    Objective: To compare the safety profile and clinical outcomes of Altieri-modified Nesbit corporoplasty using two different anesthesia methods including spinal anesthesia and local anesthesia. Materials and methods: A total of 40 patients with congenital penile curvature (CPC) and Peyronie’s disease (PD) underwent Altieri-modified Nesbit corporoplasty. Group 1 (n = 20) received spinal anesthesia, and Group 2 (n = 20) received local anesthesia. The patients were categorized into age groups (< 30 years, 31-45 years, 46-60 years, and 61-75 years) for analysis. Clinical outcomes, post-operative complications, hospital stay, pain levels, and other parameters were assessed. Results: The results showed that 2 patients (10%) reported postoperative complications, including headache, arterial hypotension, and penile foreskin necrosis from the spinal anesthesia group. Whereas, in the local anesthesia group, 1 patient (5%) reported scar phimosis. Further, post-operative pain was predominantly very mild, with 16 patients (80%) in the spinal anesthesia group and 14 patients (70%) in the local anesthesia group experiencing minimal discomfort. Age did not significantly impact hospital stay, post-operative intestinal disorders, constipation, lower urinary tract symptoms, voiding burning, or dysuria. Conclusions: Spinal anesthesia may lead to more post-operative complications and longer hospital stays compared to local anesthesia. Age does not significantly affect most clinical outcomes but can influence post-operative pain in patients receiving local anesthesia

    NoMAS: A Computational Approach to Find Mutated Subnetworks Associated With Survival in Genome-Wide Cancer Studies

    Next-generation sequencing technologies allow to measure somatic mutations in a large number of patients from the same cancer type: one of the main goals in their analysis is the identification of mutations associated with clinical parameters. The identification of such relationships is hindered by extensive genetic heterogeneity in tumors, with different genes mutated in different patients, due, in part, to the fact that genes and mutations act in the context of pathways: it is therefore crucial to study mutations in the context of interactions among genes. In this work we study the problem of identifying subnetworks of a large gene-gene interaction network with mutations associated with survival time. We formally define the associated computational problem by using a score for subnetworks based on the log-rank statistical test to compare the survival of two given populations. We propose a novel approach, based on a new algorithm, called Network of Mutations Associated with Survival (NoMAS) to find subnetworks of a large interaction network whose mutations are associated with survival time. NoMAS is based on the color-coding technique, that has been previously employed in other applications to find the highest scoring subnetwork with high probability when the subnetwork score is additive. In our case the score is not additive, so our algorithm cannot identify the optimal solution with the same guarantees associated to additive scores. Nonetheless, we prove that, under a reasonable model for mutations in cancer, NoMAS identifies the optimal solution with high probability. We also design a holdout approach to identify subnetworks significantly associated with survival time. We test NoMAS on simulated and cancer data, comparing it to approaches based on single gene tests and to various greedy approaches. We show that our method does indeed find the optimal solution and performs better than the other approaches. Moreover, on three cancer datasets our method identifies subnetworks with significant association to survival when none of the genes has significant association with survival when considered in isolation

    Nuclear matrix localization of annexin V in chicken liver

    Fractionation of internal matrix proteins from chicken liver nuclei led to the isolation of a 32 kDa protein which was identified by partial aminoacid sequence and immunological analysis as annexin V, an unreported nuclear matrix component. Our results showed that this protein is preferentially associated with the internal nuclear matrix fraction, since this is the only nuclear fraction where the protein can be immunodetected. Immunostaining on cultured cells also revealed a nuclear distribution with the exclusion of the nucleolar compartment and an association with cytosolic filamentous structures most likely corresponding to the cytoskeleton. Moreover, immunostaining on extracted cells to reveal the nuclear matrix showed a network-like distribution. Since annexin V has been reported as an inhibitor of protein kinase C, its nuclear localization in association with the internal matrix, which plays an important role in several nuclear processes, indicates its involvement in the regulation of signal transduction.     The debate on citizenship in health care is nowadays really important, not only in Italy. The critical reflections connected to the new practices of citizenship are increasing, as stated by Giovanni Moro in the Editorial. The article by Joao Arriscado Nunes is an evidence for that, especially because the experiences he mentions (the mobilization for the recognition of health rights and not only for general rights) are spread all over the world. It is not by chance, that the issue of articles proposed in this volume is strictly connected to health participation. The recognition of rights is usually the result of social movements often participated by citizens and specialists' associations involved in healthcare delivery. There are also other issues connected with the conceptual couple citizenship-participation empowerment, deliberative democracy, citizen's activism, third sector's role. But who is the "citizen"? Is s/he only a customer? What are the relationship between him/her, in terms of participation with the other healthcare professionals and health managers? The volume shows the complexity of these questions and goes deeper in the different roles played by the citizen: the care-giver/citizen, the activist/citizen, the expert/citizen.
