Isolated spontaneous dissection of the celiac trunk in a patient with bicuspid aortic valve

The spontaneous dissection of the visceral artery occurs without aortic dissection. The most consistent presenting symptom is acute onset abdominal pain. Complications consist of ischemia, aneurysm formation, and rupture. We report an exceptional case of an isolated spontaneous dissection of the celiac trunk which occurred in a 49 year old male with a previously undiagnosed bicuspid aortic valve (BAV). Comprehensive Assessment of Esophageal Disorders Associated with Hiatus Hernia: Insights from Big Data Analysis
Hiatus hernia (HH) is a prevalent endoscopic finding in clinical practice, frequently co-occurring with esophageal disorders, yet the prevalence and degree of association remain uncertain. We aim to investigate HH's frequency and its suspected association with esophageal disorders. We reviewed endoscopic reports of over 75,000 consecutive patients who underwent gastroscopy over 12 years in two referral centers. HH was endoscopically diagnosed. We derived data on clinical presentation and a comprehensive assessment of benign and malignant esophageal pathologies. We performed multiple regression models to identify esophageal sequela associated with HH. The overall frequency of HH was (16.8%); the majority (89.5%) had small HHs (<3 cm). Female predominance was documented in HH patients, who were significantly older than controls (61.1±16.5 vs. 52.7±20.0; P < 0.001). The outcome analysis of esophageal pathology revealed an independent association between HH, regardless of its size, and erosive reflux esophagitis (25.7% vs. 6.2%; OR = 3.8; P < 0.001) and Barrett's esophagus (3.8% vs. 0.7%; OR = 4.7, P < 0.001). Furthermore, following rigorous age and sex matching, in conjunction with additional multivariable analyses, large HHs were associated with higher rates of benign esophageal strictures (3.6% vs. 0.3%; P < 0.001), Mallory Weiss syndrome (3.6% vs. 2.1%; P = 0.01), and incidents of food impactions (0.9% vs. 0.2%; P = 0.014). In contrast, a lower rate of achalasia was noted among this cohort (0.55% vs. 0%; P = 0.046). Double-stranded RNA uptake for the control of the maize pathogen Cercospora zeina
DATA AVAILABILITY STATEMENT : The data that support the findings of this study are available from the corresponding author upon reasonable request.RNA interference (RNAi) using double-stranded RNA (dsRNA) against fungal pathogens is an emerging field of crop disease control. We aimed to evaluate RNAi against the fungus Cercospora zeina causing grey leaf spot (GLS) disease on maize. Orthologues of Dicer-like 1, Dicer-like 2, RNA-dependent RNA polymerase and two copies of Argonaute were identified in the C. zeina genome and were shown to be expressed in vitro and in planta. Confocal microscopy showed that C. zeina took up exogenously applied dsRNA labelled with fluorescein. GFP-transgenic C. zeina was treated with GFP-specific dsRNA, and GFP mRNA expression and protein fluorescence were reduced by 57% and 61%, respectively. A Cz3-dsRNA targeting C. zeina chitin synthase D (CHSD), phosphatidylserine decarboxylase proenzyme 3 (PSD3) and extracellular protein 2 (ECP2) was constructed. Treatment of C. zeina cultures with the Cz3-dsRNA reduced CHSD expression by 47% and reduced cell viability by 34%. Maize leaves were inoculated with C. zeina conidia, and Cz3-dsRNA was applied either with the conidia or 16 h later. GLS disease was significantly reduced compared to the water control for the 16 h post-inoculation (hpi) treatment with Cz3-dsRNA, but not for the GFP-dsRNA specificity control or treatments at 0 hpi. We hypothesized that germination of C. zeina conidia was required for effective dsRNA-mediated control, and this was borne out by microscopy observations that most of the C. zeina conidia (70%) germinated successfully on the maize leaf surface within 16 hpi.
