    Caring for a person with dementia: The results of a participatory research study

    Dementia is a widespread neurodegenerative disease in the Western context, particularly in Italy. It produces significant consequences of various kinds, both for the person who is experiencing the problem and for their caregiver. As the literature shows, the early stages of the disease represent an extremely delicate moment for families. This paper presents a study that investigated the life experiences of people with dementia and their caregivers in the early stages of the disease. Considering its field of investigation and its objective, this study was suitable for the application of participatory research methodology, by directly involving experts by experience. This article discusses the results of the qualitative research carried out by interviewing caregivers of people with dementia/cognitive deterioration. The study results enable us to understand and explore various aspects of this topic: the meaning of caring for a person with dementia; motivations of caregivers; attitudes and emotions concerning the disease; positive aspects related to caregiving; relationships between caregivers and patients, and, in general, how families deal with this situation. The results presented in this paper provide useful considerations and various suggestions for social workers and others involved in the caregiving process

    Ispirandosi a un contributo di David Wilkins, assistente sociale e padre di due figli con fibrosi cistica, l'articolo si propone di porre l'attenzione sulla disabilità vissuta dai familiari e, nello specifico, sull'impatto che ha per dei genitori l'assessment degli operatori. Infatti, nel Social Work è rilevante il ruolo svolto dalle percezioni dei genitori, in particolare nel lavoro con i minori e le famiglie. L'articolo, proponendo la storia del caregiving di un padre che riesce a coniugare le conoscenze della sua professione e quelle esperienziali di genitore-caregiver, permette di riflettere sulle dinamiche e sulle modalità di relazione che si sviluppano durante un assessment, ponendo in luce differenti aspetti significativi, primo fra tutti l'importanza del confronto e il valore della reciprocità

    La ricerca presentata in questo volume ha esplorato, usando un approccio partecipativo, i vissuti dei caregiver e dei soggetti che in prima persona affrontano una demenza all’esordio o le prime fasi del deterioramento cognitivo, con lo scopo di comprendere come potrebbero essere forniti aiuti specifici. La ricerca, infatti, ha visto coinvolti attivamente in tutto il processo di indagine soggetti con un’esperienza di vita diretta, personale o legata al loro lavoro quotidiano, del fenomeno da studiare, i quali sono stati considerati a tutti gli effetti co-ricercatori.     The health emergency has had a significant impact on residential care homes. Attention was focused on the high number of infections and deaths, while less attention was paid to the pandemic’s consequences on the work of the professionals and their experiences. The study involved seven Residential care homes in the North of Italy. The research’s general purpose was to understand the consequences of the pandemic on elderly people in care homes, their families, and professionals. Within this broader purpose, it was possible to ascertain the experiences of care workers during the first wave of the pandemic and which elements supported them in that period. The research followed a qualitative approach. Forty-four semi-structured interviews with managers, staff, residents and family members were carried out. The research findings could help social workers and residential care home managers identify operational paths aimed at promoting professionals well-being and implementing preventive measures

    L’obiettivo della ricerca è stato quello di indagare le esperienze delle persone e delle famiglie che stanno vivendo le prime fasi della demenza all’esordio, con lo scopo di comprendere come potrebbero essere forniti aiuti specifici per sostenere le persone, con i loro particolari bisogni, proprio in questa prima e delicata fase della malattia. Considerato il suo campo di indagine e il suo oggetto, tale ricerca si è prestata particolarmente all’applicazione di una metodologia di ricerca di tipo partecipativo. La ricerca infatti ha visto coinvolti attivamente in tutto il processo di ricerca i soggetti che hanno un’esperienza di vita diretta, personale o legata al loro lavoro quotidiano, del fenomeno da studiare. Sono stati utilizzati lo strumento dell’intervista semi-strutturata e del diario per indagare il punto di vista dei caregiver familiari e degli anziani affetti da deterioramento cognitivo e/o demenza in fase iniziale. I risultati ottenuti, molteplici ed eterogenei, hanno permesso di comprendere sia i vissuti dei caregiver familiari sia dei soggetti che in prima persona affrontano la malattia. Tali risultati sono stati divisi per aree tematiche, proponendo delle mappe concettuali: sei che si focalizzano sul punto di vista dei caregiver, quattro che focalizzano l’attenzione sul vissuto degli anziani. I risultati permettono di comprendere il significato che ha il prendersi cura per il caregiver e il ricevere assistenza per l’anziano; gli atteggiamenti e le emozioni di entrambi riguardo la malattia e gli aspetti legati ad essa; il ruolo della famiglia e di altre figure significative in questa situazione; il rapporto con i servizi e le modalità con cui le famiglie affrontano questo problema.The research aim was to investigate the experiences of people and families who are experiencing the early stages of dementia, in order to understand which specific helps could be provided to support people, with their particular needs, in this first and delicate phase of the disease. Considering its field of investigation and its object, this research has been particularly suited to the application of the participatory research methodology. The research in the entire research process has actively involved people who have a direct, personal or job-connected experience of the disease. Tools as a semi-structured interview and a diary has been used to investigate the point of view of family caregivers and their elderly relatives with cognitive impairment. The results obtained have been multiple, heterogeneous and allowed us to understand the experiences of the family caregivers and of the subjects who face the disease directly. These results have been divided by different areas and have been organized using concept maps. Six maps have been focusing on the point of view of the caregivers and four maps focusing on the experience of the elderly. The results allowed the researcher to understand different elements: what caring means for the caregivers and what to be assisted means for the elderly with cognitive impairment; the attitudes and emotions of both players about the disease; the role of the family and other significant figures in this situation; the relationship with the services and how families face this problem

