    SCLERODERMA-ASSOCIATED SYNDROME: ORAL MANIFESTATIONS

    Introduction: The authors propose to evaluate the existence of a correlation between Sjogren’s syndrome and progressive systemic sclerosis, and to describe oral manifest- ations of scleroderma- associated syndrome. Material and methods: 24 patients with progressive systemic sclerosis were examined: 22 women and 2 men. All the patients examined met the diagnostic criteria for progressive systemic sclerosis. A diagnosis of Sjogren’s syndrome was made based on a positive histological examination of biopsy specimens of the accessory salivary glands of the lower lip, associated with dry keratoconjunctivitis and/or xerostomia, and the detection of serum autoantibody. Each patient underwent a careful clinical testing based on: observation of oral soft tissue alteration; evaluation of possible xerostomia and correlation with Sjogren’s syndrome; evaluation of possible microstomia; evaluation of possible Candida infections; evaluation of possible oral mucous sores in patients wearing prostheses; DMFS evaluation; evaluation of periodontal health by means of uletic bleeding index; evaluation of bone absorption state; laboratory tests. Results: The research showed that Sjogren’s syndrome was present in 20.8% of patients afflicted by progressive systemic sclerosis (SSc). The patients who suffered from SSc-associated Sjogren Syndrome had a higher rate of oral manifestations. In particular, there was a higher rate of caries and paradental disease in patients who suffered from associated syndromes compared to patients who suffered exclusively from progressive systemic sclerosis. Conclusion: The observed results confirm the existence of a correlation between the two pathologies and that scleroderma-associated syndrome is a peculiar disease that can afflict oral health. Our findings underline that oral manifestations are significantly manifest in SSc-associated Sjogren Syndrome

    Validity and reliability of measurements of the parameters of smile aesthetics: a comparison between digital and plaster models

    This study aims to evaluate the validity and reliability of the TRIOS3 Color intraoral scanner (3Shape A/S, Copenhagen, Denmark) and its associated Ortho AnalyzerTM software in measuring parameters of smile aesthetics compared with measurements on plaster casts. The study sample comprised plaster casts and digital models obtained from 30 subjects. Height (H), mesiodistal diameter (MDD) and connecting space (CS) of the anterior teeth were measured with a digital calliper on the plaster models and with the orthodontic software on the digital models. Validity was assessed using a 2-tailed paired t-test; the reliability of measurements for intra-examiner was evaluated with the Intra-class Correlation Coefficient (ICC). There were no statistically significant differences between the measurements made with the two methods. The ICC is 99% for height and MDD measurements, slightly lower in the evaluation of the CS. Linear measurements made on digital model have clinically acceptable accuracy and reliability. The TRIOS3 Color intraoral scanner represents a valuable tool for dental practice, particularly in evaluating parameters of smile aesthetics

    The Effects of Low-frequency Vibration on Aligner Treatment Duration: A Clinical Trial

    OBJECTIVES: The aim of this study was to investigate the effectiveness of an orthodontic tooth movement acceleration device (AcceleDent, OrthoAccel Technologies, Houston, Texas) when used during an aligner treatment. MATERIALS AND METHODS: Adult patients who began an aligner treatment (Lineo, Micerium Lab, Avegno, Italy) were allocated to two treatment groups. The first one (Group A), with a 7-day aligner change regimen, used the AcceleDent device for 20 min per day, whereas the second one (Group B) changed the aligners every 14 days and did not use any device. The registered outcomes were the possibility of completing the treatment, the number of aligners needed and treatment duration in the two groups. Moreover, we assessed patients’ perception of pain during the first week of treatment. RESULTS: Twenty-four patients were allocated to Group A or B depending on the acceptance of AcceleDent use. Patients which used AcceleDent (Group A) completed the treatment using each aligner for fewer days than those belonging to Group B (9.0 ± 1.0 and 15.4 ± 1.2 days, respectively) (P < 0.001). As a secondary outcome, a significant difference was found in pain perception during the first week of treatment between the two groups (P < 0.05). CONCLUSIONS: This controlled clinical trial shows that is possible to apply a 7-day change regimen together with AcceleDent use and successfully complete an aligner treatment with a significant saving of time when compared to a standard 14-days change regimen. Finally, the use of this device allowed reduction in pain perception during the orthodontic treatment

    Skull-Cervico-Mandibular Unit: Correlation between Hyoid Bone, Thyroid Gland and Posture

    Purpose: The aim of this study is to evaluate the postural control of the patient with pathology or surgery of the neck area. We have examined a group of subjects with thyroid pathology and destined to total thyroidectomy. Method: Our sample comprised 10 patients (7 female and 3 male; mean age 50,9±15,8). They have been submitted to postural examination, before and after total thyroidectomy, using the baropodometric platform, the scoliosismeter and the ocular convergence test. All the values obtained from the baropodometric tests executed before and after thyroidectomy have been compared using a statistical analysis. The T Student test and a significant level of 5% (a=0,05;p£,05) have been used. Results: It appears that total thyroidectomy in examined subjects revealed an improvement of the more representative baropodometric parameters. The 25% of the subjects that before thyroidectomy showed at the examination using the scoliosismeter a tilted body axis (to right or left), after thyroidectomy showed an alignment of the same; in the 10% of the subjects that had the tilted head on one side, this returned centrally. The 70% of the patients that before thyroid removal displayed an anterior inclination of the shoulder blade, after the operation brought back their barycentre. Moreover the subjects that before thyroidectomy displayed an insufficient ocular they improved convergence after thyroidectomy. Clinical Significance: Following the comparison and scrutiny of the above tests, there was improvement in the postural arrangement confirming the tight anatomical link between the thyroid gland and the involved structures in position maintenance

    Early diagnosis of oncologic pathologies has a crucial role to determine patient’s prognosis and therapeutic path. Nonetheless, clinical errors and omissions that can occur during diagnostic, as well as detection of preneoplastic or neoplastic condition, may result in devasting consequences both for patients in terms of health and for professionals in terms of medico-legal responsibility. This study is aimed at examining in depth, through the presentation of a specific clinical case, the medico-legal aspects inherent to the diagnosis of oral cancer, analyzing the preventive, interceptive, and diagnostic strategies, the legal implications of clinical evaluation errors and diagnostic omission, and the type of medical damage produced and professional liability. The medico-legal landscape surrounding oral squamous cell carcinoma is multifaceted and characterized by diagnostic challenges, treatment complexities, and legal considerations. Health-care providers must remain vigilant in navigating these complexities to ensure optimal patient care while mitigating legal risks. By prioritizing high-quality medical records, fostering transparent communication with patients, and implementing preventive strategies, health-care institutions can strive to minimize the incidence of litigation and uphold standards of ethical practice in oral carcinoma cases. Additionally, continued research and education in forensic and legal medicine are essential in informing evidence-based practices and promoting patient safety in this evolving field

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed

    “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
