    Acute Myocardial Infarction is Reflected in Salivary Matrix Metalloproteinase-8 Activation Level

    Background: The aim of this study is to compare salivary and serum biomarker levels and degrees of matrix metalloproteinase (MMP) activation between patients with acute myocardial infarction (AMI) and systemically healthy patients (non-AMI) with similar periodontal conditions. Methods: A total of 92 patients (47 AMI and 28 non-AMI patients with gingivitis or periodontitis; and 17 systemically and periodontally healthy patients as a control group) were recruited. Clinical periodontal measurements were recorded; stimulated whole saliva and serum samples were collected. AMI patients were clinically examined within 3 to 4 days after admission to the coronary care unit. Saliva samples were analyzed for levels of MMP-8, MMP-7, and tissue inhibitor of matrix metalloproteinase (TIMP)-1. Serums were tested for MMP-8, MMP-9, TIMP-1, and TIMP-2 levels by immunofluorometric assay and enzyme-linked immunosorbent assay. Molecular forms and degree of activation of salivary MMP-8, MMP-9, and MMP-13 were analyzed by computer-scanned immunoblots. Results: Total salivary MMP-8 assessed by immunofluorometric assay method and immunoblot densitometric units was higher in non-AMI than in AMI patients' saliva but a significantly higher percentage of AMI patients was activated polymorphonuclear leukocyte type (PMN) MMP-8 (P < 0.001) regardless of periodontal diagnosis. Serum MMP-8, MMP-9, and TIMP-1 levels were significantly higher in AMI (for all markers and all comparisons, P < 0.05). Characteristic for AMI was dominance of active PMN MMP-8 in saliva. Conclusions: Enhanced MMP-8 activation in the saliva of AMI patients is evidently, in part, of systemic origin. Consequently, AMI is reflected in serum but also in saliva.     "Henry Rider Haggard’s Posthumanist Eco-consciousness"

    In this essay I argue that the late Victorian and early Edwardian novelist Henry Rider Haggard had a distinct eco-consciousness that was reminiscent of twenty-first century posthumanist philosophies as early as the 1880’s. Writing his “imperial romances” in the wake of the beginnings of what is now called the Anthropocene age, Haggard observed and understood the changing relationships between the human society and the more-than-human world in this historical period. In a manner that would be unexpected of an author widely labeled as ‘a man of his times’—and therefore a pro-imperial propagandist—in postcolonial literary criticism, he was critical of the anthropocentric ways of western civilization. Ranging from a Darwinian notion of the oneness of all living animals to a highly skeptical attitude toward the reckless exploitation of the resources of colonial lands and peoples, Haggard’s Weltanschauung included notions that were not only antithetical to imperialist discourses, but also ‘ahead of [their] times’ by more than a century. In this essay I elaborate on Haggard’s posthumanist eco-consciousness with reference to his fiction and conclude that a renewed understanding of Haggard’s literary and intellectual heritage would show how his works are still very much relevant and valuable in our time

    Decreased soluble cell adhesion molecules after tirofiban infusion in patients with unstable angina pectoris

    Abstract Aim The inflammatory response, initiated by neutrophil and monocyte adhesion to endothelial cells, is important in the pathogenesis of acute coronary syndromes. Platelets play an important role in inflammatory process by interacting with monocytes and neutrophils. In this study, we investigated the effect of tirofiban on the levels of cell adhesion molecules (soluble intercellular adhesion molecule-1, sICAM-1, and vascular cell adhesion molecule-1, sVCAM-1) in patients with unstable angina pectoris (AP). Methods Thirty-five patients with unstable AP (Group I), ten patients with stable AP (Group II) and ten subjects who had angiographycally normal coronary arteries (Group III) were included the study. Group I was divided into two subgroups for the specific treatment regimens: Group IA (n = 15) received tirofiban and Group IB (n = 20) did not. Blood samples for investigating the cell adhesion molecules were drawn at zero time (baseline; 0 h) in all patients and at 72 h in Group I. Results The baseline levels of sICAM-1 and sVCAM-1 were higher in Group I than in Groups II and III. They were higher in Group IA than in Group IB. However, the sICAM-1 and sVCAM-1 levels decreased significantly in Group IA after tirofiban infusion. In contrast, these levels remained unchanged or were increased above the baseline value in Group IB at 72 h. Conclusion The levels of cell adhesion molecules in patients with unstable AP decreased significantly after tirofiban infusion. Inhibition of platelet function by specific glycoprotein IIb/IIIa antagonists may decrease platelet-mediated inflammation and the ischemic end-point.</p

