    Development of a Clinical Rounds Module Under the CAISIS Platform

    Internship report - Summer 2012This paper presents and documents tasks and research performed during the author’s Master of Health Informatics internship at the Diagnostic Radiology Department in collaboration with the Nova Scotia Breast Screening Program (NSBSP) in 2012. The NSBSP aims to reduce female morbidity and mortality from breast cancer. To help accomplish this goal, the NSBSP has adopted a new open-source application to enhance the capturing, storing, and accessing of data. The main objective of this internship was to help the NSBSP facilitate the case review process (Mammography Round) by creating a Round module on a CAISIS platform. Mammography Rounds are conducted regularly to ensure the integrity of the diagnosis prior to treatment. The main objective of the research was achieved within the set schedule. The author was also assigned to provide an appropriate solution to exchange pathology data between CAISIS and CERNER MILLENNIUM. The author proposed using HL7 and DICOM to facilitate data exchange between CAISIS and the Legacy systems, but the solution was not accepted due to limited funding. The author then proposed a second solution using JAVA, which was accepted, and hence sending pathology data from CAISIS to CERNER MILLENNIUM was achieved. In this paper, the author provides an in-depth analysis of the strengths, weaknesses, opportunities and threats of the proposed solution via a SWOT analysis. At the conclusion of the paper, the author provides some recommendations that could be used to improve cancer data capture and exchange. These recommendations include: ? Using HL7 to create interoperability between CAISIS and the Legacy systems. ? Improving the sharing of Pathology images by adapting the use of a PACS system. ? Creating a Pathology synoptic report to increase report quality, reduce typographic errors, and increase productivity and overall cancer data quality.     A case of atypical systemic primary carnitine deficiency in Saudi Arabia

    Systemic primary carnitine deficiency (SPCD) is an autosomal recessive inborn error of fatty acid metabolism caused by a defect in the transporter responsible for moving carnitine across plasma membrane. The clinical features of SPCD vary widely based on the age of onset and organs involved. During infancy, patients might show episodes of hypoketotic hypoglycemia, hepatomegaly, elevated transaminases, and hyperammonemia. Skeletal myopathy, elevated creatine kinase, and cardiomyopathy are the main manifestations in children with SPCD, while in adults, the disorder is usually manifested as cardiomyopathy, arrhythmias, or fatigability. Here, we report a 5-year-old boy with SPCD that presented as dilated cardiomyopathy with atypical features, such as anemia, respiratory distress, and proximal muscle weakness. This report supports considering carnitine deficiency treatment in the work-up of unexplained pediatric dilated cardiomyopathy

    Pavement Surfaces Impact on Local Temperature and Building Cooling Energy Consumption

    abstract: Pavement surface temperature is calculated using a fundamental energy balance model developed previously. It can be studied using a one-dimensional mathematical model. The input to the model is changed, to study the effect of different properties of pavement on its diurnal surface temperatures. It is observed that the pavement surface temperature has a microclimatic effect on the air temperature above it. A major increase in local air temperature is caused by heating of solid surfaces in that locality. A case study was done and correlations have been established to calculate the air temperature above a paved surface. Validation with in-situ pavement surface and air temperatures were made. Experimental measurement for the city of Phoenix shows the difference between the ambient air temperature of the city and the microclimatic air temperature above the pavement is approximately 10 degrees Fahrenheit. One mitigation strategy that has been explored is increasing the albedo of the paved surface. Although it will reduce the pavement surface temperature, leading to a reduction in air temperature close to the surface, the increased pavement albedo will also result in greater reflected solar radiation directed towards the building, thus increasing the building solar load. The first effect will imply a reduction in the building energy consumption, while the second effect will imply an increase in the building energy consumption. Simulation is done using the EnergyPlus tool, to find the microclimatic effect of pavement on the building energy performance. The results indicate the cooling energy savings of an office building for different types of pavements can be variable as much as 30%.Dissertation/ThesisMasters Thesis Mechanical Engineering 201

    Kabuki Syndrome with Chiari malformation type II: A case report

    Kabuki syndrome is a rare genetic disorder, characterized by typical facial features, hypotonia, developmental delay and intellectual disabilities. We report here a Saudi female infant diagnosed as a case of Kabuki syndrome clinically and confirmed by molecular genetic testing. She was admitted at birth to neonatal ICU due to hydrocephalus and meningomyelocele and found to have Chiari malformation type II on radiological evaluation of the brain. Whole exome sequencing (WES) was sent for her and showed pathogenic variant in KDM6A which confirm the diagnosis of Kabuki syndrome
