Effects of eradication of Helicobacter pylori infection in patients with immune thrombocytopenic purpura: a systematic review
Whether the eradication of Helicobacter pylori infection can increase the platelet count in patients with immune thrombocytopenic purpura (ITP) is still a controversial issue. To provide evidence-based guidance, we performed a systematic review of the literature published in English, selecting articles reporting 15 or more total patients.We identified 25 studies including 1555 patients, of whom 696 were evaluable for the effects of H pylori eradication on platelet count. The weighted mean complete response (platelet count > 100 109/L) and overall response (platelet count > 30 109/L and at least doubling of the basal count) were 42.7% (95% confidence interval [CI], 31.8%-53.9%) and 50.3% (95% CI, 41.6%- 59.0%), respectively. In 222 patients with a baseline platelet count less than 30 109/L, the complete response rate was 20.1% (95% CI, 13.5%-26.7%) and the overall response rate was 35.2% (95% CI, 28.0%-42.4%). The response rate tended to be higher in countries with a high background prevalence of H pylori infection and in patients with milder degrees of thrombocytopenia. These findings suggest that the detection and eradication of H pylori infection should be considered in the work-up of patients with seemingly typical ITP. (Blood. 2009;113:1231-1240
Safety and efficacy of eltrombopag for treatment of chronic immune thrombocytopenia: results of the long-term, open-label EXTEND study.
Patients with chronic immune thrombocytopenia may have bleeding resulting from low platelet counts. Eltrombopag increases and maintains hemostatic platelet counts; however, to date, outcome has been reported only for treatment lasting≤ 6 months. This interim analysis of the ongoing open-label EXTEND (Eltrombopag eXTENded Dosing) study evaluates the safety and efficacy of eltrombopag in 299 patients treated up to 3 years. Splenectomized and nonsplenectomized patients achieved platelets≥ 50 000/μL at least once (80% and 88%, respectively). Platelets ≥ 50 000/μL and 2 × baseline were maintained for a median of 73 of 104 and 109 of 156 cumulative study weeks, respectively. Bleeding symptoms (World Health Organization Grades 1-4) decreased from 56% of patients at baseline to 20% at 2 years and 11% at 3 years. One hundred (33%) patients were receiving concomitant treatments at study entry, 69 of whom attempted to reduce them; 65% (45 of 69) had a sustained reduction or permanently stopped ≥ 1 concomitant treatment. Thirty-eight patients (13%) experienced ≥ 1 adverse events leading to study withdrawal, including patients meeting protocol-defined withdrawal criteria (11 [4%] thromboembolic events, 5 [2%] exceeding liver enzyme thresholds). No new or increased incidence of safety issues was identified. Long-term treatment with eltrombopag was generally safe, well tolerated, and effective in maintaining platelet counts in the desired range. The efficacy and safety of B-cell depletion with anti-CD20 monoclonal antibody in adults with chronic immune thrombocytopenic purpura
Because of its B-cell depleting effect, rituximab has entered clinical trials in several autoimmune conditions. This study assesses the efficacy and safety of rituximab in 57 adults with chronic immune thrombocytopenic purpura (ITP). All patients had platelet counts <30 x 10(9)/l, all had received two or more previous ITP treatments and 31 had undergone splenectomy. Patients received rituximab 375 mg/m(2) weekly for 4 weeks. Thirty-one patients (54%) responded, achieving a platelet count >50 x 10(9)/l: 18 achieved a complete response (CR: platelet count >150 x 10(9)/l) and 13 a partial response (PR: platelet count 50-150 x 10(9)/l). Twenty-nine responses occurred within 8 weeks of the first infusion. Sixteen of 18 CR patients (28% overall), including eight who had failed splenectomy, continued in CR after a median of 72.5 weeks; 15 of 16 are >1 year from the first infusion. Only two of 13 maintained a PR. Thirty-three patients experienced grade 1-2 adverse events and one a grade 3 event, but they all completed treatment. Circulating B cells fell to <0.03 x 10(9)/l. No changes in immunoglobulin levels or infectious complications were seen. In summary, rituximab was well tolerated with no immediate complications and induced a lasting, substantial response in 32% of adults with chronic ITP

We arrive to the conclusion that, as in the case of unital C⁎-algebras, U0(M)=M1−1∩U(M)={Ue⋯Ue(1):n∈N,hj∈Msa∀1≤j≤n}={u∈U(M): there exists w∈U0(M) with ‖u−w‖<2} is analytically arcwise connected. Actually, U0(M) is the smallest quadratic subset of U(M) containing the set eiM. Our second goal is to provide a complete description of the surjective isometries between the principal components of two unital JB⁎-algebras M and N. Contrary to the case of unital C⁎-algebras, we shall deduce the existence of connected components in U(M) which are not isometric as metric spaces. We shall also establish necessary and sufficient conditions to guarantee that a surjective isometry Δ:U(M)→U(N) admits an extension to a surjective linear isometry between M and N, a conclusion which is not always true. Among the consequences it is proved that M and N are Jordan ⁎-isomorphic if, and only if, their principal components are isometric as metric spaces if, and only if, there exists a surjective isometry Δ:U(M)→U(N) mapping the unit of M to an element in U0(N). Advances in ITP - therapy and quality of life - a patient survey.
Current guidelines recommend glucocorticoids and splenectomy as standard 1(st) and 2(nd) line treatments for chronic immune thrombocytopenia (ITP). We sought to find out how German ITP-patients are treated with respect to these guidelines. Members of a patient support association ≥18 years with a self-reported history of chronic ITP>12 months were surveyed with a web-based questionnaire. 122 questionnaires were evaluated. 70% of patients had chronic ITP for more than 5 years and 20% an average platelet count of ≤30·10(9)/L. 41% of the patients reported haematomas or petechiae more than once or twice and up to 12 times or more per year and 17% oropharyngeal and nasal bleeds. 11% had been admitted to hospital during the last 12 months. 88% had received or currently receive glucocorticoids, 27% were splenectomised. IVIG had been given to 55%, rituximab to 22%, anti-D to 12%, ciclosporin to 7%, while complementary and alternative medical treatments had been used by 36%. 50 women responded to questions concerning pregnancy. 14 (28%) had been advised not to become pregnant. 23 reported pregnancies and 10 (44%) required treatment for their ITP during pregnancy. Glucocorticoids are the most common therapy for chronic ITP but complementary and alternative treatments already come second and less than ⅓ of patients are splenectomised. This and the frequent use of complementary medicines suggests patients' dissatisfaction with conventional approaches. Many patients receive off-label therapies. There is a major need for adequate counselling and care for pregnant ITP-patients
Flexible time–space network formulation and hybrid metaheuristic for conflict-free and energy-efficient path planning of automated guided vehicles
Operations of Automated Guided Vehicles (AGVs) are desired to be more energy-efficient while maintaining high transport productivity, motivated by the green production requirements. This paper investigates a new energy-efficient planning problem for determining conflict-free paths of the AGVs in its transport roadmap. In this problem, the vehicle path and transport time in the roadmap are jointly optimized, based on a flexible time–space network (FTSN). We provide the mathematical problem formulation of the energy-efficient path planning problem. The resulting optimization problem is proved to be a non-convex mixed-integer nonlinear programming which is computationally intractable. We further propose a hybrid metaheuristic that integrates the genetic algorithm and estimation of the distribution algorithm to improve its computational efficiency. Numerical results show the effectiveness of the developed algorithm based on the FTSN framework, compared to the existing metaheuristics, the conventional path planning method, and a commercial solver. Decoupling of genome size and sequence divergence in a symbiotic bacterium
