Upgrading from a Dual-chamber Pacemaker to a Cardiac Resynchronization Therapy Defibrillator in Situs Inversus Totalis with Dextrocardia Following Mitral Valve Replacement and Tricuspid Annuloplasty Using a Left-sided Approach
A 71-year-old female patient was referred to our center to upgrade a dual-chamber pacemaker to a cardiac resynchronization therapy defibrillator (CRT-D) following the detection of worsened systolic function (ejection fraction:25%-30%) via transthoracic echocardiography. The patient had situs inversus totalis with dextrocardia. She had undergone mitral valve replacement and tricuspid annuloplasty in July 2019, with a concomitant left upper pulmonary lobectomy for neoplasm, detected at cardiac tomography incidentally. In January 2020, we performed an upgrade of the preexisting device to a CRT-D system because the patient developed heart failure, reduction in systolic function, and numerous nonsustained ventricular tachycardias. The right ventricular lead that had been previously implanted was extracted. To facilitate the intervention, we decided to flip the fluoroscopic image, obtained with a right-anterior oblique view, by 180° (right-left), creating the optical impression of a levocardial position. © 2021 Innovations in Cardiac Rhythm Managemen
La sfida dell’accessibilità nelle aree interne: riflessioni a partire dalla Valle Arroscia
Nelle aree rurali e montane “interne”, la riflessione sull’accessibilità implica un cambio di prospettiva rispetto ai contesti più propriamente urbani. Migliorare l’accessibilità di questi territori significa agire su più fronti, non solo potenziando l’offerta di mobilità ma anche agendo sul capitale territoriale locale, (ri)portando servizi, conoscenza e forme di interazione. La Strategia Nazionale per le Aree Interne (SNAI) muove in questa direzione, promuovendo un approccio place-based fondato, da un lato, sulla dotazione delle precondizioni strutturali per lo sviluppo territoriale (ivi compresa l’accessibilità) e, dall’altro, sullo sviluppo locale. A partire dal caso studio della Valle Arroscia, analizzata nell’ambito del progetto di ricerca ESPON URRUC e scelta come area progetto della SNAI in Liguria, il contributo esplora i temi e le sfide peculiari per l’accessibilità nelle aree interne e riflette sulle future prospettive di sviluppo.
In “inner” rural and mountain areas, the reflection on accessibility implies a shift of perspective with respect to urban contexts. Improving accessibility in these territories means acting on several fronts, not only enhancing the transport offer but also acting on the local territorial capital, bringing (or bringing back) services, knowledge and interaction. The National Strategy for Inner Areas (SNAI) points in this direction, promoting a place-based approach grounded, on the one hand, on the provision of the “prerequisites” for territorial development (among which accessibility) and, on the other hand, on local development. Based on the case study of Valle Arroscia, that was analysed in the ESPON URRUC project and is a selected project area of SNAI, this article explores accessibility issues and challenges for inner areas, and reflects on future directions for development
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.
