    Midline facial granuloma: distruzione totale del terzo medio

    Il Granuloma Maligno centrofacciale, definito anche “midline facial granuloma” è ancora del tutto sprovvisto di qualsiasi possibilità di successo terapeutico, sia medico che chirurgico. Il caso che presentiamo è di per sé del tutto eccezionale. E’ venuto alla nostra osservazione in una fase talmente avanzata per la totale demolizione della regione centrofacciale e con parametri ematologici tali da rendere incomprensibile la compatibilità con la vita. Due fattori in particolare si segnalano come assolutamente unici e tali da suffragare tale valutazione clinica ad un primo esame: A) la completa distruzione del terzo medio della faccia ivi comprese le orbite; B) la caduta dei valori della crasi ematica ad 1.000.000 di GR e valori di Hb di 2-3 gr/dl. Il caso si rivela interessante anche per la insorgenza nei tessuti molli della faccia già devastati dalla malattia, di un esteso epitelioma basocellulare da correlare evidentemente al lungo periodo di deficit immunitario acquisito dalla paziente

    Gli AA. esaminano le possibilità dell’imaging attuale per la definizione diagnostica di forme estese e deostruenti del Tumore Gigantocellulare Centrale (T.G.C.) del massiccio facciale. Gli studi TC ed RM vengono confrontati con la Rx tradizionale. Dall’analisi eseguita si evince, oltre alla nota superiorità semeiologica della TC nella evidenziazione delle usure ossee, la capacità di seguire con la RM la evoluzione della neoplasia nei suoi rapporti con le strutture molli fasciali, muscolari, adipose e fibromucose, in particolare dopo aver superato la barriera ossea dello scheletro maxillo-facciale, ivi compreso il piano palatale. Lo “staging” del tumore ad aggressività locale viene, con lo studio integrato dell’imaging fornito da tali mezzi, adeguatamente definito, con conseguenti significativi vantaggi per il trattamento chirurgico demolitivo e ricostruttivo

    Pain relief and Electromyographic evaluation in patients suffering from pain dysfunction syndrome of the temporomandibular joint

    Aims: To evaluate the effectiveness of the analgesic response to two electric physiotherapy techniques: electroacupuncture (E.A.) and transcutaneus electrical nerve stimulation (T.E.N.S.), in patients suffering from the pain dysfunction syndrome. Moreover, we checked the effects of E.A. and T.E.N.S. on the masseter and temporalis anterior muscles of the same patients. Method: Fourteen patients, (8 females and 6 males, aged 18-51), suffering from TMJ pain dysfunction syndrome were divided into two groups and treated 7 with E.A. and 7 with T.E.N.S.; we used continous waves at 10-50 Hz for each 20 minutes treatment; the Scott-Huskisson Visual Analogue Scale (V.A.S.) was used to assess the intensity of pain both before and after the treatments. We have investigated the changes in activity of two masticatory muscles, the masseter and temporalis anterior, using superficial electromyography. Results: Our results show that, in relation to the changes in the masseter and temporalis muscle activity, both E.A. and T.E.N.S. have a good affect on the balance of the muscle; meanwhile, using the Signed Ranked Test, the E.A. is more effective than T.E.N.S. in relieving the pain, with a significant statistical difference (p=0.03). Conclusions: The results indicate that E.A. and T.E.N.S. are effective physiotherapy techniques and suggest their use to relieve pain and to balance masticatory muscles in patients suffering from T.M.J. pain dysfunction syndrome

    Ruolo della RM ai fini della stadiazione e del trattamento del mixoma esteso del mascellare superiore

    Vengono esaminate le possibilità diagnostiche della RM nello studio del mixoma odontogeno ad estesa diffusione nel mascellare superiore. Il massiciio facciale, per la sua complessità e per gli intimi rapporti che contrae con le strutture molli circostanti, rende indaginose e non sufficientemente adeguate le metodiche d’imaging tradizionale a causa dell’aggressività locale della neoformazione che, quando è particolarmente voluminosa, invade diffusamente le componenti cavitarie, ne usura le pareti a struttura laminare, si insinua fra i tessuti molli delle regioni fugali, pterigo-mascellari e sottorbitarie, occupando anfratti in cui sfugge alla diagnostica clinica classica ed a quella strumentale convenzionale. Gli AA. hanno verificato l’utilità dell’impiego della RM per la definizione dei limiti della neoformazione, che rende la chirurgia più consona alle esigenze di un trattamento oncologico mirato alla radicalità, da una parte, ed alla conservazione di strutture ad alta complessità morfologica e funzionale, dall’altra

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed.     Variations on the Author

    “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

    Appropriate Similarity Measures for Author Cocitation Analysis

    We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.     Dispelling the Myths Behind First-author Citation Counts

    We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more sophisticated methods
