    Acaba de publicarse la nueva versión de la serie de normas UNE-EN ISO 13606, sobre comunicación de la historia clínica. Unos documentos que permiten la transferencia de información entre sistemas y que resultan de vital importancia en el panorama científico actual para servir de base a los sistemas de información sanitarios , así como a los repositorios de datos para investigación. La serie de Normas UNE-EN ISO 13606 tiene como objetivo dar soporte a la con- tinuidad asistencial, definiendo una arqui- tectura de información para comunicar la historia clínica de forma que se conserve su significado original, el contexto en que fue creada y la confidencialidad. Su ámbito de aplicación no es únicamente la transferencia de información entre sistemas con objetivos asistenciales. También puede ser muy útil en otros, como se ha visto en la situación actual de la pandemia, como la investigación o la salud pública, creando repositorios de datos de calidad provenientes de fuentes diversas.S

    OntoCR: A CEN/ISO-13606 clinical repository based on ontologies

    Objective: To design a new semantically interoperable clinical repository, based on ontologies, conforming to CEN/ISO 13606 standard. Materials and Methods: The approach followed is to extend OntoCRF, a framework for the development of clinical repositories based on ontologies. The meta-model of OntoCRF has been extended by incorporating an OWL model integrating CEN/ISO 13606, ISO 21090 and SNOMED CT structure. Results: This approach has demonstrated a complete evaluation cycle involving the creation of the meta-model in OWL format, the creation of a simple test application, and the communication of standardized extracts to another organization. Discussion: Using a CEN/ISO 13606 based system, an indefinite number of archetypes can be merged (and reused) to build new applications. Our approach, based on the use of ontologies, maintains data storage independent of content specification. With this approach, relational technology can be used for storage, maintaining extensibility capabilities. Conclusions: The present work demonstrates that it is possible to build a native CEN/ISO 13606 repository for the storage of clinical data.     Evaluation of ISO EN 13606 as a result of its implementation in XML.

    The five parts of the ISO EN 13606 standard define a means by which health-care records can be exchanged between computer systems. Starting within the European standardisation process, it has now become internationally ratified in ISO. However, ISO standards do not require that a reference implementation be provided, and in order for ISO EN 13606 to deliver the expected benefits, it must be provided not as a document, but as an operational system that is not vendor specific. This article describes the evolution of an Extensible Markup Language (XML) Schema through three iterations, each of which emphasised one particular approach to delivering an executable equivalent to the printed standard. Developing these operational versions and incorporating feedback from users of these demonstrated where implementation compromises were needed and exposed defects in the standard. These are discussed herein. They may require a future technical revision to ISO EN 13606 to resolve the issues identified

    Objetivo: Evaluar la utilidad de los Arquetipos ISO/CEN 13606 y openEHR en la representación de modelos clínicos detallados. Metodología: como editores de arquetipos se utilizaron LinkERH para ISO/CEN 13606 y los editores de Ocean Informatics y LiU para openEHR. Como caso de uso se representaron los conjuntos de datos identificados en los modelos locales de tres sistemas (hospital, UCI y atención primaria) en el dominio de la úlcera por decúbito, abarcando la observación, evaluación, instrucción y acción. Los conceptos fueron enlazados con terminologías SNOMED CT y MedDRA. Se buscaron arquetipos relacionados con el dominio en los repositorios internacionales para ser reutilizados. Resultados: Se realizó un conjunto de arquetipos equivalentes ISO/CEN 13606 y openEHR, en español e inglés. Los arquetipos proporcionan un formalismo útil para especificar datos de un modelo detallado. Los modelos producidos por las herramientas de edición de arquetipos son comprensibles para los clínicos. Los arquetipos proporcionan un marco para la implementación de las terminologías en la HCE.
