Conservation in an Islamic context a case study of Makkah
No full textThe Holy Qu’ rān contains many injunctions for Muslims to respect and conserve the natural environment but few address the built environment. Habitat at the time of the Prophet (PBOH) was in the vernacular and relatively impermanent. The first habitat was the cave, the second the tent and then simple flat roofed buildings of post and lintel construction made of mud and rubble. Later buildings were not indigenous but reflected the architectural styles and techniques of Muslim pilgrims from beyond the Arabian Peninsula. Permanent exotic buildings were later erected as reminders of holy places and events. This work advances a case to restore and preserve historic and religious sites in Makkah, Saudi Arabia. Makkah is the destination for millions of Muslim pilgrims who annually pay homage to Allah during the occasions of Hajj, Ramadan and Umra. The tranquillity and peaceful ambience that one associates with the holiest of Islamic experiences have, over the years, given way to jostling crowds of people who must be expediently housed, fed, transported, and protected. Due to the lack of planning and the insensitive but profitable development of the city, Makkah is in grave danger of becoming a bustling metropolis instead of a sanctuary where pilgrims gather to perform their religious rites and reaffirm their dedication to Allah. The author calls for professional planning and international cooperation to guide future development for this expanding and sensitive area. The author's ideas are grounded in practical and aesthetic study, therefore, the political, environmental and economic issues are examined in relationship to religious, historic and artistic values. The author makes proposals for a future Makkah that would provide pilgrims with the physical comforts, security, and serene environment they deserve—without destroying the city they came to visit. The author discusses preservation and conservation in the western world and the need for their acceptance in Muslim countries, the former being an aesthetic and intellectual concept sustained by law and the latter being the prescribed free expression of the individual unhindered by material considerations. Both worlds are rapidly being overwhelmed by materialism, but body, mind and spirit combine in making us aware of our surroundings and the way in what we see around us has come into being

No full textIdentified in 2012, the Middle East respiratory syndrome coronavirus (MERS-CoV) causes severe and often fatal acute respiratory illness in humans. No approved prophylactic or therapeutic interventions are currently available. In this study, we developed chicken egg yolk antibodies (IgY Abs) specific to the MERS-CoV spike (S) protein and evaluated their neutralizing efficiency against MERS-CoV infection. S-specific IgY Abs were produced by injecting chickens with the purified recombinant S protein of MERS-CoV at a high titer (4.4 mg/mL per egg yolk) at week 7 post immunization. Western blotting and immune-dot blot assays demonstrated specific binding to the MERS-CoV S protein. In vitro neutralization of the generated IgY Abs against MERS-CoV was evaluated and showed a 50% neutralizing concentration of 51.42 μg/mL. In vivo testing using a human-transgenic mouse model showed a reduction of viral antigen positive cells in treated mice, compared to the adjuvant-only controls. Moreover, the lung cells of the treated mice showed significantly reduced inflammation, compared to the controls. Our results show efficient neutralization of MERS-CoV infection both in vitro and in vivo using S-specific IgY Abs. Clinical trials are needed to evaluate the efficiency of the IgY Abs in camels and humans
Anti-S1 MERS-COV IgY Specific Antibodies Decreases Lung Inflammation and Viral Antigen Positive Cells in the Human Transgenic Mouse Model
No full textThe Middle East respiratory syndrome coronavirus (MERS-CoV) was identified in 2012 and causes severe and often fatal acute respiratory illness in humans. No approved prophylactic and therapeutic interventions are currently available. In this study, we have developed egg yolk antibodies (immunoglobulin Y (IgY)) specific for MERS-CoV spike protein (S1) in order to evaluate their neutralizing efficiency against MERS-CoV infection. S1-specific immunoglobulins were produced by injecting chickens with purified recombinant S1 protein of MERS-CoV at a high titer (5.7 mg/mL egg yolk) at week 7 post immunization. Western blotting and immune-dot blot assays demonstrated that the IgY antibody specifically bound to the MERS-CoV S1 protein. Anti-S1 antibodies were also able to recognize MERS-COV inside cells, as demonstrated by an immunofluorescence assay. Plaque reduction and microneutralization assays showed the neutralization of MERS-COV in Vero cells by anti-S1 IgY antibodies and non-significantly reduced virus titers in the lungs of MERS-CoV-infected mice during early infection, with a nonsignificant decrease in weight loss. However, a statistically significant (p = 0.0196) quantitative reduction in viral antigen expression and marked reduction in inflammation were observed in lung tissue. Collectively, our data suggest that the anti-MERS-CoV S1 IgY could serve as a potential candidate for the passive treatment of MERS-CoV infection

2017 update of the WSES guidelines for emergency repair of complicated abdominal wall hernias
No full textEmergency repair of complicated abdominal wall hernias may be associated with worsen outcome and a significant rate of postoperative complications. There is no consensus on management of complicated abdominal hernias. The main matter of debate is about the use of mesh in case of intestinal resection and the type of mesh to be used. Wound infection is the most common complication encountered and represents an immense burden especially in the presence of a mesh. The recurrence rate is an important topic that influences the final outcome. A World Society of Emergency Surgery (WSES) Consensus Conference was held in Bergamo in July 2013 with the aim to define recommendations for emergency repair of abdominal wall hernias in adults. This document represents the executive summary of the consensus conference approved by a WSES expert panel. In 2016, the guidelines have been revised and updated according to the most recent available literature.Peer reviewe
