Anterior Segment Optical Coherence Tomography of Long-Term Phakic Angle-Supported Intraocular Lenses.
PURPOSE: To evaluate the long-term age-related anterior
chamber changes by anterior segment optical coherence
tomography (OCT) and the impact of such
changes on the stability and behavior of angle-supported
phakic intraocular lenses (IOLs).
DESIGN: A retrospective consecutive observational
cohort study.
METHODS: A total of 23 eyes with high myopia
implanted with an angle-supported phakic IOL between
1990 and 1996 at Vissum Corporacio ́n Oftalmolo ́gica
de Alicante were included in the study. Patients were
evaluated using OCT. Anterior chamber depth, anterior
chamber width, crystalline lens rise, and the distance
between phakic IOL and endothelium were measured.
RESULTS: Fifteen years after implantation, anterior
chamber depth was 2.9 ± 0.3 mm, crystalline lens rise
748.18 ± 393.13 mm, and phakic IOL–endothelium
distance 2.1 ± 0.30 mm. The predictive model showed

Follow-up Study of More Than 15 Years of an Angle-Supported Phakic Intraocular Lens Model (ZB5M) for High Myopia: Outcomes and Complications.
IMPORTANCE:
This study confirmed the long-term outcomes of the ZB5M phakic intraocular lens (PIOL).
OBJECTIVE:
To evaluate the potential long-term risk associated with ZB5M PIOL implantation.
DESIGN, SETTING, AND PARTICIPANTS:
A retrospective, nonrandomized, consecutive cohort study of a total of 208 eyes implanted with ZB5M PIOLs at VISSUM Corporación Oftalmológica de Alicante between 1990 and 1996 identified through surgical records (208 eyes of 123 patients); 50 of these patients (97 eyes) had available records and follow-up.
INTERVENTION:
ZB5M PIOL implantation.
MAIN OUTCOMES AND MEASURES:
To evaluate the incidence and onset of each complication during the follow-up and risk factors and causes for loss of visual acuity.

Laser in situ keratomileusis for L6.00 to L18.00 diopters of myopia and up to L5.00 diopters of astigmatism: 15-year follow-up
PURPOSE: To evaluate the long-term outcomes of laser in situ keratomileusis (LASIK) for high
myopia with or without astigmatism.
SETTING: Vissum Instituto Oftalmologico de Alicante and Miguel Hernandez University, Alicante,
Spain.
DESIGN: Retrospective–prospective case series.
METHODS: Laser in situ keratomileusis was performed using the Visx 20/20 excimer laser. The
minimum follow-up was 15 years. The main outcome measures were uncorrected (UDVA) and
corrected (CDVA) distance visual acuities, manifest refraction, and corneal topography.
RESULTS: This study included 40 patients (40 eyes) with a mean age of 51.08 years G 6.67 (SD)
(range 41 to 60 years) with high myopia ( 6.00 to 18.00 diopters [D]). At 15 years, the safety index
was 1.23 and the efficacy index, 0.95. During the follow-up, a significant increase in the dioptric
power of all keratometric variables was detected (P%.028, Friedman test), the most notable
increase occurring between 3 months and 1 year (P%.005). At 15 years
Corneal cross linking and infectious keratitis: a systematic review with a meta-analysis of reported cases.
Background: Collagen cross linking (CXL) of the cornea has been developed recently as a new treatment for
multidrug-resistant infectious keratitis. The aim of this study is to summarize the previously published data and
evaluate the effectiveness of this treatment.
Results: The search identified 12 articles. The number of eyes was 104. The infectious keratitis was associated with
bacteria in 58 eyes (57%): Gram-positive bacteria in 44 (43%; 4 of which were infected with Mycobacterium (3.6%))
and Gram-negative bacteria in 14 eyes (13%), fungus in 13 eyes (12%), and Acanthamoeba in 7 eyes (7%). In 26 eyes
(25%), the microbiological culture was negative or not performed. The mean time of re-epithelization after CXL was
20.7 ± 28.1 days (minimum of 3, maximum of 145). Sixteen eyes underwent deep or lamellar keratoplasty. The
pooled analysis suggested that CXL has a favorable effect on the block of corneal melting in 85% (95%; CI 0.77,
0.91) of eyes.

Abstract
PURPOSE:
To measure multiple cytokine and chemokine production in tears of myopic patients before and after laser in situ keratomileusis (LASIK) and in human corneal fibroblast (HCF) cultures before and after excimer laser treatment.
SETTING:
Department of Neuroscience, Ophthalmology Unit, University of Padua, Italy and Vissum-Instituto de Oftalmológico de Alicante, Alicante, Spain
METHODS:
Tear samples were obtained from 15 myopic patients before LASIK and 1 and 24 hours after LASIK. Quiescent HCF cultures were treated using the same laser energy. Culture medium was collected before treatment and after 1 and 24 hours. Cytokine concentrations were determined using multiplexed bead analysis.
RESULTS:
Compared with baseline values, interleukin (IL)-12 tear levels were significantly increased 1 hour after surgery and eotaxin levels were significantly increased at 24 hours (both P<.05). Culture medium of HCF contained high levels of IL-6, IL-8, and monocyte chemotactic protein (MCP)-1 and low levels of IL-1, eotaxin, and regulated on activation, normal T expressed, and secreted (RANTES) cytokine. One hour after treatment, levels of all cytokines were significantly reduced. At 24 hours, IL-1, IL-6, IL-8, and MCP-1 levels were significantly increased compared with values at baseline and at 1 hour while RANTES cytokine and eotaxin levels had returned to baseline levels.
CONCLUSIONS:
In vivo and in vitro studies showed that after excimer laser treatment, cytokines are released to modulate the wound-healing process; however, they can potentially induce inflammation. However, these types of in vitro studies, although useful for evaluating changes in cytokine profiles before and after treatment, only partially reproduce in vivo corneal behavior
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Identifying observational studies of surgical interventions in MEDLINE and EMBASE
Background: Health technology as sessments of surgical interventions frequently require the inclusion of non-randomised evidence. Literature search strategies employed to identify this evidence often exclude a methodological component because of uncertainty surrounding the use of appropriate search terms. This can result in the retrieval of a large number of irrelevant records. Methodological filters would help to minimise this, making literature searching more efficient. Methods: An objective approach was employed to develop MEDLINE and EMBASE filters, using a reference standard derived from screening the results of an electronic literature search that contained only subject-related terms. Candidate terms for MEDLINE (N = 37) and EMBASE (N =35) were derived from examination of the records of the reference standard. The filters were validated on two sets of studies that had been included in previous health technology assessments. Results: The final filters were highly sensitive (MEDLINE 99.5%, EMBASE 100%, MEDLINE/EMBASE combined 100%) with precision ranging between 16.7% – 21.1%, specificity 35.3% – 43.5%, and a reduction in retrievals of over 30%. Against the validation standards, the individual filtersretrieved 85.2% – 100% of records. In combination, however, the MEDLINE and EMBASE filters retrieved 100% against both validation standards with a reduction in retrieved records of 28.4% and 30.1% Conclusion: The MEDLINE and EMBASE filters were highly sensitive and substantially reduced the number of records retrieved, indicating that they are useful tools for efficient literature searching.Peer reviewe
Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films.