    Association between periodontal disease and acute myocardial infarction

    Background: Coronary heart disease is the leading cause of morbidity and mortality throughout the world. Well-known risk factors independently or combined participate in both myocardial infarction and atherosclerosis. Recent data have shown that viral and bacterial infections may also contribute to the acute thromboembolic events. The aim of the present study was to investigate the possible association between periodontal health and coronary heart disease in patients with acute myocardial infarction and chronic coronary heart disease. Methods: A total of 120 patients, 60 with acute myocardial infarction (AMI) and 60 with chronic coronary heart disease (CCHD) were included in this study. The patients in the AMI group (50 men and 10 women; mean age 53.8 +/- 9.5 years) were admitted to the Department of Cardiology, University Hospital of Ege because of AMI. The CCHD patients group (42 men and 18 women; mean age 58.5 +/- 11.6 years) had no documented history of recent acute coronary events. All patients were clinically examined and completed a medical questionnaire. Missing teeth, restorations, probing depth (PD) and bleeding on probing (BOP) were recorded. Blood samples were taken on admission for measurements of serum total cholesterol, triglycerides, high density lipoprotein cholesterol (HDL-cholesterol), low density lipoprotein cholesterol (LDL-cholesterol), and fasting blood glucose level. Sample proportions were compared by chi square test, quantitative variables with Student t test. The relation of clinical parameters and conventional risk factors to AMI was assessed with logistic regression analysis. Results: The number of sites with PD greater than or equal to4 mm, the percentage of sites exhibiting BOP, smoking status, total cholesterol, LDL-cholesterol, and triglycerides were statistically different between AMI and CCHD groups (P 4, the number of restorations, smoking status, and triglycerides levels were significantly associated with AMI (P < 0.05). Conclusions: The results of this study indicate that periodontal disease may be associated with acute myocardial infarction. To our knowledge, this is the first study that reports the importance of periodontal health in the occurrence of acute myocardial infarction in a Turkish population. We propose that prospective randomized studies are needed to determine whether periodontal disease is a risk factor in the occurrence of acute myocardial infarction

    Opinnäytetyössä luotiin mielikuvia aiheesta ”itä ja länsi kohtaavat” ja suunniteltiin yritys Globe Hopelle printti/printtejä. Haluttiin ymmärtää entistä paremmin turkkilaisten ja suomalaisten kulttuurisia erilaisuuksia ja yhtäläisyyksiä. Mitä tapahtuu kun ”itä ja länsi kohtaavat”? Jotta saataisiin vastaus tähän kysymykseen, haettiin tietoa turkkilaisten ja suomalaisten juurista. Suunnittelutehtävää varten esiteltiin myös yksityiskohtaisemmin joutsenta suomalaisuuden symbolina ja sutta turkkilaisuuden symbolina. Suunnittelussa haluttiin ottaa huomioon tietoa menneisyydestä, mutta myös käsitys nykypäivän sekä suomalaisuudesta että turkkilaisuudesta koettiin tärkeänä. Tietoa turkkilaisten käsityksistä omasta kansallisesta identiteetistä nykypäivänä saatiin keskusteluin turkkilaisten kanssa. Suomalaisuutta työssä edusti tekijä itse. Tämä kaikki tieto muodosti tekijän kokemuksen molemmista kulttuureista. Suunnittelussa inspiraation lähteenä toimivat itse tehdyt maalaukset ja omat kokemukset suomalaisuudesta ja turkkilaisuudesta. Harjoittelu Globe Hopella tuotti jo paljon ideoita printtien suhteen ja heti harjoittelun jälkeen printtien suunnittelu jatkui. Työn tuloksena parani ymmärrys idän ja lännen eroavaisuuksista ja yhtäläisyyksistä, syntyi omia mielikuvia ja mielikuvista printtejä Globe Hopelle.In the thesis some images of the topic "East meets west" were created, and some prints were planned for the company Globe Hope. The aim was to gain a better understanding of Turkish and Finnish cultural differences and similarities. What happens when the "East meets West"? In order to answer this question, information on the Turkish and Finnish roots were sought. For the planning process the swan was also presented in more detail as a symbol of the Finnish culture and the wolf as a symbol of the Turkish culture. In the design process information from the past had to be taken into consideration, but also the perception of the present-day Turkish and Finnish cultures was seen as important. Information about Turkish perceptions of their own national identity today were gained by discussions with the Turks. The Finnish culture was represented by the author herself. All this information created the author's experience in both cultures. In the design process, the self-made paintings and own experience of Finnish and Turkish cultures were the source of inspiration. Work practice at Globe Hope already produced a lot of ideas in terms of prints, and print designing continued immediately after the work practice. The result of the work was the improved understanding of the East-West differences and similarities. Some own mental images were born and from those images some prints for Globe Hope were created

    Circadian and Infradian Rhythms of Vasovagal Syncope in Young and Middle-Aged Subjects

    Background: The most cardiovascular physiological and pathophysiological events show a circadian rhythm. It is thought that the autonomic nervous system and biologic factors play a key role in the pathogenesis of vasovagal syncope (VVS). In this study, we investigated the circadian and infradian variation of VVS. Methods: A prospective consecutive series of 246 patients (142 women; mean age 36 +/- 7 years) with recurrent syncope and with a positive head-up tilt testing (HUT) were included in this study. The daily and weekly distributions of the syncopal episodes were investigated. The assessments of episodes were done according to the days of the week and three time periods of the day (6 a.m.-12 noon, 12 noon-6 p.m., 6 pm-12 midnight). Results: According to the results of HUT, 76 patients (31%) had cardioinhibitory, 62 patients (25%) had vasodepressor, and 108 patients (44%) had mixed type of VVS. During the initial passive phase, 40% of patients (99/246) showed positive response. Time to syncope was 20 +/- 2 minutes during HUT. The mean number of syncopal episodes was 4 +/- 2/years. In all, 1,070 episodes were evaluated. The distribution of the episodes in 6-hour intervals was significantly different from uniform occurrence (38%, 33%, and 29%, respectively) (P = 0.02). The frequency of episodes was higher in the morning (P = 0.045) and in the middle of the week (P = 0.046). A significant difference was found between week and weekend days in terms of the frequency of episodes (75.5% vs 24.5%, P = 0.01). Conclusion: VVS may show a circadian and infradian rhythm.
