No full textA férfi inkontinencia, főleg annak súlyos és nagyon súlyos fokú formája jelentős mértékben rontja a páciensek életminőségét. A betétek és pelenkák viselése nem csak kellemetlen és kényelmetlen, de korlátozza a beteget mindennapi tevékenységének elvégzésében is. A panaszok csökkentésére, illetve megszüntetésére, valamint az életminőség javítására számos konzervatív és sebészi alternatíva áll rendelkezésünkre.
Az inkontinencia leggyakoribb oka idős korban urológiai műtétek során kialakuló iatrogén ártalom: ezek a műtétek a BPH kezelésére szolgáló TURP, illetve a prostatarák kezelésére alkalmazott RP. Mindkét műtét esetén kialakulhatnak postoperatív szövődmények, melyek közül diplomamunkámban a iatrogén inkontinencia sebészi kezelését veszem górcső alá.
Az inkontinencia súlyosságától függően kell megválasztanunk a beteg számára legjobb terápiás megoldást, ugyanis amennyiben a konzervatív terápiás alternatívák nem nyújtanak kielégítő megoldást a panaszokra, sebészi beavatkozás válik szükségessé. Ebben az esetben enyhe inkontinencia esetén a fix vagy állítható szalagokat és szalagrendszereket alkalmazzuk, középsúlyos inkontinenciánál az állítható szalagrendszerek vagy az AMS 800 művi záróizom alkalmazható, míg súlyos és nagyon súlyos inkontinencia esetén az AMS 800 művi záróizom implantáció a „gold standard” műtéti megoldás.
Az AMS 800 mellett más művi sphincterek is forgalomba kerültek, nevezetesen a FlowSecure system és a Zephyr ZSI 375. Nagyon fontos azonban megjegyezni, hogy míg az AMS 800 művi sphincter 1972 óta a piacon van, tehát több mint 40 éves tapasztalattal rendelkezünk az eszközzel és a kezelt betegek utánkövetésével kapcsolatban, addig a FlowSecure system-et 2006-ban, a Zephyr ZSI 375 művi sphinctert 2009-ben mutatták be először. Az 1972-es bevezetés óta világszerte már több, mint 94.000 férfinak ültették be az AMS 800 művi sphinctert. Ez a hatalmas betegszám, illetve a rendelkezésre álló több évtizedes tapasztalat az AMS 800 rendszer egyik nagy előnye a többi művi sphincterrel szemben. Továbbá megemlítendő, hogy bár a másik két művi sphincter az AMS 800 gyenge pontjait volt hivatott kiküszöbölni, ezek a rendszerek sem lettek mentesek a postoperatív szövődményektől. A FlowSecure system plusz „stress relief” ballonja emeli a mechanikai meghibásodás és ezzel a reoperáció esélyét, a Zephyr ZSI 375 esetén pedig a betegek a scrotumban elhelyezett túl nagy méretű szabályozó egység (pumpa és ballon egybeépítve) miatt panaszkodnak. Az újabb rendszerek hatékonyságáról, tartósságáról, illetve ami a legfontosabb a betegek elégedettségéről az elkövetkezendő évek, évtizedek tapasztalatai fognak tanúbizonyságot adni.
Összefoglalva elmondható, hogy a súlyos és nagyon súlyos fokú inkontinencia sebészi kezelésére a művi sphincterek közül az AMS 800 a bizonyítottan leghatékonyabb, „gold standard” műtéti megoldás. A DE OEC Urológiai Klinikán 2011 szeptemberétől 2015 májusáig eddig 10 alkalommal végeztek AMS 800 beültetést. A betegek postoperatív 100%-os kontinencia aránya (napi 0 vagy 1 biztonsági betét) kiválóan példázza, hogy a szakszerűen implantált és rendszeresen kontrollált AMS 800 művi sphincter a ma elérhető legjobb alternatíva a súlyos és nagyon súlyos fokú inkontinencia kezelésére.P.J.általános orvostudományinappalimagyaregyetem

Going Beyond Counting First Authors in Author Co-citation Analysis
Full text linkThe present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed

Variations on the Author
Full text link“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
