Is duodenojejunal anastomosis to the left of the superior mesenteric vessels a feasible option for tumors of the angle of Treitz?
Tumors of the angle of Treitz are a rare entity. Only 3%-5% of gastrointestinal stromal tumors (GISTs) occur at the level of the duodenum, and their location at the duodenojejunal junction is very uncommon. Surgery is the treatment of choice, while adjuvant medical therapy is used on the basis of the degree of radicality of the excision and the tumor's proliferative profile. These factors primarily influence the prognosis. Due to the frailty of the vascular viability of the left duodenum, which can be injured during surgery, it is generally recommended to perform digestive reconstruction at the level of the right portion of the duodenum. We here report the case of a patient with a large GIST located at the duodenojejunal junction behind the ligament of Treitz. We found reconstructive digestive anastomosis at the level of the third part of the duodenum to be a safe procedure
Role of Preoperative Biliary Drainage in Resectable Hilar Cholangiocarcinoma
Background: To review the literature to investigate indications, advantages and complications of the different procedures for biliary drainage
(percutaneous or endoscopic techniques) for resectable hilar cholangiocarcinoma.
Methods: Pubmed and Medline databases were interrogated for articles published between January 1970 and November 2014. After
screening for relevance, 56 articles were selected for the review.
Results: Hilar cholangiocarcinoma is the most common primitive adenocarcinoma of the bile ducts, involving the proximal extra hepatic
biliary system. Prognosis has been strongly related to tumour’s resectability and extended combined hepatic and biliary resections are often
required in order to achieve radical margin and survival benefit. Obstructive jaundice is the most common clinical presentation; preoperative
hyperbilirubinemia is an independent risk factor for increased operative morbidity and mortality.
Conclusion: PBD was shown to be helpful only in those patients having a bilirubin level greater than 3-10 mg/dl; we might conclude that
PBD is probably useless with bilirubin level below 3 mg/dl, advisable between 3-10 mg/dl and mandatory above 10 mg/dl especially in patients
undergoing an extended hepatic resection.
In the initial management of HC, ENBD should be preferred as it is characterized by a less invasive approach and less complications (lower
morbidity) . In case of suboptimal jaundice regression or in presence of complications PTBD might be combined, leaving EBD as last option.
Drain should be limited to FLR. The optimal drain duration is debated: the drain should be left in place the time needed to reduce bilirubin level,
meanly 2-6 weeks

Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
Pyogenic vertebral osteomyelitis complicating abdominal penetrating injury : case report and review of the literature
Pyogenic vertebral osteomyelitis is a rare condition usually associated with endocarditis or spinal surgery. However, it may also occur following abdominal penetrating trauma with associated gastrointestinal perforation. Diagnosis might be challenging and appropriate treatment is essential to ensure a positive outcome. In trans-abdominal trauma, 48 hours of broad-spectrum antibiotics is generally recommended for prophylaxis of secondary infections. A case report of vertebral osteomyelitis complicating trans-colonic injury to the retroperitoneum is presented and clinical management is discussed in the light of literature review
Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.
