    Microbial Association of Gastroduodenal Diseases with Special Reference to Epstein–Barr Virus and Helicobacter pylori and their Effect on Expression of miRNAs: A Review

    Gastroduodenal diseases are very commonly encountered among patients who attend hospital with the complaints of dyspepsia. These diseases range from inflammatory ones like gastritis and peptic ulcer disease to neoplastic ones like gastric carcinoma and lymphoma. Gastroduodenal diseases may be caused by various factors of which microbial association with a bacterium called Helicobacter pylori and a virus called Epstein–Barr virus (EBV) is noteworthy. It has also been noted in various studies that the coinfection by these two organisms may also play a significant role in an exaggerated inflammatory response in cases of gastritis which may ultimately lead to carcinoma (Correa’s cascade of carcinogenesis). As a result, it appears critical to identify both H. pylori and EBV in samples taken for biopsy in various gastroduodenal illnesses. Recent research has also suggested that these two separate etiologies may work together to cause gastric cancer, with miRNAs playing a key part in this process. This review presents the status of the current research on the association of gastroduodenal diseases with EBV and H. pylori along with the emerging context of the connection with miRNA expression. This will help to understand these complex etiologies having significant bearing on human health as well as highlight the need for intensive research in the subject

    Kimura's disease: A diagnostic challenge experienced with cytology of postauricular swelling with histopathological relevance

    Kimura's disease is a rare, chronic inflammatory disorder of unknown cause. It is endemic in Asia, affecting more number of males than females, with a ratio of 3:1. The typical clinical manifestations include a triad of painless unilateral cervical lymphadenopathy or subcutaneous masses predominantly in the head and neck region, blood and tissue eosinophilia, and an elevated serum immunoglobulin E (IgE) levels. Variable conditions both benign and malignant may mimic Kimura's disease both clinically and on fine needle aspirates. The confirmatory diagnosis is established only by histopathological examination. We report a case of Kimura's disease in a patient who underwent multiple investigations in view of her past history and family history of pulmonary Koch's. Fine needle aspiration cytology (FNAC) was performed thrice with consistently similar result of reactive lymphadenitis with numerous histiocytes and eosinophilia. The final diagnosis of Kimura's disease could finally be established only on histopathological examination

    Efficacy of ChatGPT in solving attitude, ethics, and communication case scenario used for competency-based medical education in India: A case study

    BACKGROUND: Competency-based medical education (CBME) is a method of medical training that focuses on developing learners’ competencies rather than simply assessing their knowledge and skills. Attitude, ethics, and communication (AETCOM) are important components of CBME, and the use of artificial intelligence (AI) tools such as ChatGPT for CBME has not been studied. Hence, we aimed to assess the capability of ChatGPT in solving AETCOM case scenarios used for CBME in India. MATERIALS AND METHODS: A total of 11 case scenarios were developed based on the AETCOM competencies. The scenarios were presented to ChatGPT, and the responses generated by ChatGPT were evaluated by three independent experts by awarding score ranging from 0 to 5. The scores were compared with a predefined score of 2.5 (50% accuracy) and 4 (80% accuracy) of a one-sample median test. Scores among the three raters were compared by the Kruskal–Wallis H test. The inter-rater reliability of the evaluations was assessed using the intraclass correlation coefficient (ICC). RESULTS: The mean score of solution provided by ChatGPT was 3.88 ± 0.47 (out of 5), indicating an accuracy of approximately 78%. The responses evaluated by three raters were similar (Kruskal–Wallis H P value 0.51), and the ICC value was 0.796, which indicates a relatively high level of agreement among the raters. CONCLUSION: ChatGPT shows moderate capability in solving AETCOM case scenarios used for CBME in India. The inter-rater reliability of the evaluations suggests that ChatGPT’s responses were consistent and reliable. Further studies are needed to explore the potential of ChatGPT and other AI tools in CBME and to determine the optimal use of these tools in medical education

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed.     “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

    Appropriate Similarity Measures for Author Cocitation Analysis

    We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.     Dispelling the Myths Behind First-author Citation Counts

    We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more sophisticated methods
