Operating room efficiency improving through data management
No full textData recording has a long tradition in healthcare (Murphy, 2012; O'Connor/Neumann, 2006). Due to evidencebased medicine influences and opportunities provided by modern information technology the number and overall importance of clinical data measurement has increased (Walshe/Rundall, 2001). Unfortunately the huge amount of collected data has not always high quality standards, neither their analysis is based on scientific results (Wager/Wickham Lee/Glaser 2005). Operating rooms (OR) are critical hospital units not only in patient safety terms but also in expenditure terms (Guerriero,/Guido, 2011; Maryamaa/Kirvela 2007). There is the urgent need for new tools which sustain Hospitals decision makers work (Agnoletti/Buccioli/Padovani et al., 2013). The primary goal of the ORMS (Operating Room Data Management) project was to provide the OR decision makers with an information basis to increase OR efficiency and patient safety. Data analysis is based on scientific literature (Macario, 2006; Williams/DeRiso/Figallo et al., 1998; McIntosh/Dexter/Epstein, 2006; Dexter/Dexter/Masursky et al., 2009; Wachtel/Dexter, 2009; Dexter/Epstein/Marcon 2005; Wachtel/Dexter 2009) and the project teams' experience with tracked data. The system login is layered and different users have access to different data outputs depending on their professional needs (Manager, Anesthesiologist and Surgeon). Every profile includes subcategories where operators can access more detailed data analyses. The total number of surgical procedures has increased from 4892 in 2009 to 5616 in 2010 and decreased to 5120 in 2011. In 2012 there is to state a small increase to 5180 surgical procedures. Due to the introduction of ORMS managers optimized significantly performance indicators of the surgical theater over the years and enabled in this way to find an output number which is in line with given resources. © Springer International Publishing Switzerland 2014

A number of different options are widely used, alone or in combination, in order to minimize the pain related to surgery. Moreover, interest in local anesthetics in the treatment of some painful degenerative conditions such as myofascial trigger point pain, shoulder impingement syndrome, or patellar tendinopathy is increasing. Numerous clinical trials have shown that lidocaine–tetracaine combination, recently approved for adults aged 18 or older, is effective and safe in managing pain. The present paper gives an overview of the recent literature regarding the efficacy and safety of lidocaine–tetracaine combination use. Percutaneous coronary injection of bone marrow cells in small experimental animals: Small is not too small
No full textIntracoronary infusion of bone marrow cells (BMCs) is thought to induce cardiac regeneration in ischemic heart
disease and dilated cardiomyopathy. The aim of our study was to develop a new method to inject BMCs into coronary
arteries of small experimental animals.
Transient atrioventricular block (AVB) was induced in 25 rats and 39 hamsters by intracarotid injection of
adenosine 50-triphosphate (ATP). Contrast echocardiography was obtained. BMCs (0.2–0.5 ml) were collected through
femoral puncture, stained with PKH26 and injected into the carotid artery (CA). Animals were immediately sacrificed
or followed for 1 month. To evaluate BMCs transfer from CA to myocardium, AVB and BMCs injections were
performed in 10 hamsters subjected to coronary ligation for 30 min.
Induction of transient AVB was possible in all animals by injecting 20–30 mg of ATP. Animals recovered a basal
cardiac activity spontaneously or by dopamine injection. Flash injection of contrast medium through the CA induced
staining of aortic root, coronary arteries, and myocardium. BMCs injection was possible in all cases. No immediate or
late ECG changes were observed. Immediately after injection in healthy animals, histological examination showed the
presence of BMCs in small coronary arteries and, after 1 month, the absence of infarction. In ischemic hearts, the
presence of BMCs in the myocardium was observed 24 h after ischemia.
ATP-induced AVB block allows for percutaneous intracoronary injection of BMCs in small experimental animals
with no immediate or late mortality and morbidity. This method offers new perspectives for the investigation of BMCs
coronary infusion and engraftment in heart diseases
Biotype and susceptibility to antimicrobial agents of rabbit Escherichia coli
No full textNon-invasive Enteropathogenic Escherichia coli (EPEC) represents one of the most
frequent pathogens involved in fatal enteropathy of commercial ra bbits in Italy. We here
report the characterisation of the biotype and the resistance to antimicrobial agents of 60
strains of E. coli isolated from 150 rabbits affected by diarrhoeic enteropathy.
Parasitological, bacterioscopical, bacteriological and histological examinations were
performed on caecal samples. Biotyping was made according to CAMGUILHEM and MILON
(1989), and the antibiotic resistance against 18 different antibiotics was evaluated using
the Kirby-Bauer method. All the strains were isolated from caeca that showed catharral
to haemorrhagic entero-tiflitis and, microscopically, attachment/effacement (A/E) lesions.
Twelve different biotypes were identified, among which B12 and B14 biotypes were
constantly present. Antibiotic resistance patterns varied considerably among the
biotypes and within the same biotype among the rabbitries. Only fluoroquinolones
resulted highly efficacious against E. coli. The results suggest to adopt quarantine
measures for restocking rabbits in order to identify potential reservoirs of EPEC as well
as to determine the prevalent biotype/biotypes and its/their susceptibility to antimicrobial
agents
