    The amputation and reconstructive procedure of the management of open tibial fracture type III is still of interest and controversy because the amputation effort is considered as a punishment of primary treatment but a reconstructive method in massive lower extremity trauma is difficult. The MESS is a scoring system that can be applied to mangled extremity and help one determines which mangled limb will eventually come to amputation. The record of 28 open tibial fracture type III patients had been reviewed at the SARDJITO hospital 1990-1995. All seven open tibial fracture type Ill C patients with MESS of greater than seven were amputated. According to four tables of blind comparison analyzing MESS value those patients of more than seven was found to have a positive predictive of 100 % that may be best treated by primary amputation. In conclusion the MESS is an early and accurate predictor for identifying the extremity that may be best treated by amputation or reconstruction.Key words : open fracture - MESS - tibial fracture - amputation - mangled extremit

    All open fractures especially open fracture of the lower leg grade III A and III B, should be assumed to be contaminated, therefore they tend to be infected. The time of intervention is one of the risk factors of infection complication.A descriptive analysis has been conducted aiming at evaluate the occurrence of infection and the time of treatment on patients with open fracture of the lower leg grade III A and III B. In this study 102 cases (80 males and 22 females) who had open fracture of the lower leg grade III A and III B were admitted to Dr. Sardjito General hospital, Yogyakarta. Patients were evaluated according to the inclusion and exclusion criteria. Out of fifty-four cases underwent intervention before 8 hours after injury (within golden period) 25 cases (46,29%) had been infected. The rest (48 cases) who had treatment after 8 hours since trauma, 83,33% (40 cases) were infected.A statistical analysis (X2 = 13,520, df=1 and p < 0,001) showed that there was significant difference of infection complication of open fracture of the lower leg grade III A and III B and the intervention between within and beyond golden period.Key words: open fracture - tibia fracture - infection - golden period - debridemen

    Seven patients of open tibial fractures type III C were treated in the Emergency Unit, Sardjito hospital from 1990 to 1995. Three patients underwent early amputation and four patients ultimately required secondary amputation from three to twelve days after vascular repair intervention.Open tibial fractures associated with vascular trauma need an immediate decision between early amputation and limb salvage.     Hoffa Fracture is a fracture at femoral condyle caused by direct injury of the knee at flexion position; there fore the articular surface is broken. Distal fragment of fracture can be displaced or undisplaced. The Hoffa fracture is divided into three types according to prognostic implication. Type I, the intra articular fracture line reaches the supracondyle of the femur and some of the soft tissues still attached to the distal fragment. Type II, intra articular fracture without any attachment of the soft tissue to the distal fragment. and type III where the fracture line slight anteriorly and proximally to the condyle of the femur with some attachment of the soft tissues and ligaments to the distal fragment. A case of young motor cyclist who involved in collision against a bridge is reviewed. He has type I Hoffa fracture Which is equal to grade III B Gustilo classification system. The conservative procedure of open wound management and skeletal traction was adequate. The problem was that, the radiographic interpretation was difficult if the anterior part of the femoral condyle was intact, undisplaced or slightly displaced. Therefore CT-Scan and tomography were indicated. In type I and type III Hoffa fracture, some soft tissue structures remain attached to the distal condyle fragment and that there will be some blood supply to this fragment. However, in type II fracture the soft tissue may completely be unattached especially type II C and it could predispose to avascular necrosis or nonunion.Key Words : Fracture - intraarticular - femoral condyle - avascular necrosis - non-unio

