawat diharapkan melakukan tindakan keperawatan sesuai dengan standart asuhan keperawatan dan standart operasional sesuai kondisi pasien dengan menerapkan terapi fisioterapi dada sebagai tindakan mandiri keperawatan. Pada keluarga pasien diharapkan untuk lebih memperhatikan terkait pola pemenuhan kebutuhan nutrisi dan lainnya di rumah baik saat dalam kondisi sehat maupun saki
Going Beyond Counting First Authors in Author Co-citation Analysis

counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Variations on the Author
Full text link“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Appropriate Similarity Measures for Author Cocitation Analysis
Full text linkWe provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. Note that the objective for the hut study was to evaluate the IRS treatment applications for residual efficacy against Anopheles mosquitoes, including the local An. coluzzii mosquito population. Cows were only used to attract mosquitoes into the huts and no tests were carried out directly on the cows. The author checklist is intended for use with studies where experiments are carried out on animals, which is why we have had such difficulty in completing this for the hut study, as many of the questions do not relate to how the cows were used
