COVID-19 revisiting inflammatory pathways of arthritis
No full textCoronavirus disease 2019 (COVID-19) is an infectious disease, caused by severe acute respiratory syndrome coronavirus 2, which predominantly affects the lungs and, under certain circumstances, leads to an excessive or uncontrolled immune activation and cytokine response in alveolar structures. The pattern of pro-inflammatory cytokines induced in COVID-19 has similarities to those targeted in the treatment of rheumatoid arthritis. Several clinical studies are underway that test the effects of inhibiting IL-6, IL-1β or TNF or targeting cytokine signalling via Janus kinase inhibition in the treatment of COVID-19. Despite these similarities, COVID-19 and other zoonotic coronavirus-mediated diseases do not induce clinical arthritis, suggesting that a local inflammatory niche develops in alveolar structures and drives the disease process. COVID-19 constitutes a challenge for patients with inflammatory arthritis for several reasons, in particular, the safety of immune interventions during the pandemic. Preliminary data, however, do not suggest that patients with inflammatory arthritis are at increased risk of COVID-19

Gal-Ed diskutiert Mangers Leben und Schaffen in weitem Zusammenhang des Schicksals der Jiddischen Sprache und Kultur im 20. Jahrhundert. Das Buch verweist auf die interessante Spannung zwischen der Herrenlosigkeit und gleichzeitigen europäischen Zugehörigkeit von Jiddisch als eine Minderheitensprache. In dieser Hinsicht stellt die Biographie einen wichtigen Beitrag sowohl zu den Jiddishstudien, als auch zur Literaturwissenschaft allgemein dar.Efrat Gal-Ed´s book is an extensive biographical work on the Yiddish poet Itzik Manger. Gal-Ed discusses his work and life, situating him in the broader context of linguistic and ethnic developments of the 20th century. The author presents Manger as a European poet, engaged in the European cultural developments, who simultaneously was a poet of unrecognized minority language. This biography of Manger contributes both to literary and Jewish studies

Extraartikuläre weichteilrheumatische Erkrankungen (Weichteilrheumatismus) und Rückenschmerzen
No full textExtraartikuläre oder weichteilrheumatische Erkrankungen umfassen ca. ein Viertel aller Erkrankungen des rheumatischen Formenkreises. Dazu gehören generalisierte Phänomene (wie z.B. Fibromyalgie) sowie lokalisierte Schmerzmechanismen im Bereich der Wirbelsäule und der Gelenke. Erkrankungen des subcutanen Binde- und Fettgewebes gehören genauso zu dieser Kategorie wie Myopathien und deren Differentialdiagnosen. Die entzündlichen Myopathien werden im Kapitel O 27 abgehandelt.
Aufgrund der Häufigkeit dieser Erkrankungen sind sie nicht nur Alltag in einer rheumatologischen Sprechstunde sondern auch in einer hausärztlichen Praxis. Bei den nicht-entzündlichen Formen fehlen häufig spezifische Bluttests. Durch die Verbesserung der bildgebenden Verfahren (z.B. Ultraschall, Magnetresonanz-Tomographie) ist es in den letzten Jahren jedoch möglich geworden, auch lokale „Reizungen“ für den Untersucher sichtbar zu machen. In der Mehrzahl der Fälle wird die Diagnose jedoch aufgrund der Anamnese und der klinischen Untersuchungen gestellt. Häufig finden sich auslösende Faktoren wie mechanische Überbelastungen oder ungewohnte Belastungen sowie repetitive Tätigkeiten, die sich ergonomisch ungünstig auf die jeweiligen Strukturen auswirken. Psychische Faktoren lassen die Schmerzintensität als auch die Ausdehnung der Symptomatik häufig exazerbieren und erschweren die Differentialdiagnose. Die folgenden Kapitel beschreiben die wichtigsten weichteilrheumatischen Erkrankungen und geben diagnostische sowie therapeutische Hinweise

Diasporas within and without Africa : dynamism, heterogeneity, variation
No full textThe book deals with two types of “African diasporas”, the first of which originated in the migration histories of the Indian Ocean and brought new groups into Africa. This is illustrated by case studies of Hadrami communities in Sudan and Zanzibar, and the Malay community in Cape Town, that produced trade links as well as processes of Islamization. The second type originated with the failing African states and cases discussed are an Eritrean diaspora in Germany, alongside Sudanese diasporas in Norway and the USA, and a Somali diaspora in Norway. The papers deal with processes of homemaking, political mobilization in the diaspora through local organisations, religious networks and cyberspace nationalism. The central conceptual argument is that “diaspora” is not only a post-modern reaction to the xenophobia of Western nation states but must be seen as part of a broader history of human migration and intercultural experience.
