    Decision-Stage Method: Convergence Proof, Special Application, and Computation Experience

    This paper presents a new method for obtaining exact optimal solutions for a class of discrete-variable non-linear resource-allocation problems. The new method is called the decision-state method because, unlike the conventional dynamic programming method which works only in the state space, the new method works in the state space and the decision space. It generates and retains only a fraction of the points in the state space at which the state functions are discontinuous; and thus overcomes to some extent the curse of dimensionality. It carries the cumulative decision-strongs associated with these points, and thus avoids the backtracking entailed by the conventional dynamic programming method for recovering the optimal decisions. A concise and complete statement of the method is given in Algorithm 2 and it is proved that the algorithm finds all exact optimal solutions. In addition the method is adapted for solving some problems with special structures such as block-angular or split-block-angular constraints and the resultant substantial advantages are demonstrated. The performance of Algorithm 2 on many resource-allocations problems is reported, along with investigations on many tactical decisions which have substantial impact on the performance. The performance of the computer implementation of Algorithm 2 is compared with that of the MMDP algorithm and it showed that for the class of problems at which the two are aimed, the decision-state Algorithm 2 performed better than MMDP algorithm both in terms of storage requirement and solution time. In fact, it achieved an order of magnitude saving in storage requirement.

    Progressive decrease of outflow gradient and septum thickness after percutaneous alcoholization of the interventricular septum in hypertrophic obstructive cardiomyopathy

    Background: The aim of this study was to evaluate the immediate- and mid-term results of percutaneous transluminal septal myocardial ablation (PTSMA) of the interventricular septum performed in 15 consecutive patients with hypertrophic obstructive cardiomyopathy between 1996 and 1999. Methods: Prior to intervention, all patients (7 males, 8 females, mean age 62 ± 11 years) complained of severe dyspnea (NYHA functional class III-IV) despite medical treatment with β-blockers and/or verapamil. Family history of hypertrophic cardiomyopathy was present in 2 cases. Dehydrated alcohol (4.8 ± 1.5 ml/pt) was selectively infused into the first septal perforator artery through over-the-wire balloon catheters. In 5 patients a second or a third septal branch was treated because the intraventricular gradient persisted above 50 mmHg after the initial alcohol infusion. Results: Alcohol infusion induced an average peak creatine phosphokinase level of 1524 ± 427 IU/l. No iterating ventricular arrhythmias occurred during the procedure or in the 2-3 days of continuous ECG monitoring after the procedure. Two patients (13%) developed a complete atrioventricular block after the procedure, requiring permanent double-chamber pacing. Electrocardiographic changes included a > 2 mm ST segment elevation and transient right bundle branch block or left anterior/left posterior hemiblock in all patients. Peak basal intraventricular gradient decreased from 80 ± 27 to 24 ± 27 mmHg (p < 0.01) during cardiac catheterization and from 81 ± 27 to 35 ± 25 mmHg (p < 0.01) at the echocardiograpic control performed during the hospital stay. At follow-up (mean 5.1 ± 3.6 months), all patients were in NYHA functional class I or II. Repeat echocardiography showed a further significant decrease in intraventricular gradient to 25 ± 26 mmHg (p < 0.01) and a progressive decrease in intraventricular septum thickness (25 ± 5 mm before treatment, 21 ± 6 mm before hospital discharge, 17 ± 3 mm at follow-up, p < 0.01). Conclusions: PTSMA of the intraventricular septum effectively relieves symptoms in selected patients with hypertrophic obstructive cardiomyopathy. The immediate decrease in intraventricular gradient is followed by a further decline at follow-up with a progressive reduction in the intraventricular septum thickness

    The transcription factor PDX1 plays a critical role during β-cell development and in glucose-induced insulin gene transcription in adult β-cells. Acute glucose exposure leads to translocalization of PDX1 to the nucleoplasm, whereas under conditions of oxidative stress, PDX1 shuttles from the nucleus to the cytosol. Here we show that cytosolic PDX1 expression correlated with β-cell failure in diabetes. In isolated islets from patients with type 2 diabetes and from diabetic mice, we found opposite regulation of insulin and PDX1 mRNA; insulin was decreased in diabetes, but PDX1 was increased. This suggests that elevated PDX1 mRNA levels may be insufficient to regulate insulin. In diabetic islets, PDX1 protein was localized in the cytosol, whereas in non-diabetic controls, PDX1 was in the nucleus. In contrast, overexpression of either IL-1 receptor antagonist or shuttling-deficient PDX1 restored β-cell survival and function and PDX1 nuclear localization. Our results show that nuclear localization of PDX1 is essential for a functional β-cell and provides a novel mechanism of the protective effect of IL-1 receptor antagonist on β-cell survival and function

    Coronary artery bypass surgery and angioplasty provide symptomatic relief in patients with ischemic heart disease, but despite advancement in technique and devices, these methods are not applicable in a subset of patients with angina refractory to medical treatment. Bypass surgery may not be feasible because of lack of suitable conduits, diffuse coronary artery disease or poor distal run-off, and coronary angioplasty is sometimes not applicable due to chronic total occlusion, diffuse disease or extreme tortuosity. Transmyocardial laser revascularization and the stimulation of neoangiogenesis by a variety of growth factors have recently emerged as a new tool in the management of these patients. In the first part of this article, we review laser-induced direct myocardial revascularization, its indications, potential risks, and published clinical trials. The induction of neoangiogenesis using different growth factors or the genes encoding for them will be the subject of the second part of our review
