How to minimize low enrolling sites: A case study in diabetes
India is becoming one of the fast evolving destinations for conducting global clinical trials. This case study shows that, as expected, Indian sites have a higher subject recruitment rate than the global average there. However, there is scope to enhance subject recruitment performance by looking at the relative performance of the Indian sites. The case study looks at high and low performing Indian sites to define the attributes associated with performance. Clinical trial insight has been used to develop a series of steps to help facilitate in identification of sites and improve patient recruitment process. Tips are provided on how the CRA can review historical and competing study data, the importance of assessing the availability and interest of the PI and study team. With this knowledge, the CRA can pro actively help sites identify and resolve potential issues related to start up delays and recruitment by creating a site specific recruitment plan and lastly encourage and motivate sites to achieve the target as per the agreed plan. The importance of CRA-Investigator relationship is another critical element in achieving high recruitment performance. Analysis of these trends can serve as indicators for site performance and help to differentiate the low from high recruiting sites

Nurse education to improve use of BIS protocol for patients on neuromuscular blocking agents: a Quality initiative
Background: Bispectral index (BIS) monitoring measured sedation levels of patients on neuromuscular blockers (NMB) in the intensive care unit (ICU) setting but monitoring lacked efficiency due to nurses’ unfamiliarity with the associated BIS protocol. Sedation effectiveness was highly dependent on nurse-related factors such as knowledge, skill, experience, and confidence. Objective: The objective of this quality improvement project was to improve ICU nurses’ level of knowledge and perception on BIS monitoring and increase the use of BIS protocol for management of patients receiving NMB agents. Methods: Educational intervention included a pretest and posttest method of evaluation. The project included theoretical and clinical sessions on NMB agents, BIS monitoring, and BIS sedation protocol. Effects were measured using the Knowledge Questionnaire (KQ) at baseline and four months after the intervention. BIS sedation protocol usage was assessed monthly from 1/1/2021 - 4/28/21. Results: N= 52 ICU nurses. Project was conducted in a large metropolitan hospital in Texas. KQ’s post-educational intervention demonstrated improvements (Mdn = 25, Range = 13[14-27]) which were higher than pre-educational levels (Mdn = 19.5, Range = 6 [21-27]; t= 16.954, p <. 001), Cohn’s d 2.35). The mean scores on BIS perceptions were higher post-education, between 4 to 6 on all five subscales. Protocol usage in January was 33.3%, February was 62.5%, March was 54.5 %, and April was 66.6 %. Conclusion: This educational intervention demonstrated effectiveness in improving overall ICU nurses’ knowledge levels. While not at 100%, this protocol demonstrated improvements over the four-month period. Continued education and newer strategies need to be adopted to achieve 100% usage.Nursing PracticeCollege of Nursing and Health Science

Primary Spinal Epidural Diffuse Large B-cell Lymphoma: Case Report and Literature Review
Primary spinal epidural lymphoma (PSEL) comprises a group of tumors present only in the spinal epidural space with a histopathological picture of lymphoma and negative diagnostic workup for lymphoma at other sites. We present the case of an older male adult with primary spinal diffuse large B-cell lymphoma (DLBCL) presenting with spinal cord compression who was treated with surgery followed by high dose methotrexate in combination with RCHOP (rituximab, cyclophosphamide, prednisone, vincristine, and doxorubicin). This case report and review of literature on DLBCL limited to the spine provide a novel chemotherapy regimen and a comprehensive perspective on the optimal management of these patients
Role of fine needle aspiration cytology (FNAC) of testes in male infertility
Fine needle aspiration of the testis, which is minimally traumatic, has questioned the need of a more invasive open biopsy in the evaluation of male infertility. This study aims to evaluate the efficacy of fine needle aspiration cytology (FNAC) as compared to open testicular biopsy in the evalu-ation of male infertility by correlating diagnoses from tes-tis FNA cytology with biopsy histology. We have also studied the necessity of bilateral or unilateral FNACs in the workup of the infertile man. This study was a prospec-tive study of 57 infertile men in whom bilateral testicular fine needle aspiration as well as testicular biopsy was done. The correlation between the 2 methods was 91.9%. Dis-crepancies between cytology and histology were mainly the result of insufficient smears. These data also suggest that bilateral FNACs and biopsies can be restricted to patients in whom there is appreciable difference in tes-ticular size or consistency
Nurse education to improve bispectral-index-monitoring for patients on neuromuscular blocking
Will an educational module on NMB agents and application of BIS monitoring protocol among ICU nurses with more than six months experience improve these nurses’ knowledge and perception of BIS monitoring and increase the use of the BIS monitoring protocol at the end of three months</p
