    Oncological and Functional Outcomes of Radical Cystectomy and Orthotopic Bladder Replacement in Women. Editorial comment

    During the last decades, orthotopic bladder substitution (OBS) after radical cystectomy for bladder cancer in women has been increasingly proposed. Meanwhile, strict patient selection criteria, improved surgical techniques, long-term follow-up in large patient cohorts, and validated evaluation have defined the role of OBS in men. Several issues must be clarified in women: oncological outcome, complication rates, functional results, quality of life, and sexual function. As a matter of fact, the results of OBS in women are substantially unchanged in the last decade as stated in this paper. From the oncological standpoint, the patient outcome seems objectively satisfactory but selection criteria are different in the published series. Fortunately, the problem of urethra recurrence seems less relevant in properly selected patients. However several issues are still a matter of debate. Functional results in terms of continence and hypercontinence are still unsatisfactory because the optimal way to reconstruct the pelvic anatomy is still undefined. Early and late complication rates are poorly evaluated. Little data are also available about the quality of life and sexual function. Validated evaluation criteria in a large patient cohort are needed to objectively evaluate the results and to improve the substantially unchanged results reported in last decade.     Abstract If we think about the launch date of Mitomycin C, we could think that this supplement is very much focused on history of Medicine, and, yes, Mitomycin certainly has been a main contributor to the history of Urology. However, it is even more striking to think that even after fifty years of its use by Urologists worlwide, Mitomycin C remains an absolute "Must". The simplicity of use, the efficacy and the tolerability of Mitomycin explain its established role. In fact, "long standing" does not mean "old". The new technologies, introduced progressively in the last decades, have fully confirmed the present and future role of this drug, that is well illustrated in the articles contained in this supplement.     A growing amount of experimental evidences has progressively shown that aging and carcinogenesis share many molecular pathways. During aging, the accumulation of genetic and epigenetic changes, the diminishing of telomere length, the progressive disruption of mechanisms for DNA damage repair, glucose metabolism regulation, cell cycle control, and stem cells self-renewal lead to induction of senescence or apoptosis and loss of replicative capacity. The same cellular dysfunctional mechanisms characterizing the aging process have been found to be involved, with different level of evidence, in cancerogenesis and are reviewed herein

    Una rapida occhiata ai testi disponibili sul mercato mostra chiaramente che abbondano i testi di tecnica chirurgica urologica, mentre c’è una sostanziale carenza di testi che trattano il tema delle Complicanze nella chirurgia urologica, oggetto di questo ampio e approfondito manuale. Fino a oggi, infatti, è stata la tradizione orale a trasmettere le conoscenze e le esperienze su questo importante tema. In particolare, il decorso postoperatorio in tutta la sua complessità non trova una trattazione specifica e analitica, sebbene ogni professionista sia consapevole che qualsiasi intervento chirurgico deve tenere in considerazione le possibili complicanze in modo da attuare tutte le strategie necessarie a tutelare il paziente. Today, many surgical disciplines make use of the DaVinci robotic system in performing surgery, even complex ones.