    Gerontological social work is a working area that engages many social workers, however there are few studies that highlight its characteristics. Often it is represented by students and professionals as not very interesting and connected exclusively to the provision of standard services. This paper presents the results of a research carried out in a District of Lombardy, aimed at describing the work of social workers who deal with dependent people and highlighting their strengths in dealing with the situations of people in difficulty. Semi-structured interviews were conducted with the 5 social workers working in the non-self-sufficiency area in the 4 District Municipalities. Social workers were asked to describe some situations that had a positive outcome. 32 situations were described and meaningful perspectives were pointed out, highlighting the complexity of Gerontological social work and the importance of an approach that focuses on care and relationships

    Per effetto delle dinamiche demografiche e della crisi delle risorse fiscali disponibili per i sistemi di welfare, accade che s’imponga sempre più alla nostra attenzione la condizione di non autosufficienza, sia quella che riguarda persone adulte con disabilità sia, soprattutto, quella legata all’età anziana. Il volume intende indagare, attraverso un metodo di ricerca originale, il tema del fronteggiamento sociale dei problemi di vita e della maniera in cui esso può essere facilitato dai professionisti dell’aiuto. L’indagine, di sicuro interesse per professionisti del settore e studiosi, mostra il lavoro effettivamente svolto dagli assistenti sociali nelle situazioni di non autosufficienza, con l’obiettivo di mettere in luce le caratteristiche distintive del loro apporto e di identificare le pratiche professionali che possono risultare maggiormente efficaci nell’innestare e sostenere i processi di assistenza

    Participatory research and relational social work: an intersection of knowledge

    The model of Participatory Research (PR) has been used for a long time in the field of social research, particularly in social work research (Fleming et al., 2014). In this context, its use is growing, and is suitable for investigating conditions such as vulnerability and social fragility (Aldridge, 2015). Within the Research Centre on Relational Social Work of the Milan Catholic University, researchers are experimenting with participatory research methods in different fields (poverty and social exclusion, elderly people suffering from dementia, child protection and foster care). In this research the participatory methodology was used in investigating a particular methodological paradigm known as Relational Social Work (RSW) (Folgheraiter, 1998; 2003; 2007) and connections between the two areas were observed and studied. More specifically, it was possible to observe that the fundamental principles inspiring the RSW model are able to provide PR with a clear theoretical framework and that the operational RSW methodology can offer useful guidelines applicable in the context of PR. In the oral presentation, we will highlight the contribution that Relational social work can offer to participatory research in order to better explain the basic steps of the research, the role of the participants and how we can address some critical issues. The guiding value principles of RSW are the same that can be found in PR: promotion of empowerment, enhancement of experiential skills of citizens and service users, reciprocity in the process construction and recognition of equality between practitioners and service-users, and promotion of an anti-oppressive work perspective. Starting from these inspiring principles, the methodology developed in RSW can contribute significantly to defining the role of researchers in PR, intended as a facilitator of joint work and relationships within the group, and that of co-researchers, whom can be seen as a coping network moving collaboratively towards shared purposes. Even during the various stages of a research, it is possible to derive meaningful insights from RSW. In particular, the initial phase is crucial, since in it all participants are required to jointly define the purpose and recognise themselves as members of a group (or network) in which each member provides a unique and priceless contribution, regardless of the professional role. Starting from this setting it is possible to promote real, non-rhetorical participation across all research phases and the helping process. From RSW it is also possible to obtain elements to constructively address some critical issues and ethical questions that open within PR. Firstly, we can reflect about the gap from the freedom of expression of co-researchers and the need to ensure the scientific value of research. Secondly, RSW can help to go beyond the imbalance of power between professional researchers and “experts by experience”

    Un questa fase della ricerca ci si è proposti di procedere a una ricostruzione descrittiva dell’esperienza di alcune persone non-autosufficienti e dei loro caregivers, utenti dei servizi socio-assistenziali. Tali esperienze sono state analizzate al fine di identificare i “punti di svolta” e gli elementi più significativi in merito alla qualità della vita, in particolare per quanto attiene al rapporto con i Servizi e gli operatori. Da tale analisi, integrata con quanto emerso nella prima parte dello studio, sono state focalizzate alcune indicazioni prospettiche da tenere in considerazione per un eventuale sviluppo/rafforzamento del servizio sociale