    This study was aimed at describing the management of reformatory of At-Taqwa Muhammadiyah on building the studentsÔÇÖ personality MTs Muhammadiyah Padang Luar. The study was qualitative case study by applying interview, documentation and observation as instruments. The data were analyzed by using triangulation method. The results revealed that the buliding of studentsÔÇÖ personality was done by internalizing the moral value both godÔÇÖs interaction and humanÔÇÖs interaction such praying; memorizing quran, and social intercourse.     This study aims to develop the learning of mathematics 1st junior class VII, on Numbers and Set. The learning tools developed consists of a syllabus, Lesson Plan (RPP), Sheet Teaching Materials (LMA), Student Activity Sheet (LAS), instructional media, and Authentic Assessment Sheet (LPA). This study is the research, development model 4-D which consists of four stages: Define, Design, Develop, and Disseminate. In this study the development process undertaken only to phase 3. The learning tools that have been developed, validated by two-person of experts and two practitioners then conducted a limited trial to the junior high school students in coastal areas. Data validation and students' responses collected through questionnaires which are then revised on the advice. From the results of the validation data showed the average score was 3.93 for syllabus (very valid), RPP was 3.44 (very valid), LAS was 3,67 (very valid), LMA was 3.83 (very valid), LMP was 3.92 (very valid),  and the average score of the students' response was 3.76 (very practical). From the development of learning tools that have been carried out it is concluded that a learning tool developed very valid and ready to be tested on students of SMP / MTs by a larger amount.</jats:p

    Abstrak Kemampuan Pemecahan Masalah Matematis (KPMM) sangat penting dalam pembelajaran matematika. Namun berdasarkan PISA 2018, penelitian terdahulu serta hasil kuesioner menunjukkan bahwa siswa masih memiliki KPMM yang rendah. Rendahnya KPMM siswa dikarenakan matematika yang bersifat abstrak. Media pembelajaran dapat membuat sesuatu yang abstrak menjadi lebih konkrit serta penggunaan pendekatan scientific dapat memfasilitasi KPMM siswa. Penelitian ini bertujuan menghasilkan media pembelajaran matematika berbasis pendekatan scientific berbantu Articulate Storyline 3 materi garis dan sudut untuk memfasilitasi KPMM siswa SMP/MTs yang valid dan praktis. Metode penelitian ini adalah Research and Development (R&D) dengan Model Pengembangan yaitu Model 4D. Penelitian ini melibatkan 21 siswa kelas VIII SMP di Kota Duri. Data yang dikumpulkan adalah data kevalidan dan data kepraktisan. Data kevalidan diperoleh dari angket validasi yang diserahkan kepada para ahli atau validator. Data kepraktisan diperoleh dari angket respon siswa yang disebar kepada responden. Berdasarkan analisis data diperoleh persentase rata-rata validasi aspek media bernilai 97,21 (sangat valid), aspek materi 97,07 (sangat valid), aspek pendekatan scientific 92,08 (sangat valid) dan aspek KPMM 95,22 (sangat valid). Hasil kepraktisan pada small group test bernilai 90,43 (sangat praktis) dan pada field test sebesar 83,96 (praktis). Dari hasil penilaian ahli, small group test dan field test disimpulkan media pembelajaran matematika yang dihasilkan valid dan praktis. Abstract Mathematical Problem Solving Ability (KPMM) is very important in learning mathematics. However, based on PISA 2018, previous research and the results of the questionnaire showed that students still had low CAR. The low KPMM of students is caused by abstract mathematics. Learning media can make something abstract more concrete and the use of a scientific approach can facilitate students' KPMM. This study aims to produce mathematics learning media based on a scientific approach assisted by Articulate Storyline 3 material on lines and angles to facilitate valid and practical KPMM for SMP/MTs students. This research method is Research and Development (R&D) with the Development Model, namely the 4D Model. This research involved 21 students of grade 8 SMPS in Duri City. The data collected in this study are validity data and practicality data. Validity data was obtained from validation questionnaires submitted to experts or validators. Practicality data was obtained from student response questionnaires which were distributed to respondents. Based on data analysis, it was obtained that the average percentage of the media aspect validation was 97.21 (very valid), the material aspect was 97.07 (very valid), the scientific approach aspect was 92.08 (very valid) and the KPMM aspect was 95.22 (very valid). . The practicality result on the small group test was 90.43 (very practical) and on the field test it was 83.96 (practical). From the results of expert assessments, small group tests and field tests, it was concluded that the mathematics learning media produced was valid and practical
